MR of hemorrhagic acoustic neuromas.

The magnetic resonance (MR) appearance of hemorrhagic acoustic neuromas has not previously been reported. Four patients with surgically proven acoustic neuromas containing spontaneous hemorrhage were preoperatively evaluated with MR imaging. All patients presented with new onset of symptoms and three had subarachnoid hemorrhage. Signal changes on T1- and T2-weighted spin echo sequences were characteristic for acute, subacute and chronic intraparenchymal hemorrhage. Recognition of hemorrhagic changes on MR is important since more rapid surgical intervention is required in this patient subgroup.

Coaxial needle system of MR- and CT-guided aspiration cytology.

The use of aspiration cytology guided by imaging devices such as ultrasound, CT, or magnetic resonance (MR) has revolutionized the evaluation of disease processes throughout the body. Previously described coaxial needle systems for interventional procedures have a variety of limitations that are particularly problematic for use in the head and neck, CNS, and other areas where critical and complex anatomy is present in a very small region. We describe a new coaxial needle system for CT- and MR-guided aspiration cytology that overcomes these limitations.

Lipomatous lesions of the parotid gland. Potential pitfalls in fine needle aspiration biopsy diagnosis.

Nine cases of benign fatty tumors of the parotid studied initially by fine needle aspiration (FNA) biopsy and subsequently diagnosed by open biopsy were reviewed. All lesions presented as enlargements of the parotid gland. The cytologic diagnoses were correct in only five of the nine cases. While four of six FNA samples from lipomas were correctly diagnosed by cytology, the other two samples were classified as inadequate. Three cases of diffuse infiltration of the parotid by mature-appearing adipose tissue were cytologically characterized as "no diagnostic change" in two cases and sialosis in one case. The clinical, radiologic, cytologic and histologic features of these tumors are described to increase the familiarity of cytologists with these lesions, which may prevent unnecessary surgical intervention in some patients.

MR body stereotaxis: an aid for MR-guided biopsies.

The use of a new body stereotactic system to facilitate magnetic resonance (MR) guided biopsies is described. A skin entry point is first found using a localizer MR scan. The articulating arm of the stereotactic unit is then used to aim the MR needle at the entry point and accurately maintain the needle at the correct angle to intersect the target area. Complex angles may be used to allow needle passes outside the scan plane. This is accomplished by angling the arm out of the plane of the section.

Pneumosinus dilatans of the sphenoid sinus.

Four cases of pneumosinus dilatans of the sphenoid sinus are reported, supplementing the eight cases previously reported in the literature. This rare entity is characterized by expansion of a paranasal sinus that contains only air. In one patient, severe visual loss due to compression of the optic canal by the adjacent enlarged sinus was seen. Galactorrhea occurred in one patient, and three of the four patients reported headaches. There was dehiscence of the sinus roof in two cases, which apparently resulted in a cerebrospinal fluid fistula in one. Previous reports of this entity are reviewed, and the radiographic findings and clinical presentations are discussed. It is proposed that the term "pneumosinus dilatans" be used to describe all dilated, air-filled sinuses with outwardly bulging walls when the primary cause is uncertain.
