[Clinical usefulness of a new tumor marker CYFRA21-1 in patients with lung cancer].

OBJECTIVE: To set the serum cut-off value of CYFRA21-1 in lung cancer patients in china, and to evaluate the clinical usefulness of CYFRA21-1 as a new tumor marker of lung cancer. METHODS: CYFRA21-1 levels were measured with two monoclonal antibodies (Ks19.1 and BM19.21) in the serum of 72 patients with lung cancer, 50 patients with benign lung diseases and 33 post-therapy lung cancer patients. To set the cut-off value, ROC curve was use. RESULTS: (1) Serum concentrations of CYFRA21-1 in patients with lung cancer were significantly higher than those with benign lung diseases (P < 0.001). CYFRA21-1 levels in patients with squamous cell carcinoma were significantly higher than those in patients with any other subtypes. The more advanced the clinical stages, the higher the levels of CYFRA21-1. (2) If the threshold of CYFRA21-1 was set at 5.5 micrograms/L, its sensitivity and specificity for lung cancer were 63% and 94%, respectively. The sensitivity for squamous cell carcinoma was 78%, which was the highest among all the pathological subtypes. (3) The sensitivity of CYFRA21-1 in non-small cell lung cancer increased with the advancement of clinical stages. (4) CYFRA21-1 could be a good index in monitoring patient's condition and predicting prognosis for lung cancer. (5) When CYFRA21-1 was used as a screening index for lung cancer, the threshold of CYFRA21-1 should be set at the upper left corner of the receiver operating characteristic curve of stage I-II. The value of CYFRA21-1 in early diagnosis for non-small cell lung cancer was found limited. CONCLUSION: CYFRA21-1 is a sensitive and specific tumor marker of non-small cell lung cancer, especially of squamous cell carcinoma.

[Long-term efficacy of plasma-derived hepatitis B vaccine among Chinese children--a 12-year follow-up].

OBJECTIVE: To evaluate protective efficacy of a plasma-derived hepatitis B vaccine during 12 years after vaccination. METHODS: A 12-year follow-up of 350 children who participated a randomized, double blind, placebo-controlled trial in 1982. RESULTS: During the 12 years after vaccination, more vaccinated children had anti-HBs of 10 S/N ratios or more than did the controls. At the 12 years 46.1% of the vaccinated and 33.9% of the controls retained anti-HBs levels of S/N ratios > or = 10 (P < 0.05). However, at 12 years median S/N ratio of anti-HBs of the vaccinated children with detectable anti-HBs was lower than that of the controls (15 vs 36, P < 0.0001). Protective efficacy against HBsAg carriage status at 5-10 years after vaccination varied from 83% to 100%. At 12 years, 14 chronic HBsAg carriers were identified among the controls, whereas only 1 case was noted in the vaccinated group (7.6% vs 0.6%, P < 0.01), account for an efficacy of 92%. CONCLUSIONS: Protective efficacy against chronic HBsAg carriage was satisfactory at 12 years after vaccination. No evidence was showed to support a booster of hepatitis B vaccine at this time.

[Persistence of HB vaccine immune protection and response to hepatitis B booster immunization].

OBJECTIVE: To determine the persistence of immune protection of China-made, plasma-derived HBV vaccine in infants and the time table of booster immunization. METHODS: Serological effect of 7-year follow-up study and antibody response of an experimental study on booster were detected using radioimmunoassay. RESULTS: The protective anti-HBs positive rates of 1,018 children, who were vaccinated according to the regimen of three doses of 10 micrograms HBV vaccine, declined from 75.0% during the first two years to 48.2% at 7 years after the first dosages, however, the positive rates for HBsAg and anti-HBc always fluctuated at low frequency. 144 children aged 6 or 7 years old, who received the HBV vaccine in their infancy and were negative for both HBsAg and anti-HBc before booster, were selected from 1,018 children of the follow-up study and boosted with 1 microgram (intradermally) or 2 micrograms (hypodemically) HBV vaccine. Their anti-HBs GMT and anti-HBs positive rate were 190.6 mIU/ml and 89.6% at one month after booster, significantly higher than 14.7 mIU/ml and 54.9% before booster (P < 0.01), and declined back to 25.3 mIU/ml and 75.5% at 12th month; among 65 children with the anti-HBs negative before booster, 40 of them had the level of anti-HBs > or = 100 mIU/ml one month after booster, suggesting the retention of immunological memory in most of them. CONCLUSIONS: No need for revaccination against hepatitis B at 7th year after the initial immunization due to better persistence of immune protection of the vaccine.

[Experimental study on preventive effect of traditional Chinese medicine replenishing recipe on acute oxygen toxicity caused by hyperbaric oxygen].

OBJECTIVE: To seek for the effective traditional Chinese medicine (TCM) replenishing recipe (RR) prescription that being able to prevent the onset of oxygen convulsion and to offer some clues in preventing the oxygen convulsion with TCM-RR. METHODS: Mice were administered orally with four respective representative RR, Sijunzi Decoction, Siwu Decoction, Liuwei Dihuang Decoction, and Renshen Dihuang Decoction. Then the convulsion latent periods were recorded and SOD vitalities in brain and lung were determined. RESULTS: Compared with the control groups, the convulsion latent period of Sijunzi Decoction group was prolonged significantly (P < 0.01) and its SOD vitality in lung was maintained higher (P < 0.01) after convulsion, while the other three RR results have no significant difference (P < 0.05). CONCLUSION: Sijunzi Decoction has certain preventive effect on acute oxygen convulsion.

[Cell clone technique for Taxus cuspidata Sieb. et Zuicc].

The cell clone technique was studied for Taxus cuspidata with needle-derived calli cells. The plating efficiency(PE) of the third generation of suspension culture cells was found to be the highest one and the PE of the medium solidified with phytagel was found to be almost the same as that of the medium solidified with low melting agarose gel. Solid-solid double layer culture was found to promote the PE of Taxus cuspidata cells moderately. A large number of stable cell lines with high growth rate were obtained and the browning problem of cells was completely solved via plate culture of the cell clones of T. cuspidata. The growth rate of the best cell line reached 0.4 g.FW/g.FW.d.

[Effects of mitomycin C on haze after photorefractive keratectomy for myopia in rabbits].

OBJECTIVE: To investigate the effects of mitomycin C on haze after photorefractive keratectomy. METHODS: Forty-five rabbits underwent bilateral 193 nm excimer laser photorefractive keratectomy to correct 8 diopters of myopia. All eyes were allocated randomly to be treated with 0.008% mitomycin C during operation, or 0.1% dexamethasone after operation, or no medical treatment as control. Clinical and histopathologic examinations were made with slit-lamp microscope, specular microscope, light microscope and transmission electron microscope. RESULTS: At 4, 8 weeks after operation, the corneal haze was significantly less in mitomycin C group than that in the control group or in dexamethasone group, and the corneal haze was significantly less in dexamethasone group than that in the control group. At 1, 4, 8 weeks, the number of keratocyte in the anterior stroma of ablation area was significantly less in mitomycin C group than in the control group or in dexamethasone group. The difference between dexamethasone group and the control group was insignificant. The changes in time of reepithelialization, thickness of epithelium and keratocyte number of anterior stroma in ablation area were statistically insignificant among all groups. CONCLUSION: Mitomycin C can reduce corneal haze by inhibiting the proliferation of keratocyte and has no toxicity on cornea. Mitomycin C is more effective than dexamethasone on haze.

[Pathological changes of viscera after brain death: an experimental study].

In order to study the viscera changes after brain death while the function of the heart and lung was sustained artificially, 34 cats were used and divided into three groups. The results showed that pathological changes of viscera (heart, lung, liver and kidney) developed, including 1. ischemic cardiomyopathy, endocardiac hemorrhage, focal necrosis, mitochondria and microfilament destruction. 2. pulmonary congestion, edema and parenchymal inflammation. 3. ischemic lesions in the central band of hepatic lobular and ischemic lesions in renal proximal convoluted tubule.

[A program of HLA system for parentage testing and its application].

A software of the HLA blood group system for parentage testing was developed with FoxPro 2.5 system. Chinese prompts helped to follow the process during program running. It is easy to use. 63 cases of parentage testing in our department were calculated, and the results were identical with those of manual calculation. It was believed that this program is useful in the parentage testing.

[Synthesis and biological activities of 1-alkyl-1-benzyl-4-(3-chloro-2-hydroxy)propyl piperazinium halides].

Seven 1-alkyl-1-benzyl-4-(3-chloro-2-hydroxy) propyl piperazinium halide hydrochloride were synthesized. Preliminary biological activity test showed that the presence of electro-attracting group in the benzyl distinctly increased the analgesic and anti-inflammatory activities of the compounds. For example, the analgesic activity of Ie and the anti-inflammatory activities of Id and Ie were all higher than those of Ia and Ib. The different anion of quaternary ammonium have some effect on the biological activity. However, this effect is not regular.

[Effects of daurisoline and its three optical isomers on ischemic injury in cultured pheochromocytoma (PC12) cells].

In this study, the protective effects of (-)-R.R-daurisoline and its three optical isomers on ischemic injury in cultured PC12 cells induced by treating cells with NaCN in glucose-free medium were investigated. Cell viability was measured using MTT assay. The results indicated that these compounds, especially (-)-S.R and (+)-R.S isomers were found substantially to attenuate ischemic injury in PC12 cells in a dose-dependent manner. The IC50 values of (-)-R.R, (-)-S.R, (+)-R.S and (+)-S.S isomers were shown to be 18.6 x 10(-6), 2.4 x 10(-6), 5.9 x 10(-6) and 90 x 10(-6) mol.L-1, respectively. Intracellular free Ca2+ concentration in PC12 cells was measured using AR-CM-MIC cation measurement system with Fura-2/AM as Ca2+ fluorescent indicator. (-)-R.R-daurisoline and its three optical isomers: (-)-S.R, (+)-R.S and (-)-S.S were found to markedly inhibit the increase of cytosolic free Ca2+ concentration induced by NaCN (20 mmol.L-1) in a dose-dependent manner. Their IC50 were found to be 3.55 x 10(-6), 0.59 x 10(-6), 1.29 x 10(-6) and 24.3 x 10(-6) mol.L-1 respectively. It is suggested that the cytoprotective effects of daurisoline and its isomers were mediated by blocking Ca2+ influx into cells.

[Protective effects of (-)-S.R-daurisoline on neuronal injury in rat primary cortical cultures].

The neuroprotective effects of (-)-S.R-daurisoline on glutamate-induced neurotoxicity were studied in rat primary cortical cultures. The inhibitory effects of (-)-S.R-daurisoline on glutamate-elicited free intracellular Ca2+ increase were also studied in freshly dissociated single brain cells isolated from new born rat using AR-CM-MIC Cation Measurement System. Our experimental results demonstrated that (-)-S.R-daurisoline could obviously inhibit the neurotoxicity induced by glutamate and significantly increased cell viability in dose-dependent manner. In inhibiting glutamate-induced neurotoxicity, the IC50 value of (-)-S.R-daurisoline was found to be 3.4 mumol.L-1. (-)-S.R-daurisoline was also shown to markedly inhibit glutamate-elicited increase of cytosolic free Ca2+ concentration in dose-dependent manner with IC50 value of 2.0 mumol.L-1. Our results showed that (-)-S.R-daurisoline has an obvious protective effect on glutamate-induced neurotoxicity in primary cortical cultures. The protective mechanism of (-)-S.R-daurisoline may be relevant to inhibit Ca2+ influx into cells via glutamate-mediated ligand-gated ion channels.

[Determination of levofloxacin in plasma and cerebrospinal fluid with HPLC and its pharmacokinetics in patients undergoing neurosurgical operations].

A RP-HPLC method was developed to determine the concentrations of levofloxacin in plasma and cerebrospinal fluid and its pharmacokinetics were studied in patients undergoing neurosurgical operations. C18H37 column was eluted with the mobile phase consisting of 10 mmol.L-1 KH2PO4-10 mmol.L-1(C4H9)4Br-CH3CN(45:45:10, pH 3.0) and the utraviolet absorbance was monitored at 295 nm. Ciprofloxacin was used as internal standard. The mean recoveries were 74.76% in plasma and 82.43% in cerebrospinal fluid, with the lowest detectable limits of 10 micrograms.L-1 and 6 micrograms.L-1, respectively. The RSD for the intra-day and inter-day were all less than 5%. A single oral administration of 300 mg levofloxacin tablet was given to 10 patients undergoing neurosurgical operations. The pharmacokinetic parameters in blood and in cerebrospinal fluid could be described by one compartment open model. The pharmacokinetic parameters were: blood Ke 0.12 +/- 0.04 h-1, T1/2 6.05 +/- 1.68 h, Tmax 1.05 +/- 0.29 h, Cmax 3.67 +/- 0.42 mg.L-1, AUC 33.43 +/- 7.32 mg.h.L-1, CLs 9.46 +/- 2.53 L.h-1, Vd 77.49 +/- 7.39 L; cerebrospinal fluid Ke 0.11 +/- 0.04 h-1, T1/2 6.95 +/- 1.88 h, Tmax 3.56 +/- 1.24 h, Cmax 1.68 +/- 0.25 mg.L-1, AUC 23.70 +/- 5.62 mg.h.L-1, CLs 13.70 +/- 5.11 L.h-1, Vd 126.61 +/- 13.20 L.

[Studies on synthesis and analgesic activities of bifentanyl derivatives].

In order to look for potent analgesic compounds and to study the relationships between structures and activities, twenty bifentanyl derivatives were synthesized and their analgesic activities were tested. The structures of the compounds were confirmed by spectral analysis and elemental analysis. Compounds IIa and IIb showed strong analgesic activities in preliminary pharmacological test. Further pharmacological examinations are in progress.

[Gastroenteric absorptive kinetic study on strychine in Jiufensan].

After P. O. Jiufensan, the rest curve of strychine in stomach and intestines of mice per kg was determined by TLC scanning. And the gastroenteric absorptive kinetic rule of strychine in rivo was studied, As a result, the strychine in stomach and intestine of mice accorded with pharmacokinetic model of one room. Also the main pharmacokinetic parameters of this preparation were examined.

[Effect of different animal skin on the transdermal speed constant of sinomenine].

OBJECTIVE: To study the effect of different animal skin on the transdermal speed constant of sinomenine. METHODS: Improved franz-cell was used for the transdermal study, pH6.8 phosphate buffer was used as the receptor and HPLC method was used for the determination of sinomenine through different animal skin. RESULTS: The transdermal speed constant of the skin of hareless mice was much close to which of person. The higuchi equation of sinomenine through skin of hareless mice stored under -20 degrees C for 0d, 3d and 7d were Q = 40.34t1/2 + 196, Q = 38.01t1/2 + 25.3, Q = 41.55t1/2 + 24.7 respectively. CONCLUSION: The results indicate that skin of hareless mice is suitable for the transdermal study of sinomenine, and the effect of storing time (0-7d) under -40 degrees C is not obvious.
