Nucleic acid metabolism disturbance in progressive systemic sclerosis (systemic scleroderma).

The content of helical forms of RNA and DNA in plasma was determined in 28 patients with progressive systemic sclerosis (PSS). Various disturbances of nucleic acid (NA) metabolism were noted in those patients: the DNA level was increased in comparison with the standard, and the RNA level was decreased. During treatment a normalization of the given indices was observed. The DNA content in plasma of PSS patients correlated with activity of the process, RNA content with the course of the disease.

[The function of the bronchial system in patients with rheumatoid arthritis].

The authors studied the functional indices of the bronchopulmonary system in 23 patients with rheumatoid arthritis. The vital capacity of the lungs (VCL), indices of bronchial patency: relation of the forced exhalation volume during the 1st second to VCL (FEV/VCL), the rate of the air flow of the forced exhalation when recording the curve the flow-volume (FEV)--peak (Vpeak), the rate in decreasing lung volumes, in 25, 50 and 75% of the whole volume of the forced exhalation (V25, V50, V75). VCL was not decreased in any of the cases, FEV1/VCL comprised 80.04 +/- 44%, Vpeak--110.30 +/- 3.30% of the norm. On the basis of these data a conclusion was made that the patency of the major bronchi was normal. V75 was decreased in 13 out of 23 patients; on the average it comprised 73.78 +/- 4.96% of the norm that significantly differs from the results obtained in healthy individuals (87.1 +/- 1.96 of the norm). An isolated decrease in terminal rates of FEV indicates the reduction of the patency of the minor respiratory ducts. A decrease in V75 was observed more often and was the more marked the longer the duration of the disease. There are no distinct connections between the pathological changes in the minor bronchi condition and the indices of the activity of the inflammatory process and humoral immunity: ESR the level of total protein, alpha 2-globulins, gamma-globulins, rheumatoid factor, circulating immune complexes.

[Rheological disorders in rheumatoid arthritis with systemic manifestations].

A rise in blood viscosity with low rates of the shift which hinders the blood flow in minor vessels was revealed in patients with rheumatoid arthritis (RA). Hemorheological disorders are closely connected with clinical features of RA. Thus, a rise in blood viscosity, enhancement of the aggregation activity of erythrocytes and thrombocytes were most marked in patients with systemic manifestations and high laboratory activity. When investigating the system of hemostasis in patients with RA accompanied by systemic manifestations, the authors noted a tendency to blood hypercoagulation which closely correlated with the rise in blood viscosity. The degree of markedness of hemorheological disorders in RA depends on immunological shifts. Hyperviscosity of the blood in patients with systemic manifestations was considerably attributable to the presence in the blood of pathological immune complexes with a sedimentation constant of 10S-18S, 22S or their combinations. Thus, hemorheological disorders and blood viscosity as the main indices represent an important link of RA pathogenesis closely associated with the autoimmune and immune complex processes determining the development of vasculitis and progress of RA.

[Immunoglobulin therapy of rheumatoid arthritis].

The authors studied the effect of the Soviet-made immunoglobulin drug intended for intramuscular administration on the clinico-immunological indices of 15 patients with rheumatoid arthritis (RA). The drug was administered in a dose of 1500-1800 mg for 7 days running of each month. The control group included 15 patients with RA who received the drug in extremely low doses in accordance with the same pattern. Significant positive changes in the majority of the clinico-laboratory indices were noted in the main group. Individual assessment of the drug efficacy allowed one to note improvement in 8 patients (out of 14) in one month, in 7 (out of 12) in 3 months, in 4 (out of 7) in 6 months. One patient exhibited considerable improvement of her state and one developed fever due to which the treatment was discontinued. The results of the therapy correlated with deexpression of Ia-antigens under the effect of the drug on the peripheral blood lymphocytes.
