A 63-year-old man was admitted to the respiratory intensive care unit for pneumonia. Fifteen years earlier, hairy cell leukemia had been diagnosed and the patient underwent splenectomy. A clinical suspicion of legionellosis, later confirmed by both serology and isolation of the microorganism, prompted initiation of high dose erythromycin intravenously. The patient steadily deteriorated and passed away eight days later. This is the first reported case of hairy cell leukemia in which the diagnosis of Legionella longbeachae sero-group 1 infection was based on both serology and isolation.

The costs for a complet functional restoration of the dentition and the parodontium (orthodontics not included) in a sample of 1392 adolescents aged 16-20 (Geneva, 1970-1972) have been estimated on the basis of an approved scale of fees (1). These costs are analyzed with respect to age, sex, occupation and socio-professional status of the parents.
Five successive measurements of the peak expiratory flow rate have been made on a large number of children and adolescents in various age groups and both sexes. The predominant part of the increase in individual performance is already achieved during the first three trials. Besides, the maximum value of the five measurements apparently does not differ essentially from the mean value of the two or three highest measurements.
New tables have recently been published containing (1) the usual percentiles of body weight and stature in each of the successive 6 months age groups, (2) two different age independent estimates of weight percentiles related to height, and (3) 50%, 80%, and 95% contours for the bivariate joint distribution of height and weight in each age group. All of the tables and charts are given for boys and girls separately. In view of the randomness of the sampling and the characteristic heterogeneity of the population concerned, these new normal limits may be declared valid not only for the entire Swiss area, but even for adjacent regions.
UNLABELLED: The present paper reports some initial numerical results referring to an epidemiological study of the main dental and periodontal diseases prevalent in late adolescence. The study itself is based on a random sample survey covering the Geneva adolescent population, ages 16 to 20, and takin in 1970-1972. SAMPLE SIZE: 1392, i.e. 13% of the population. The first part depicts the overall dental health status of the mentioned sample in terms of DMF teeth counts (total recording) as depending on age groups (16-17, 17-18, 18-19 and 19-20 years), occupation (students, apprentices, and young labourers) and sex (boys, girls). The second part is devoted more specifically to the lasting effects ascribable to fluorine prophylaxis: variations of the DMF tooth count, as a function of age, for adolescents protected by fluoride intake in comparison with adolescents not so protected; estimated effect, due to fluoridation, on severity of carious lesions, again in relation with age. It is concluded that regular intake of fluoride tablets decreases overall caries frequency and slows down development of more severe forms.
Discriminant analysis and its corresponding classification procedures are illustrated by means of two practical applications pertaining to a dental health survey. The discriminating variables refer to dental caries, oral hygiene, periodontal diseases and fracture of anterior teeth. Their relative discriminating power is discussed.
Charts were reviewed for 63 patients whose chronic pseudomonas osteomyelitis was treated with high doses of extended-spectrum penicillins for prolonged periods. The incidence of untoward drug reactions was significantly higher than expected. Carbenicillin evoked adverse reactions in 22.8% of patients. However, most of these reactions were mild, and a change of drug was required in only 5.7% of cases. No adverse drug reactions were observed with cumulative doses of less than 750 g. In contrast to carbenicillin, the ureidopenicillins were associated with adverse reactions in 67.7% of patients; most reactions were moderate to severe in intensity; a cumulative dose of greater than 250 g produced adverse reactions; and discontinuation or change of therapy was required in 51.6% of cases. The main adverse reactions to both carbenicillin and the ureidopenicillins included rash, drug fever, leukopenia, eosinophilia, thrombocytopenia, and hepatic damage.
We present a new method for evaluating in vivo changes in bone mineralization in the peripheral skeleton, using computed tomography (CT). A set of bone mineralization indices are generated from numerous CT images of the patient's distal radius. The cross-sectional anatomy displayed by the CT scan allows for separate evaluation of the cortical and trabecular bone. Correction for possible drift of the CT number scale (Hounsfield scale) is achieved by scanning standard solutions of dipotassium hydrogen phosphate simultaneously with the forearm. Preliminary data indicate that this is a precise method for evaluating in vivo changes in bone mineralization.
Medial aplasia of the spine is a rarely recognized malformation characterized by isolated absence of spinal segments, with vertebrae present cranially and caudally. We analyzed six patients and the five cases described in the relevant literature. The extent of defect ranges from one to 11 segments and leads to spinal instability with resulting kyphosis. All patients have a thoracic or high lumbar level of paralysis and, in varying degrees, typical contractures of hips, knees, and feet. The differentiation between medial aplasia of the spine and the syndrome of caudal regression is discussed.

The term cutaneous T-cell lymphoma (CTCL) has recently been coined to describe a group of rare lymphoproliferative T-cell disorders classically beginning in the skin and pursuing a chronic, progressive, and indolent course eventually followed by visceral involvement. This term encompasses the spectrum of the traditional mycosis fungoides and Sézary syndrome. A unique case of CTCL that developed a fulminant course leading to death within a few months following the initial symptoms is presented. The clinical course bears a striking similarity to a group of acute T-cell malignancies, while the cytologic-pathologic findings favor the diagnosis of Sézary syndrome. Regardless of the exact classifications, this entity appears to be a highly malignant T-cell disorder, resistant to chemotherapy.
The use of antibiotic prophylaxis in emergency cesarean section has become the standard of care. Concern over fetal exposure has been the reason that antibiotics are traditionally given post cord clamping, but this does not allow adequate antibiotic levels in maternal blood and tissue to be achieved at the stage of incision and during surgery. We evaluated by bioassay maternal and fetal serum and placental ceftriaxone concentrations following the intravenous administration of 1.0 g of ceftriaxone upon the decision to perform cesarean section. The preoperative prophylaxis with ceftriaxone resulted in adequate protective levels in the mother and very low fetal blood levels.
Mice infected with Brucella melitensis were treated with azithromycin or roxithromycin at a dose of 50 mg/kg/day i.p. alone and in combination with streptomycin 75 mg/kg/day for 14 days. Streptomycin at this dose was previously documented to be ineffective against murine brucellosis. Azithromycin- and azithromycin/streptomycin-treated animals demonstrated a significantly better cure rate than controls. Therapy failure was observed in all mice treated with roxithromycin 50 mg/kg/day i.p. alone or in combination with streptomycin 75 mg/kg/day. Our findings demonstrate that azithromycin cures experimental murine brucellosis and may be an effective alternative in the therapy of human brucellosis.
OBJECTIVE: To evaluate the diagnostic value and potential risk of fine needle aspiration biopsy (FNAB) of the spleen in patients with diffuse splenomegaly due to an undetermined cause. STUDY DESIGN: Retrospective evaluation of the medical records and cytologic material from 58 patients on whom FNAB was performed between 1967 and 1993. RESULTS: Sixty-five FNABs were performed on 58 patients. Lymphoproliferative diseases were found in six; in four additional cases, metastatic adenocarcinoma, sarcoidosis, Candida albicans and enterococcal infection were demonstrated. The remaining 55 biopsies either showed normal splenic tissue or were nondiagnostic. No complications were recorded. CONCLUSION: Splenic FNAB may be useful in some patients with diffuse splenomegaly. The diagnostic yield is rather low. The procedure, however, is simple, safe and well tolerated.
