The results of 600 consecutive renal transplant operations (540 cadaver and 60 living related donor) in 491 patients were assessed with respect to urological complications. In nearly all cases the ureter was inserted into the bladder using a Leadbetter-Politano ureteroneocystostomy technique. The bladder was drained by Foley catheter for 5 days and the operation site by vacuum drain for a variable period. After 26 operations in 26 patients, 27 episodes of ureteric obstruction were identified (one reobstruction). These occurred between 1 day and 45 months after transplantation and involved the lowest third of the ureter 19 times, the middle third 3 times and the proximal third 4 times. They were due to stricture (11), pelvic collection (7), redundant or twisted ureter (3) or other cause (5). An anatomical predisposing factor in either patient or graft was identified in 31 per cent of these caes. After 18 operations in 18 patients, 21 episodes of urine leakage were identified (three re-leakages). These occurred between 1 day and 6 months after transplantation and an anatomical predisposing factor in either patient or graft was present in 59 per cent. Thus, overall, 7 per cent of patients developed a urological complication in this series. This rate has declined in recent years in parallel with a general reduction in steroid dose for immunosuppression. This last feature, combined with the surgical technique described, has resulted in a low incidence of urological complication after transplantation, being now between 1 and 2 per cent.
1 A new imidazo-pyridine hypnotic (zolpidem 10 mg and 20 mg) was compared with placebo as premedication before general anaesthesia in female patients undergoing minor gynaecological surgery. Efficacy and tolerance before and after anaesthesia were assessed. Psychomotor testing was used to study recovery from anaesthesia. 2 Both doses of zolpidem produced good sedation pre-operatively but only the higher dose was associated with anterograde amnesia. 3 Premorbid anxiety scores were low in the group of patients studied and were unaffected by either dose of zolpidem. 4 There were no significant effects on the course of anaesthesia. However, postoperatively there was a tendency for wake-up to be delayed in those patients who received either dose of zolpidem. 5 Postoperative recovery, as indicated by tests of psychomotor performance, was noticeably delayed with a dose of 20 mg compared with placebo whilst psychomotor performance had returned towards baseline levels 3 h after wake-up in those patients who had received placebo. The zolpidem 10 mg group was intermediate. 6 Zolpidem may be a suitable premedicant when hypnosis and amnesia only are required.

The characteristics of the train-of-four (TOF) response were studied electromyographically during onset and spontaneous offset of neuromuscular blockade with bolus doses of vecuronium (ED95, and ED95 X 2). During onset of blockade there was less fade with the larger than the smaller dose of vecuronium, demonstrating a variable and dose-related relationship between the ratio of height of the initial twitch, T1, and fourth twitch, T4. With both doses TOF fade was more pronounced during recovery than during onset of block, but at the same T1 values during offset, both doses were associated with similar degrees of fade during recovery. Thus with bolus doses of vecuronium the T4 ratio during recovery bears a fixed relationship to initial T1 depression that is independent of dose.
Oral temazepam was compared with papaveretum and hyoscine for pre-operative medication. The oral premedication was associated with less subjective unpleasant side-effects and greater anxiolytic properties. It was adjudged to be superior by the anaesthetic and nursing staff as well as by the patients themselves. Oral premedication was also less time-consuming to administer, and there was a small saving in the cost of drugs and equipment.

The Extended Objective Measure of Ego Identity Status (EOM-EIS) was examined for reliability and validity among early/middle adolescent subjects. The EOM-EIS represents a combination of the Objective Measure of Ego Identity Status and a structured interview of identity status. The instrument was designed to measure ego identity with regard to occupational, religious, political, philosophical, and social contexts along ideological and interpersonal dimensions. A total of 467 students aged 12 to 18 (median = 15.73) responded to the instrument. In general the results from the analyses of reliability, validity, demographic characteristics, and psychosocial maturity yielded results which parallel the theoretical framework and the results from college-aged subjects.

The prevention of pressure sores for patients with spinal cord injuries includes pressure relief through the patient shifting position and, thus, weight. We have designed a microprocessor-based device to mount on the seat of a conventional wheelchair. This device has applications in hospital rehabilitation programs for improving patient care and in research to study the cause and prevention of pressure sores. It can use audible alarms, which can be disarmed, to train patients to regularly shift weight. The device stores in its memory data on the interval between and the duration of weight shifts. The contents of this memory can be transmitted to a laboratory microcomputer system and displayed in tabular, graphical, or histogram formats. This device is more versatile than previous timer-based systems, not only because its timer and alarm parameters easily can be reprogrammed, but especially because it can store and play back detailed data on the patient's performance. The device is powered by rechargeable NiCad batteries, which can last several days without recharging.
Two patients presenting for removal of phaeochromocytoma are described. Vecuronium was used to provide neuromuscular blockade by two different methods of administration: an infusion and a large bolus dose. Both gave satisfactory results and we suggest that vecuronium may be the neuromuscular blocking agent of choice for patients with phaeochromocytoma.
