Plasma ammonia concentrations in newborns and children.

Many, mostly congenital, hyperammonemic syndromes may occur in the first days of life. For the diagnosis of these disorders plasma ammonia levels physiologically present in neonates and early childhood should be known, preferably by the use of an enzymatic method. Full-term newborns had higher capillary ammonia concentrations on the first day of life (30-144 mumol/l) than on the fifth day (31-104 mumol/l). In the same individuals the arterial (38-89 mumol/l) are lower than the capillary concentrations (69-112 mumol/l). The venous concentrations in school children were not higher than in adults. The scatter of capillary values, however, is great. For diagnostic purposes of disorders of ammonia detoxication sampling of arterial or venous blood is preferable.

Avoidance of perinatal transmission of hepatitis B virus: is passive immunisation always necessary?

Screening of 8918 pregnant women revealed that 107 (1.2%) were HBsAg-positive. 92 of them (50 of German and 42 of predominantly Asian origin) have already delivered and were followed up for 12 months together with their infants. 14 infants of HBeAg-positive (or anti-HBe-negative) carrier mothers received a single dose of hepatitis B immunoglobulin (HBIG) immediately after birth, while 60 infants of anti-HBe-positive mothers were not immunised. 57 of the 60 "unprotected" children remained seronegative for HBsAg and HBeAg. 3 children showed HBs-antigenaemia immediately after birth; 2 of these lost HBsAg and developed anti-HBs after 6 and 9 months. The third lost HBsAg after 4 months without an antibody response developing. This suggests that the HBsAg particle only was transmitted rather than the whole virus. 12 of the 14 infants born to HBeAg-positive, or anti-HBe-negative carrier mothers were protected with one single high dose of HBIG. 2 had HBs-antigenaemia, 1 had HBeAg as well. These data show that in a West European population the risk of perinatally acquired HBV infection in the infants of anti-HBe-positive carrier mothers is small, and that passive immunisation of this group is not necessarily indicated.

The hypereosinophilic syndrome and lymphoblastic leukemia with extra C-group chromosome and q14+ marker.

Hypereosinophilic syndrome is probably a disease of diverse etiologies. We studied the bone marrow of a patient with HES and found a population of L-1 lymphoblasts. These cells failed to grow in double-layer agar cultures, were "null cells," and contained a 14q+ marker chromosome consistent with a malignant lymphoproliferative disorder. Complete remission was induced with vincristine, prednisone, and L-asparaginase but the patient died from the consequences of cardiac fibrosis. An underlying lymphoproliferative process should be carefully sought in patients with HES to afford the best opportunity for cure.

Murine resistance to type III group B streptococci.

We examined the role of serum, complement and the reticuloendothelial system in mouse resistance to type III group B streptococci (GBS). Normal serum obtained from 21 day-old mice failed to protect chick embryos against a lethal intravenous (IV) inoculation of GBS-III-SS620/50. Mice that were decomplemented using cobra venom factor remained resistant to intraperitoneal (IP) inoculation of 10(6) colony-forming units (CFU) of GBS-III-22620/50, GBS III-Bell and GBS-III-Minnesota. In contrast, mice prepared with IP oleic acid were killed with 10(4) CFU of GBS-III-SS620/50 IP. However, when mice were injected IV with oleic acid, they remained resistant to IP inoculation of GBS-III-SS620/50. These results suggest that peritoneal macrophages play a role in murine defense against IP infection with GBS-III-SS620/50.

Immunoglobulins and beta-1-C globulin in hydatidiform mole vesicles.

Qualitative and quantitative determinations of immunoglobulins (Ig's) and beta-1-C globulin were carried out in fluid vesicles of hydatidiform moles and carrier mothers. The mean concentration of IgG was 9.9 per cent, IgA had 14.3 per cent, in relation to maternal values. No IgM concentrations were detected in molar fluid. Beta-1-C globulin showed 60.5 per cent concentration in relation to mother's serum. It is suggested that in trophoblast of molar vesicles a selective transudate migration of Ig's by a mechanism of pinocytosis takes place to the vesicular fluid.
