Obtaining reliable measurements of knee extensor torque produced during maximal voluntary contractions: an experimental investigation.

The purpose of this study was to develop and test a protocol that could be used to obtain reliable measurements of knee extensor torque produced during maximal voluntary contractions. On each of 3 days, 10 subjects performed six consecutive maximal voluntary contractions, in the same randomized order, for each of the following 10 conditions: concentric isokinetic contractions at velocities of 30 degrees, 90 degrees, 120 degrees, and 180 degrees/s; eccentric isokinetic contractions at velocities of 30 degrees, 90 degrees, 120 degrees, and 180 degrees/s; and isometric contractions at 40 and 60 degrees of knee flexion. The peak torques produced were examined to determine on which day and during which contraction subjects produced the greatest torques for each condition. This information was used to develop a practice protocol. Fifteen different subjects were tested following this protocol. Subjects participated in two practice sessions, a test session, and a retest session. Intraclass correlation coefficients (ICCs) were calculated to determine the degree of agreement between torques for the test and retest sessions. The ICCs ranged from .87 to .98. The protocol developed appears to be useful for obtaining reliable measurements of knee extensor torque.

Intertester reliability of judgments of the presence of trigger points in patients with low back pain.

The purpose of this study was to determine the intertester reliability of assessments of the presence of trigger points in the region of the lumbar spine of patients with low back pain (LBP). A total of six trigger points described by Travell and Simons were studied. The trigger point examination procedures described by Travell and Simons were used by 12 physical therapists. Randomly paired therapists examined 50 patients for 197 trigger points. The Kappa coefficient, percent agreement, the observed proportion of positive agreement (Ppos), and the observed proportion of negative agreement (Pneg) were used to describe reliability. Kappa values ranged from .29 to .38. Percent agreement ranged from 76% to 79%. Ppos ranged from .43 to .52. The low Kappa and Ppos values suggest different therapists are unable to reliably determine when a trigger point is present in a patient with LBP. This study suggests the usefulness of examining for the presence of trigger points in patients with LBP should be questioned.

Effects of therapeutic ultrasound on bone marrow in dogs.

With a thermocouple in the medullary cavity of one dog's femur, the increases in bone marrow temperature were demonstrated at five different intensities of ultrasound. The right distal femurs of six dogs were treated ten times, three dogs at 1.5 w/cm2 and three dogs at 2.5 w/cm2 each for five minutes. At a later date, two of these dogs received an additional ten minutes of ultrasound at 2.5 w/cm2 for ten treatments. Histological and hematological studies gave presumptive evidence of bone marrow damage and regeneration. A marked increase in oil was also seen in the bone marrow smears.
