Phenylalanine hydroxylase gene mutations in the United States: report from the Maternal PKU Collaborative Study.

The major cause of hyperphenylalaninemia is mutations in the gene encoding phenylalanine hydroxylase (PAH). The known mutations have been identified primarily in European patients. The purpose of this study was to determine the spectrum of mutations responsible for PAH deficiency in the United States. One hundred forty-nine patients enrolled in the Maternal PKU Collaborative Study were subjects for clinical and molecular investigations. PAH gene mutations associated with phenylketonuria (PKU) or mild hyperphenylalaninemia (MHP) were identified on 279 of 294 independent mutant chromosomes, a diagnostic efficiency of 95%. The spectrum is composed of 71 different mutations, including 47 missense mutations, 11 splice mutations, 5 nonsense mutations, and 8 microdeletions. Sixteen previously unreported mutations were identified. Among the novel mutations, five were found in patients with MHP, and the remainder were found in patients with PKU. The most common mutations were R408W, IVS12nt1g-->a, and Y414C, accounting for 18.7%, 7.8%, and 5.4% of the mutant chromosomes, respectively. Thirteen mutations had relative frequencies of 1%-5%, and 55 mutations each had frequencies < or = 1%. The mutational spectrum corresponded to that observed for the European ancestry of the U.S. population. To evaluate the extent of allelic variation at the PAH locus within the United States in comparison with other populations, we used allele frequencies to calculate the homozygosity for 11 populations where >90% ascertainment of mutations has been obtained. The United States was shown to contain one of the most heterogeneous populations, with homozygosity values similar to Sicily and ethnically mixed sample populations in Europe. The extent of allelic heterogeneity must be a major determining factor in the choice of mutation-detection methodology for molecular diagnosis in PAH deficiency.

A comparison of the effects of valproate and its major active metabolite E-2-en-valproate on single unit activity of substantia nigra pars reticulata neurons in rats.

Numerous findings suggest that the substantia nigra pars reticulata (SNR) acts as a seizure-gating mechanism and constitutes a site where protection against a broad spectrum of seizures can be obtained by inhibiting the activity of GABAergic output neurons to SNR target regions, particularly the superior colliculus. Most pharmacological studies on SNR neurons have been conducted with local microinjection of drugs, whereas studies with systemic administration of anticonvulsant drugs, e.g., the antiepileptic drug valproic acid (VPA), have yielded inconsistent data. In the present study, we examined the dose response and time course of the effects of anticonvulsant doses of VPA and its major active metabolite E-2-en-VPA on extracellularly recorded spontaneous single unit activity of nondopaminergic SNR neurons in a large group of chloral hydrate-anesthetized rats. The activity of each neuron was recorded over a period of at least 30 min after intravenous drug or vehicle administration. Both drugs rapidly decreased the firing rate of SNR neurons. Compared with predrug base-line values of neuronal firing of SNR neurons, the average peak inhibition of neuronal activity was approximately 16% at the lowest dose (50 mg/kg), approximately 20% (VPA) or 30% (E-2-en-VPA) at 100 mg/kg and approximately 50% (VPA) or 60% (E-2-en-VPA) at the highest dose (200 mg/kg) tested. ED50 values calculated from these data were 240 mg/kg for VPA and 159 mg/kg for E-2-en-VPA, respectively. Determination of drug levels in SN at time of peak effect after administration of 100 mg/kg VPA or E-2-en-VPA indicated that E-2-en-VPA exhibited its inhibitory effect on SNR firing at significantly lower concentration than VPA, suggesting that E-2-en-VPA is more potent than VPA in this regard. Except one SNR neuron, all neurons recorded in 52 rats reacted with a reduction in firing rate to administration of VPA or E-2-en-VPA, demonstrating a rather homogeneous susceptibility of SNR neurons to these drugs. The inhibition of SNR neurons by these drugs could be a result of increased GABA turnover, which has previously been reported to be induced in this region by systemic administration of VPA. We conclude that the ability of VPA and E-2-en-VPA to reduce firing of SNR neurons could be critically involved in their effectiveness in inhibiting seizures in various experimental models.

Dose-dependent anticonvulsant and proconvulsant effects of nitric oxide synthase inhibitors on seizure threshold in a cortical stimulation model in rats.

In the central nervous system, nitric oxide (NO) is increasingly being considered as a trans-synaptic retrograde messenger, being involved for instance in cellular responses to stimulation of glutamate receptors of the NMDA subtype. Thus, compounds that modify NO production, such as NO synthase inhibitors, may provide a means of altering NMDA receptor function. The functional consequences of NO synthase inhibition are, however, complicated by the fact that NO not only serves as a messenger to activate guanylyl cyclase and so to raise cGMP in target cells in response to NMDA receptor stimulation but also to induce feedback inhibition of the NMDA receptor via a redox modulatory site on the receptor complex. This may explain the contrasting results obtained previously with NO synthase inhibitors in animal models of ischaemia and seizures. In the present study, we tried to resolve the reported discrepancies about the effects of NO synthase inhibitors in seizure models by studying such drugs at various doses in a novel model of cortical seizure threshold. In this model, the threshold for seizures in rats is determined at short time intervals by applying ramp-shaped electrical pulse-trains directly to the cerebral cortex, allowing one to determine the time course of anti- or proconvulsant drug effects in individual rats. Two NO synthase inhibitors, NG-nitro-L-arginine and NG-nitro-L-arginine methyl ester, were compared with a clinically effective antiepileptic drug, i.e. valproate.(ABSTRACT TRUNCATED AT 250 WORDS)

Nutritional therapy for pregnant women with a metabolic disorder.

Nutritional therapy is essential for a normal reproductive outcome in phenylketonuric women. In homocystinuria, fetal outcome is good in women whose disorder is responsive to vitamin B6 therapy and is poor in women whose disorder is unresponsive to therapy. Pregnancy in galactosemia is rare because of the almost universal ovarian dysfunction present in female patients with this disorder. Transplantation of the fertilized ovum is a promising possibility for these women. In women with MSUD, there has been only one case of pregnancy reported to date.

Mutation analysis in families with discordant phenotypes of phenylalanine hydroxylase deficiency. Inheritance and expression of the hyperphenylalaninaemias.

Neonatal hyperphenylalaninaemia caused by mutations in the gene encoding phenylalanine hydroxylase (PAH) represents a wide spectrum of metabolic phenotypes, ranging from classical phenylketonuria (PKU) to mild hyperphenylalaninaemia (MHP). The marked interindividual heterogeneity is due to the expression of multiple PAH mutations in genetic compounds. We have investigated four unusual families in which both PKU and MHP were present. In each family three different mutations in the PAH gene were identified, including two associated with PKU and one associated with MHP. The unexpected outcome of discordant phenotypes within the families described is explained by previously unrecognized parental MHP. By mutation analysis we have also predicted the phenotypical outcome in a hyperphenylalaninaemic infant born to a mother who before pregnancy had been diagnosed as having MHP. Our results demonstrate the utility of nucleic acid analysis in follow-up in PKU screening programmes.

[The evaluation of orthodontic treatments. Reflections on the Eismann procedure].

This study presents a broad overview of the fundamental questions involved in evaluating orthodontic treatment methods using a formalized index system. Eismann's numerical success evaluation, which he published 25 years ago and which is an example of this method, is examined in some detail. Both the dividends and limits of the Eismann method are discussed on the basis of our own studies and those of others and modifications to the method are suggested. Not withstanding the limitations inherent in any model based analysis, the Eismann approach is shown to be superior to many similar systems and it thus remains the method of choice for conducting scientific studies of the course of treatment over time. In addition possibilities of further developing and enhancing the method are presented.

[Treatment of endometriosis with the GnRH agonist buserelin (Suprecur): a multicenter study].

A phase III multicentre study was carried out with the GnRH agonist buserelin (Suprecur) at five university institutions (Dept. of Obstetrics and Gynaecology of Essen, Kiel, Tübingen, Münster and Erlangen) to evaluate the effect of Buserelin (3 x 300 micrograms daily intranasal) on endometriosis for a period of 6 months. Endometriosis was verified by light microscopy in all cases. The data of 430 patients could be evaluated. The extent of the endometriotic lesions was studied before the start and at the end of therapy. The mean age of the patients was 30 +/- 5 years. For 300 patients (66.4%) sterility was a main problem. Previous treatment for endometriosis consisted of operation in 20.5%, treatment with danazol in 16.8% and treatment with gestagens in 11.6% of the patients. Under treatment FSH changed from 11.0 +/- 31.4 mIU/ml to 7.0 +/- 5.6 mIU/ml at the end of therapy; LH from 11.1 +/- 12.9 mIU/ml to 7.4 +/- 7.3 mIU/ml; estradiol from 113.9 +/- 89.0 to 35.9 +/- 60.3 pg/ml and progesterone from 7.2 +/- 11.2 ng/ml to 0.8 +/- 1.4 ng/ml. The effect on endometriosis was judged according to the revised American Fertility Society score (rAFS 1985). The changes of the rAFS-score stages were as follows (before/after therapy): Stage 0: 0%/42.8%; stage I: 43.5%/38.5%; stage II: 31.1%/11.2%; stage III: 18.1%/2.2%; stage IV: 6.3%/0.2%. The total mean AFS-stage changed from 1.9 to 0.7. The frequency of clinical symptoms improved (before/after therapy): dysmenorrhoea 65.0%/11.2%; pelvic pain 48.2%/14.1%; pain at the time of gynaelogical examination 40.0%/9.7%.(ABSTRACT TRUNCATED AT 250 WORDS)

Microradiography of the feline marginal periodontium with a microfocal high-resolution x-ray system.

The present paper is a cross-sectional microradiographic study. Postmortem hemimandibles were radiographed with a microfocal high-resolution method. Microradiographic appearances of the feline periodontium are described and discussed. Mean relative alveolar bone support in clinically and histologically normal cats was 46.9%. Site-specific alveolar bone support (ABS) was calculated twice (blind). The 95% confidence limits of the method error for one observation were estimated. Differences between two site-specific measurements did not exceed +/- 2.72%. Microfocal radiography yielded valuable additional information about the feline periodontium and made it possible clearly to identify important dental and periodontal structures such as the CEJ (cementoenamel junction) and the bottom of infrabony defects.

The international collaborative study of maternal phenylketonuria: status report 1994.

Neonatal screening for phenylketonuria (PKU) has created a problem as females with PKU are reaching child-bearing age. Surveys have revealed that maternal phenylalanine blood concentrations greater than 1200 mumol/l are associated with fetal microcephaly, congenital heart defects and intrauterine growth retardation. It is estimated that as many as 3000 hyperphenylalaninemic females may be at risk of producing these fetal abnormalities. To examine this problem, the international maternal PKU collaborative study was developed to evaluate the efficacy of a phenylalanine-restricted diet in reducing fetal morbidity. Preliminary findings have indicated that phenylalanine restriction should begin before conception for females with PKU planning a pregnancy. Dietary control should maintain maternal blood phenylalanine levels between 120 and 360 mumol/l and should provide adequate energy, protein, vitamin and mineral intake. Pregnant hyperphenylalaninemic females who achieved metabolic control after conception or by the 10th week of pregnancy had a better offspring outcome than anticipated. The results of 402 pregnancies are reviewed.

Speech enhancement based on physiological and psychoacoustical models of modulation perception and binaural interaction.

A novel approach for analyzing and filtering speech is described and evaluated which utilizes the "modulation spectrogram," i.e., the two-dimensional representation of modulation frequencies versus center frequency as a function of time. This approach is based on physiological findings of a tonotopical organization of modulation frequencies perpendicular to carrier frequencies as well as psychoacoustical findings of "modulation tuning curves." In addition, an interaction is assumed between the representation of modulation frequencies and the representation of auditory space as described by physiological and psychological models of binaural hearing. A noise-reduction algorithm based on this approach was implemented and tested which enhances or suppresses each combination of modulation frequency and center frequency according to its phase and intensity relation between the two input signals (i.e., both stereo channels of a dummy-head recording). When tested in several situations with interfering speakers and background noise both in anechoic and reverberant environment, the algorithm provided a small but a very robust increase in speech intelligibility which corresponds to approximately 2 dB in signal-to-noise ratio. Possible applications of this algorithm are noise reduction in adverse acoustical situations, digital hearing aids, processing schemes and preprocessing for speech recognition.

[Rapid temperature measurement by calculating the final temperature within the response time].

For the early detection of an epidemic among animals in mass production, rapid information about the temperature of the animals is important. The response time of conventional thermometers is too long to permit rapid measurement. For adequate accuracy the measuring time required is about five times the response time. Using a microcomputer and regression computation, measurement of the final temperature can be effected within the period of a single response time. In addition, several thousand data can be stored for evaluation at a later date.

[Preventive-curative dental care in a large East German urban population. The 4-year experiences of a Dresden prevention study with 16- to 35-year-old subjects].

The present 4-year Dresden preventive study was carried out to realize a dental care concept with preventive emphasis in 6 Dresden state health services. 2502 patients aged 16-35 years were randomly selected in 1988. The effectiveness of this study was evaluated by means of the parameters oral health behaviour, oral health state and preventive care needs. The spectrum of methods included extended anamnesis, plaque index according to Silness & Löe, DMF/T, DMF/S, GPM/T index, CPITN and saliva tests in case of high caries risk. The dental care based on a basic and a risk strategy. The basic strategy included a complex symptomatic dental treatment and a dental prophylaxis, repeated once every 3 month. The patients were given instruction, demonstration, remotivation, professional oral hygiene measures and topical application of fluorides. The 4-year preventive intervention resulted in an improved consultation and oral hygiene behaviour. The nutritional behaviour did not change essentially. Within the cohort plaque index decreased from 1.01 to 0.68, the GPM/T index from 11.7 to 10.4 as well as the preventive care needs. It was found a low caries increment. The care concept with preventive emphasis is recommendable by its effectiveness.
