[Intramural hematomas of the small bowel (duodenum excluded) during long-term anticoagulant treatment. Report on 6 cases of which 2 required operation (author's transl)].

Hematomas developed in the small bowel of 6 patients receiving long-term anticoagulant therapy with antivitamins K. As the report by Chapuis and Mercier during the 81st French Congress of Surgery has fully covered this subject, the authors present their cases to illustrate their clinical features, while emphasizing certain particular characteristics: 1 - Associated hemorrhagic lesions were frequently observed. 2 - Early diagnostic follow-through barium meal examinations of the small intestine could be conducted without any particular problem. 3 - Laparotomy was necessary in two patients because of shock or local signs on examination. 4 - Small bowel lesions can vary in severity, sometimes in the same patient (case no 1). Some hematomas heal spontaneously after discontinuation of anticoagulant treatment, while others can, theoretically, develop necrotic reactions. Small bowel resection was necessary in two patients. 5 - A frequent occurrence was the association of lesions in the right colic and ileal regions. 6 - Finally, anticoagulant treatment can often be reduced, discontinued, or replaced by other therapy.

Evaluation of myocardial metabolism after prolonged circulatory arrest and under profound hypothermia. An experimental study in young dogs.

15 young dogs between the ages of 2 and 4 months and weighing less than 6 kg underwent circulatory arrest for 1 h combined with profound hypothermia and extracorporeal circulation. During reperfusion, myocardial metabolism was studied by comparing the oxygen arteriovenous content difference and lactate balance at different temperatures. As the myocardial temperatures rose, the oxygen arterial venous difference increased from 4 vol/100 ml at 25 degrees C to 10 vol/100 ml; lactate balance changed from -19.3 to +8% which because of a large rise in arterial lactate indicates a considerable increase in lactate consumption. This study shows that profound and generalized hypothermia ensures good myocardial protection during at least 1 h of ishcemia and confirms results obtained with other forms of cardiac hypothermia.

[Myocardial metabolism after one hour of anoxia and deep generalised hypothermia. Experimental study in the dog (author's transl)].

Fifteen young dogs weighing less than six kilograms underwent circulatory arrest for one hour and profound hypothermia with extra-corporeal circulation. During reperfusion, myocardial metabolism was studied by comparing the oxygen arterio-venus (02 A-V) difference and lactate consumption at different temperatures. As the myocardial temperature rose, 02 A-V difference increased from 4 vl/100 cc at 25 degrees C to 10 vol/100 cc and K1 lactate balance rose from -19,3% to + 8% which indicates a large increase in lactate consumption. This study shows that profound and generalized hypothermia ensures good myocardial protection during at least one hour of ischemia. This experimental study confirms results obtained with other forms of cardiac hypothermia.

[Gangrenous cholecystitis without lithiasis. A report on a case diagnosed by simple radiography of the abdomen (author's transl)].

The authors report a case of gangrenous cholecystitis without lithiasis but with secondary infection of the gallbladder by anaerobic germs. Pre-operative diagnosis was possible from simple x-rays of the abdomen which demonstrated an obvious fluid level in the gallbladder, and by parietography. An immediate cholecystectomy was performed. Bacteriological examination of the bile confirmed the presence of Clostridium perfringens. The main bile duct appeared normal and the bile in the common bile duct was sterile. Convalescence was uneventful with antibiotic cover, in spite of the age and condition of the patient. Gangrenous cholecystitis occurs more frequently without gallbladder stones in humans. Clinical signs are not specific but simple x-rays of the abdomen are pathognomonic and enable pre-operative diagnosis to be made. Urgent cholecystectomy appears to be the ideal treatment. This affection has a high mortality and early operation is therefore justified.

[Flow through various cardiac valves prostheses (author's transl)].

We have studied the flow through various cardiac valves prostheses, with the assistance of an artificial left ventricular pump. We took photographs during the whole of the cardiac cycle but we present here only the view of the middle systolic period. As regards the mechanics prostheses (Bjork-Shiley, Lillehei-Kaster, Saint-Jude-Medical, Starr-Edwards) the flow changes with the shape and opening of the valve. Biologic prostheses (Carpentier-Edwards, Hancock, Ionescu-Shiley) present an impertubated flow.

[Pulmonary wounds (author's transl)].

A study of 22 patients with pulmonary wounds has confirmed the advantages of the traditional therapeutic attitude adopted in the majority of cases, and the rare indications requiring urgent or early operation for pulmonary lesions. Hunting gun pulmonary wounds, in which there is predominant "contusion" require individual treatment, and in 2 of the 4 cases treated in this serie, secondary excision was necessary.

[Artificial left ventricle in paracardiac position between the apex of the heart and the ascending aorta (author's transl)].

A device for assistance of the left side of the heart in acute failure is placed in the paracardiac position. The blood is drained into the apparatus through the apex of the heart and is reinjected into the ascending aorta by an artificial pump. The flow of blood within the prosthesis and at the level of the inlet and outlet values is studied by a system of lighting in thin layers. Several types of original valve have been tested but the most satisfactory results were obtained with a classical bead valve.

[Artificial left ventricle between the apex of the heart and the ascending aorta (author's transl)].

The artificial left ventricule is placed in the paracardiac position to ensure a short circuit of the natural left ventricule which is left in position. The prosthesis is implanted on the tip of the heart to ensure wide drainage of the left ventricule and on the ascending aorta to obtain systolic injection under physiological hemodynamic conditions. On both sites of implantation are sutured dacron tubes which permit insertion of the prosthesis without interrupting the general circulation and its withdrawal by simple ligature of the tubes of dacron.

[Aspiration-drainage of an emphysematous bulla in a patient with severe respiratory insufficiency].

The authors report the case of a patient with severe alveolar hypoventilation, with a large emphysematous bulla in the upper part of the right pulmonary field. The drainage aspiration of the bulla as a life saving technique greatly improved the respiratory function. Blood gases reverted to normal and the bullae disappeared. Literature reviewing. Discussion of radiological and functional criteria of compressing the pulmonary parenchyma around the bulla.
