[Vascular injury problems in childhood and adolescence].

Because of the marked elasticity of healthy blood vessels in children and adolescents, vascular trauma is very rare. In penetrating injuries, arteriography is not usually necessary but this procedure is essential in cases of blunt trauma to the blood vessels. The type of reconstructive procedure depends on the injury and its extent and can be performed by direct vascular anastomosis, by the insertion of a vein patch or by the transplantation of an autogenic venous segment. General anaesthesia is usually essential. Interrupted one-layer end-to-end anastomosis is advocated. The surgical technique of vascular repair must be meticulous. 12 children and adolescents (most of them had severe multiple injuries including vascular injuries) were followed up clinically and via arteriography. In 11 patients, the reconstructed vessels were found to be functioning normally.

Beagle puppy model of intraventricular hemorrhage. Effect of indomethacin on local cerebral glucose utilization.

The newborn beagle puppy has been demonstrated to provide a good model for neonatal intraventricular hemorrhage (IVH). A study was designed to determine if indomethacin can prevent IVH, and if indomethacin would produce changes in local cerebral glucose utilization (LCGU). By computerized random design, newborn beagle puppies were pretreated with either indomethacin (a known inhibitor of prostaglandin synthetase) or saline, and then assigned either to receive a hemorrhagic hypotension/volume reexpansion insult or to receive no insult. Pretreatment with indomethacin produced a marked drop in the incidence of IVH as well as significant alterations in the blood pressure responses to the hemorrhagic hypotension/volume reexpansion insult. Carbon-14 autoradiography was used to determine LCGU: no alterations were demonstrated in cerebral metabolism in uninjured pups pretreated with indomethacin compared to saline-pretreated animals. In addition, although the hemorrhagic hypotension/volume reexpansion insult produced marked alterations in LCGU in both groups of traumatized pups, indomethacin prevented the changes in LCGU in the germinal matrix and white matter that were found in the saline-pretreated animals.

Beagle puppy model of intraventricular hemorrhage. Effect of indomethacin on cerebral blood flow.

The newborn beagle puppy has been demonstrated to provide a good model for neonatal intraventricular hemorrhage (IVH). A study was designed to determine if indomethacin can prevent IVH and if indomethacin would produce changes in cerebral blood flow (CBF). Newborn beagle puppies were randomized by computer into two groups: one was pretreated with indomethacin, a known inhibitor of prostaglandin synthetase, and the other was saline. The dogs in both groups were then assigned either to undergo hemorrhagic hypotension/volume reexpansion insult or to receive no insult. Twenty percent of all pups receiving indomethacin and undergoing the insult experienced IVH, compared to 71% of the pups undergoing insult that had been pretreated with saline. Significant alterations in the blood pressure responses to the hemorrhagic hypotension/volume reexpansion insult were noted in the former group compared to the saline-pretreated pups subjected to insult. Finally, employing carbon-14 autoradiography for the determination of CBF, it was demonstrated that indomethacin decreases resting CBF of the newborn beagle pups and, in indomethacin-pretreated animals subjected to insult, prevents the increases in CBF seen in the saline-pretreated traumatized pups.

[Morphology of prune belly syndrome].

Presenting a case of our own, the different forms and their severity of the prune-belly-syndrome are discussed. Considerations about the pathogenesis let us accept a common superordinate disturbance in the differentiation of myotoma and urinary system.

[Histochemical studies on striated muscle of ischemic and angiosurgically reperfused lower extremities].

There have been reports on an increased oxidative capacity in muscle tissue from the diseased legs of patients with intermittent claudication. In biopsy from specimens the gastrocnemius muscle of 25 patients the effect of arterial reconstructive surgery were studied, using fresh frozen cryostate sections and histochemical reactions for some oxidoreductases. The present study describes additional histochemical changes in the leg muscles in patients suffering from arterial insufficiency. The changes observed are correlated to the clinical severity of the disease.

[Electrophoretic behavior of peripheral blood lymphocytes in thermally injured patients].

The division of peripheral blood lymphocytes in two populations succeeds by means of the electrophoresis equipment PARMOQUANT 2. These two fractions correspond to the B and the T cells according to Sabolovic. Continuous controls of the course in 9 patients with severe burn injuries showed an increase of the slowly migratory lymphocytes on the second to third day after the trauma. There occurs a slow normalisation towards the initial values in the end of the second week after the burn. This did not happen in case of the death of the patient because of his burn injury.

[Diagnostic and therapeutic problems in endangiitis obliterans].

Out of all patients (n = 313) suffering from peripheral vascular disorders seen at our Department from 1975 to 1981 21 patients had Buerger's disease (Thrombendangitis obliterans). In 4 of them immunological deposits could be traced in the basement membrane of their capillaries. In our opinion nicotine has a stimulating immunological effect on the vessel wall and can be considered as the trigger mechanism for Buerger's disease.

Beagle puppy model of intraventricular hemorrhage.

Intraventricular hemorrhage (IVH) is a major neurological problem of the preterm infant and originates in the germinal matrix tissues of the developing brain. The newborn beagle pup has been demonstrated to provide an excellent model for this neonatal neuropathological condition. By the production of hemorrhagic hypotension followed by relative volume reexpansion, such hemorrhages can be produced in this animal. Carbon-14 (14C)-iodoantipyrine was used for measuring the cerebral blood flow in this experimental model and demonstrated that, although the germinal matrix is a relatively low-flow area in the newborn beagle pup, the selective flow to this region increases significantly in animals with IVH.

[Are brain-protective measures in the reconstruction of extracranial vascular occlusions necessary?].

Surgical reconstruction in extracranial vascular occlusions with a healing chance of 80 per cent and a mortality rate of 1 to 4 per cent in stage II after Vollmar has well established itself. The indication for reconstruction and the surgical technique are to a large extent standardised. The question of brain-protective measures during the intervention is still open. At the Surgical Clinic of the Medical Academy Magdeburg 74 vascular reconstructions were carried out in the period from 1975 to 1980. Only when multiple occlusions or stenoses were found, the intraluminal shunt technique was employed as a brain-protective measure in six patients. Postoperative neurological complications were only found in two patients in the form of a transient hemiparesis. The late results after five years showed during or an improvement of the complaints in 50 per cent. The total mortality during the observation period was II per cent. We are of the opinion that brain-protective measures in the reconstruction of extracranial vascular regions need not be employed as a routine measure.

[Does oral contraception cause deep venous thromboses? (author's transl)].

From 1975 to 1980 48 women suffering from deep venous thrombosis were seen at the Surgical Dept. of the Medizinische Akademie Magdeburg. In 22 cases no objective cause could be found, 16 women, however, had been put under oral contraceptives for many months and even for years. Although there is no causal evidence between oral contraception and deep venous thrombosis until now, oral contraception has to be considered as a risk factor concerning the formation of deep venous thrombi in women.

Iridium- 191 angiocardiography for the detection and quantitation of left-to-right shunting.

An osmium- 191 leads to iridium- 191 generator that can deliver multiple doses of Ir- 191m for first-pass radionuclide angiography has been developed. Iridium- 191m has a physical half-life of 4.96 sec and decays with emission of 65-keV and 129-keV photons in 58 and 30% abundance, respectively. Using a gamma camera, Ir- 191m radionuclide angiography was carried out, in dogs and ten patients, for the detection and quantitation of left-to-right shunting. In a one-year-old patients, 25 mCi of Ir- 191m results in a whole-body radiation absorbed dose of 35 mrad. Multiple Ir- 191m angiograms can be performed, seconds to minutes apart, without interference from background. The 15.4-day half-life of Os- 191 permits transportation of the generator to centers far from the production facility. With the low radiation dose, high information density, and the ability to repeat studies with Ir- 191m, clinical use of radionuclide angiography should be expanded.

[Analysis of the metaplastic disturbances in dislocated hip joints in the orthopedic hospital of the Medical Academy in Magdeburg (author's transl)].

One of the most frequent and prognostically unfavorable complications occurring with therapy for dislocated hip joints is a metaplastic disturbance at the coxal end of the femur. We undertook a differentiation between metaplastic disturbances in necrosis of the center of the femur head and restorative disturbances. In the evaluated period between 1964 and 1974, 1298 children (2236 treated hips) were examined. Between 1965 and 1968 when the old, classical therapy for dislocation was employed, we established a metaplastic disturbances rate of 20%. The rate for necrosis of the femur head was 9% and the restorative disturbance rate was 11%. The metaplastic disturbance rate was lowered to 5.65% as a result of the modern, functional, early therapy instituted between 1969 to 1974. Here we established a rate of necrosis for the femur head of 0.45% and a restorative disturbance rate of 5.2%. This falls within the range established by Huber (5.5%) and Unger (6.06%). The possibility of an endochondral dysostosis must always be considered when bilateral restorative disturbances are present. We observed that restorative disturbances always occurred more frequently in connection with dysplasias. We fell that the cause is the endogenous tendency toward restorative disturbances of the center of the femur head. With early therapy, the possibility of a dislocation can, however, be avoided. In contrast to Kiltz, our later results and follow-up examinations indicate a poorer prognosis for necroses of the center of the femur head.
