An unusual case of double tuberculous broncho-oesophageal fistulas in an adult.

Broncho-oesophageal fistula associated with tuberculosis is rarely reported in the literature and has not been reported in Hong Kong. We describe a 30-year-old Chinese human immunodeficiency virus (HIV)-negative man with double tuberculous broncho-oesophageal fistulas proven by histology. Constitutional symptoms of active tuberculosis were absent and chest radiograph did not show an obvious lung lesion. Our case shows that broncho-oesophageal fistula can be the sole manifestation of active tuberculosis and that the diagnosis should be suspected in patients who are seen with chronic respiratory symptoms in areas where the prevalence of tuberculosis is high.

[Reinsemination by intracytoplasmic sperm injection 24-hour after conventional in vitro fertilization failure].

OBJECTIVE: To examine the efficacy of intracytoplasmic sperm injection (ICSI) for rescue of conventional in vitro fertilization (IVF) failure. METHODS: From June 1997 to October 1998, 17 women with non-male factor infertility undergoing conventional ovarian hyper-stimulation protocol and IVF in our center, with fertilization failure were reinseminated by ICSI. RESULTS: Intracytoplasmic sperm injection of 130 unfertilized mature oocytes resulted in degeneration in 25 oocytes, successful fertilization occurred in 100 (76.9%) of the survival oocytes, and embryo transfer were done in 18 cycles. Two viable pregnancies resulted. CONCLUSION: If chromosome abnormalities of embryos arising from reinsemination were ignored, our results suggest that ICSI is a successful means of rescuing conventional IVF failure.

[Clinical significance of ProGRP31-98 in patients with small-cell lung cancer].

OBJECTIVE: To determine the clinical significance of serum level of pro-gastrin-releasing peptide 31-98 (ProGRP31-98) for small-cell lung cancer (SCLC) in comparison with neuron-specific enolase (NSE). METHODS: Serum level of ProGRP31-98 was measured by ELISa in 30 patients with SCLC, 30 patients with non-small-cell lung cancer (NSCLC), 10 patients with SCLC who had received treatment, 15 patients with benign lung diseases and 15 normal subjects. The receiver operating characteristic (ROC) curve was used to set the cut-off value and evaluate the diagnostic accuracy. RESULTS: The serum level of ProGRP31-98 was significantly increased in patients with SCLC as compared to that of the normal controls as well as that in any other groups of patients. It was higher in SCLC patients with extensive disease than in patients with limited disease. In patients with distant metastases, it was also higher than in those without. Increase in serum ProGRP31-98 and NSE was both seen in SCLC patients, but the former was of much greater magnitude when compared to the normal controls. Given the cut-off levels of 40 pg/ml for ProGRP31-98 and 8 micrograms/L for NSE, their sensitivity of diagnosis in SCLC was 73% and 60%, respectively. The area under ROC curve of ProGRP31-98 was significantly larger than that of NSE. Similar results of diagnostic sensitivity and area under ROC curve were observed in SCLC patients with limited disease. All patients responded to chemotherapy showed marked decrease in ProGRP31-98. CONCLUSION: ProGRP31-98 is a more specific and sensitive marker than NSE for the diagnosis of SCLC.

[Clinical observation of tiaojining recipe in combination with corticosterone in infantile primary nephrotic syndrome].

OBJECTIVE: To investigate the effects of Tiaojining recipe (TJNR) combined with corticosterone (CS) in treating infantile primary nephrotic syndrome (IPNS). METHODS: Sixty inpatients with IPNS were divided into two groups, which consisted of 30 cases as the treated group treated by TJNR combined with CS and 30 cases of the control group were treated by CS alone for 8 weeks. The changes of urinary protein, serum albumin, blood cholesterol, platelet and blood pressure before and after treatment were observed. RESULTS: The total effective rate of the treated group was significantly higher than that of the control group (P < 0.01). The times for urinary protein disappearance of the treated group was significantly shorter than that of the control group (P < 0.05). Compared with the control group. TINR could prevent hypertension, lower lipidemia and platelets (P < 0.01). CONCLUSION: TJNR could enhance the curative effects of CS on IPNS and reduce the side-effects of CS safely and effectively.

[Comparison of immunology effects between live attenuated hepatitis A vaccine and inactivated hepatitis A vaccine].

OBJECTIVE: To study the immunology effects of live attenuated hepatitis A vaccine in different doses and schedules groups, and compare with inactivated hepatitis A vaccine (Havrix, Smithkline). METHODS: 318 susceptible children were enrolled in Guangxi and Hebei province. These subjects were divided into six groups. Each group was vaccinated with either inactivated vaccine or different doses and schedules of live attenuated vaccine. Serum specimens were collected and tested for anti-HAV antibody at different time after the vaccination. RESULTS: GMT of each group arrived at the peak 1 months after re-vaccination, then declined, and GMT of 6 months after booster dose was still significantly higher than that after primary immunization. Sero-conversion rates in all groups reached 100% after a booster dose, and kept at 6 months after booster dose. CONCLUSIONS: A booster dose of live attenuated vaccine can induce secondary immune response well. The immunology effects after booster dose are comparable with inactivated vaccine and it should be useful to the immune persistence.

[The investigation on the safety and immunogenicity of inactived hepatitis A vaccine AVAXIM].

OBJECTIVES: To investigate the safety and immunogenicity of AVAXIM in children and adults. METHODS: One hundred and twenty-one children aged 6 - 12 years and 108 adults with negative HAV antibody, were inoculated by 0, 6 month procedure with inactivated HAV vaccine produced by Pasteur Merieux Connaught (AVAXIM 160 antigen unit). RESULTS: The positive rates of anti-HAV IgG among children group and adult group were 98.15% and 94.17% respectively after one month of first inoculation, and became 100% and 97.22% after one month of second inoculation. The antibody positive rate was higher in children than in adults, but did not show statistical significance (P < 0.05). The side effects of AVAXIM inoculation were mild, mainly local pain with occurrences 1.7% - 2.8% in both groups. No serious systemic and local reactions were noticed. CONCLUSION: AVAXIM had good safety and immunogenicity, seen not only in adults, but also in children.

[The expression of cell adhesion molecules on lymphatic endothelial cells in rectum adenocarcinoma of human].

This study was conducted to explore the mechanism of lymphatic dissemination and metastasis of cancer cells; the specific aim was to investigate the expression of cells adhesion molecules on lymphatic endothelial cells. Immunohistochemical method was used. The rectum tissues and lymphnodes were taken from five normal human subjects. The cancer peripheral tissues and lymphnodes, and the metastasized lymphnodes were taken from eight patients with rectum adenocarcinoma. The result showed that the intercellular adhesion molecule-1 (ICAM-1) and carcinoma embryonic antigen (CEA) were expressed on lymphatic endothelial cells in cancer peripheral tissues and metastasized lymphnode. The expression of ICAM-1 and CEA in cancer peripheral tissues and lymphnodes was much more remarkable than that in metastasized lymphnodes. These data suggest that ICAM-1 and CEA be cell adhesion molecules between cancer cells and lymphatic endothelial cells.

[Effects of pulsed Nd:YAG laser irradiation on shear bond strength of composite resin bonded to porcelain].

OBJECTIVE: To evaluate the shear bond strength of composite resin bonded to pretreated porcelain surfaces by use of a pulsed Nd:YAG laser irradiation and by use of 8% hydrofluoric etching. METHODS: Surfaces of porcelain were roughened with coarse diamonds until the opaque body porcelains were exposed. The samples were divided into four groups. Samples of the first group were etched with 8% hydrofluoric acid for 3 min by following the manufacter's instructions. Samples of the 2nd, 3rd and 4th groups were separately irradiated by a pulsed Nd:YAG laser of three energy parameters of 15 Hz, 40 mJ; 0.6 W; 15 Hz, 60 mJ, 0.9 W and 15 Hz, 80 mJ, 1.2 W different respectively, each specimen was irradiated for 1 min. Then, silane coupling agent was equally placed on the treated surfaces for 3 min, the activator was used on each surface. The adhesive was applied and light-cured. The composite resin material was finally bonded to the treated porcelain surface, forming a 3 mm x 3 mm x 2 mm resin mass. Samples were then tested for shear bond strength between the treated porcelain and composite resin by using a SWD-10 electric universal testing machine with a crosshead speed of 1 mm/min until fracture. At the end of testing, the data was evaluated by using a two-way analysis of variance (ANOVA) comparing the shear bond strength. On the other hand, characteristic morphology of laser irradiated surfaces of porcelain was prepared and observed by a scanning electron microscopy. RESULTS: The shear bond strength of laser groups of 0.6 W, 0.9 W, 1.2 W and etching group was respectively 8.61, 14.07, 11.22 and 13.47 MPa. There was no significantly difference among groups of laser 0.9 W, 1.2 W and the etching group. This suggested that irradiated on surfaces of porcelain by Nd:YAG laser at the above laser energy could attain the same effectiveness as hydrofluoric acid etching. But the shear bond strength of laser 0.6 W group was significantly lower than the etching group. It suggested that shear bond strength would obviously decrease when laser energy was 0.6 W or lower than this parameter, and had no use in treatment on porcelain surfaces comparing to acid etching. SEM showed that porcelain surfaces irradiated by Nd:YAG laser exhibited rough, umbilicate, lava crater-like structure. This structure helped to produce mechanical retention between resin composite and porcelain. CONCLUSION: The pulsed Nd:YAG laser could replace etching method to pretreat the fracture porcelain surfaces for bonding with composite resin at appropriate energy parameters.

[The study of occlusal contact areas of upper premolars and molars in the unilateral cleft lip and palate].

OBJECTIVE: Occlusal contact plays an important roll in masticatory procedure. It infers to biting force and masticatory performance. In order to analysis the oral function in UCLP, the author tested the occlusal contact areas of upper premolars and molars in UCLP. METHODS: 22 UCLP patients (male 13, female 9, permanent teeth) were selected, which had no temporomandibular joint dysfunction. All of them had received plastic surgery treatment and never received orthodontic therapy. Their occlusal contact areas of upper premolars and molars in intercaspid position were recorded by performed soft wax, and the results were managed by computer with the software of micro image analysis system. All the data were input to computer and be analyzed by statistics. RESULTS: 1. In normal people, the occlusal contact areas of upper premolars and molars had no differences in bilateral and in sexual. 2. there was no sexual difference in UCLP as that in normal group, and the occlusal contact areas in cleft side or non-cleft side had no difference yet. 3. The occlusal contact areas in UCLP were lower than that in normal people. 4. The decrease of occlusal contact areas was the most important factor to low masticatory performance in UCLP patients. CONCLUSION: After plastic surgery in UCLP, orthodontic therapy will improve masticatory contact and contribute to craniofacial esthetic and masticatory function.

[Determinations and analysis of masticatory performance in unilateral cleft lip and palate].

OBJECTIVE: Masticatory function is one of the most important physical functions in craniomaxilla system. Masticatory performance is evaluated with the degree of broken dawn tested food in a limited period of time. Unilateral cleft lip and palate (UCLP) is a congenital abnormality in craniofacial growth and developing. It seriously damages face esthetics and masticatory function in UCLP patients. METHODS: 28 UCLP patients (male 17, female 11, permanent teeth) were selected, which all had received plastic operation treatments and never received any orthodontic therapy. The masticatory performance was determined by the advanced light absorbtion method and took peanuts as tested food. Meanwhile, malocclusal patterns and castings were identified in clinic. All of the data were treated with t-test and step wise-regression in statistics. RESULTS: 1. Masticatory performance of male was higher than that of female in normal people, but there was no sexual difference in UCLP. 2. Masticatory performance in UCLP was lower about 53.97% than that in normal people. 3. Different masticatory times within limited time had no effects on masticatory performance. 4. Different Patterns of malocclusion had different effects on masticatory performance. CONCLUSION: Orthodontic therapy is necessary to improve masticatory function in UCLP after plastic surgery.

[Transcription regulation of intercellular adhesion molecule (ICAM-1) on lymphatic endothelial cells of rectum adenocarcinoma of human].

This study inquired into the mechanism of cancer lymphatic metastasis. The immunohistochemistry method and DNA hybridization in situ were employed in examining the peritumoral rectum tissues and metastatic lymph nodes from 8 rectum carcinoma patients and the rectum tissues and lymph nodes from 5 normal subjects. The results showed that the proteins of ICAM-1 and nuclear factor kappa B (NF kappa Bp65) were expressed on the lymphatic endothelial cells of the rectum adenocarcinoma patients, and there was a NF kappa B binding consensus sequences on ICAM-1 promoter in lymphatic endothelial cells of rectum adenocarcinoma patients. When DIG-AKP labeled 38 bp oligonucleotide probe was used, there was no the expression of ICAM-1 and NF kappa Bp65 on the lymphatic endothelial cells of the normal human lymph node and rectum tissues. It is suggested that the activation of ICAM-1 promoter may critically depend on NF kappa Bp65 homodimers or heterodimers binding to a variant kappa B site on the lymphatic endothelial cells of the rectum adencarcinoma of human. These observations may indicate the potential roles of NF kappa B in cancer metastasis, thus giving clues to facilitate the development of a novel anti-metastasis strategy, that is obstruction the NF kappa B-activation pathways.

[Studies on biosorption of Pd2+ by bacteria].

The strain R08, showed relatively strong ability of adsorbing Pd2+, was screened from different source of bacterial strains. The strain R08 was identified as Bacillus licheniformis. The optimum pH value of Pd2+ biosorption by R08 dead biomass is 3.5. The biosorption is a rapid and non-temperature dependent process. The biomass and Pd2+ initial concentration in solution affected the biosorption process. The bosorptive capacity could reach 224.8 mg/g under the conditions of Pd2+ 200 mg/L, 0.4 g biomass/L, pH3.5 and 30 degrees C for 45 min. TEM observation showed that R08 biomass is able to adsorb and reduce Pd2+ ions to Pd0 particles. IR spectrum analysis indicated that -COO- and HPO(4)2- groups on the cell walls may involve in process of adsorbing Pd2+.

[Intracytoplasmic injection with sperm from ejaculation, epididymis and testis in treating different male infertility with oligo-asthenozoospermia or azoospermia ].

OBJECTIVE: To retrospectively study 400 intracytoplasmic sperm injection (ICSI) cycles by using three different sources of sperm. METHOD: Different male infertility with oligo-asthenozoospermia or azoospermia was treated by ICSI using sperm from ejaculation, epididymis and testis. RESULTS: In the ejaculation group (n = 277), the fertilization rate was 69.6% and the clinical pregnancy rate 28.9%. In the 104 cycles using epididymal sperm, the fertilization and clinical pregnancy rate was 65.9% and 37.5% respectively. And the fertilization and clinical pregnancy of testis sperm (n = 19) was 65.7% and 21.1%. The difference of fertilization and clinical pregnancy rate among the three groups were not significant. CONCLUSIONS: Male infertility of different causes can be treated by ICSI. The outcome has nothing to do with the common parameter of semen except that the sperm motility is 0, but the effectiveness of ICSI should be studied by long-term observation.

[The electrophilic and electrophobic effect in molecular vibration spectra].

A new point of view for electronic effect of molecular vibration spectra was brought: the electrophilic effect of substitution group can enhance the strength of covalent bond. Therefore, its stretching vibration frequency will be increased. Contrarily, electrophobic effect makes frequency decrease. As far as the coordination bond is concerned, if electrophobic substitution group links up the atom which provides the electron doublet then the strength of coordination bond will be enhanced and the stretching vibration frequency of the bond will be increased. The electrophilic substitution group makes the frequency decrease. If substitution group links with the atom that accepts electron, the electronic effect of substitution group is on the contrary.

Gadolinium induces domain and pore formation of human erythrocyte membrane: an atomic force microscopic study.

Lanthanide cations bind to human erythrocyte membranes and enhance cell permeability. It was postulated that this effect is due to their likeness with calcium ions, which have been used to induce perforation of cells. However, the nature and mechanism of the perforation are still not clear. In the present work, the change in surface topography of erythrocyte membranes exposed to various gadolinium species was imaged with an atomic force microscope (AFM) in order to get direct evidence of perforation. The images of the whole cell and regions in nanometer scale showed that the normal surface is featured by closely packed nanometer size particles. The AFM images showed that Gd(3+) binding to erythrocytes led to domain structure at low concentration and pore formation at higher concentration. The domain structures that appeared after incubation with 1.0x10(-6)-1.0x10(-5) mol/l Gd(3+) solution for 30 min are featured by the particles aggregated to form ranges and the separations among them enlarged to gorges. With a higher concentration, 2.5x10(-5) mol/l Gd(3+), the further aggregation developed into crater-shaped 'pores'. By washing with EDTA the 'pores' can be resealed but the domain structure remained. The anionic complex of Gd(3+), [Gd(Cit)(2)](3-) of this concentration, can only induce the domain structure formation. The domain and 'pore' structures mediated by Gd(3+) concentrations might be responsible for both enhanced permeability and perforation. The mechanism of Gd-induced domain formation and perforation is discussed on the basis of aggregation of membrane proteins and the coexistence of different phases of membrane lipids resulting from Gd(3+) binding.

Localization of the PAK1-, WASP-, and IQGAP1-specifying regions of Cdc42.

The Rho family small GTPase Cdc42 transmits divergent intracellular signals through multiple effector proteins to elicit cellular responses such as cytoskeletal reorganization. Potential effectors of Cdc42 implicated in mediating its cytoskeletal effect in mammalian cells include PAK1, WASP, and IQGAP1. To investigate the determinants of Cdc42-effector specificity, we utilized recombinant Cdc42 mutants and chimeras made between Cdc42 and RhoA to map the regions of Cdc42 contributing to specific effector p21-binding domain (PBD) interaction. Site-directed mutants of the switch I domain and neighboring regions of Cdc42 demonstrated differential binding patterns toward the PBDs of PAK1, WASP, and IQGAP1, suggesting that switch I provides essential determinants for the effector binding, but recognition of each effector by Cdc42 involves a distinct mechanism. Differing from Rac1, the switch I domain and the surrounding region (amino acids 29 to 55) of Cdc42 appeared to be sufficient for specific binding to PAK1, whereas determinants outside the switch I domain, residues 157-191 and 84-120 in particular, were necessary and sufficient to confer specificity to WASP and IQGAP1, respectively. In addition, IQGAP1, but not PAK1 nor WASP, required the unique "insert region," residues 122-134, of Cdc42 to achieve high affinity binding. Microinjection of the constitutively active Cdc42/RhoA chimeras into serum-starved Swiss 3T3 cells showed that although preserving PAK1- and WASP-binding activity could retain the peripheral actin microspike (PAM)-inducing activity of Cdc42, interaction with PAK1 or WASP was not required for this activity. Moreover, IQGAP1-binding alone by Cdc42 was insufficient for PAM-induction. Thus, Cdc42 utilizes multiple distinct structural determinants to specify different effector recognition and to elicit PAM-inducing effect.

Stimulation of DNA replication in Saccharomyces cerevisiae by a glutamine- and proline-rich transcriptional activation domain.

Glutamine-rich Sp1 and proline-rich CTF1, two extensively studied mammalian transcription factors, bind to origins of replication in DNA tumor viruses and stimulate viral DNA replication in mammalian cells. Here it is shown that, when tethered to a plasmid-borne cellular origin of replication, the activation domains of both proteins can enhance origin function in Saccharomyces cerevisiae. Hydrophobic patches in Sp1 and CTF1 that mediate transcriptional activation in higher eukaryotes are also important for activation of replication in yeast. However, only the activation domain of CTF1 can enhance initiation of replication from a chromosomally embedded origin. This correlates with the ability of CTF1 to alter the local chromatin structure around the chromosomal origin of replication. The CTF1-induced chromatin remodeling occurs at multiple stages of the cell cycle. These findings strongly suggest a high degree of conservation in the mechanisms used by various types of transcription factors to stimulate viral and cellular DNA replication in eukaryotes.

Tumor gangliosides inhibit the tumor-specific immune response.

Tumor gangliosides are highly immunosuppressive membrane glycosphingolipids that are shed into the tumor cell microenvironment. We directly tested the impact of shed gangliosides on the in vivo antitumor immune response in a syngeneic fully autochthonous system (FBL-3 erythroleukemia cells, C57BL/6 mice, and highly purified FBL-3 cell gangliosides). The major FBL-3 ganglioside was identified as GM1b by mass spectrometry. Substantial ganglioside shedding (90 pmol/108 cells/h), a requisite for their inhibition of the immune function of tumor-infiltrating leukocytes, was detected. Immunosuppression by FBL-3 gangliosides was potent; 5-20 microM inhibited the tumor-specific secondary proliferative response (80-100%) and suppressed the generation of tumor-specific CTLs (97% reduction of FBL-3 cell lysis at an E:T ratio of 100:1). In vivo, coinjection of 10 nmol of FBL-3 gangliosides with a primary FBL-3 cell immunization led to a reduced response to a secondary challenge (the increase in the draining popliteal lymph node mass, cell number, and lymphocyte thymidine incorporation were lowered by 70, 69, and 72%, respectively). Coinjection of gangliosides with a secondary tumor challenge led to a 61, 74, and 42% reduction of the increase in lymph node mass, cell number, and thymidine uptake and a 63-74% inhibition of the increase of draining lymph node T cells (CD3+), B cells (CD19+), and dendritic cells/macrophages (Mac-3+). Overall, the clear conclusion that tumor-derived gangliosides inhibit syngeneic antitumor immune responses implicates these molecules as a potent factor in promoting tumor formation and progression.
