Vesical exstrophy with epispadias. Twenty-year follow-up.

A case we believe to be the longest with successful anatomic closure of exstrophy and epispadias witholt reflux or incontinence is reported. The persistence of the squamous metaplasia in the bladder despite twenty years of "turn-in" is noteworthy. The original method of closure reveals no "secret" to account for the success. The method of repair of traumatic rupture is presented.

Stereopsis and the representation of space.

Four stereoblind and four normal subjects were tested on a mental rotation task. It was hypothesized that, if stereopsis is an important input for building up the perceptual system that represents three-dimensional space, then subjects lacking it ought to be deficient at mental rotations in depth. Stereoblind subjects were equally efficient at picture-plane and depth rotations, and were nonsignificantly better than normal subjects at rotations in depth. It was concluded that in the absence of stereopsis other cues are sufficient for the development of the "three-dimensional" perceptual system. A puzzling paradox was raised, however, by the finding that the introspections of the two groups differed markedly.

[Isolation, crystallization and partial characterization of a cationic protease from thermoactinomyces vulgaris].

A simple method was developed for the isolation of the cationic endopeptidase from a crude extract prepared from the culture medium of Thermoactinomy ces vulgaris, consisting in chromatography on Sephadex G 75 and subsequent separation on CM-Sephadex in 50 mM Tris/HCl, pH 8, using a NaCl-gradient (0 - 0,2 M). This procedure results in a 4,2 fold increase of the elastolytic activity (substrate: N-acetyl-(L-ala)3methyl ester) of the enzyme. It moves as a single band in SDS-gel electrophoresis and crystallizes as needles up to 0,2 mm in length after standing at 5 degrees C in 50 mM Tris/HCl (pH 8,3).

[Differentiation of the proteinuria by microelectrophoresis in continuous polyacrylamide-gradient gels (author's transl)].

It is possible to differentiate the proteinuria of glomerulonephritis by means of microelectrophoresis in polyacrylamide-gradient gels. The advantages of this method (quick, cheap, concentration of the urine not required) led to its introduction into clinical use. Upon separating the urinary proteins according to their molecular weight and form, four patterns of proteinuria may be differentiated: low molecular, intermediate and high molecular. Those forms of glomerulonephritis which show constant morphological and clinical findings (e.g. minimal proliferative glomerulonephritis, mesangioproliferative glomerulonephritis with crescents) can be related to one of the four patterns of proteinuria, whereas the pattern of proteinuria in other forms of glomerulonephritis (e.g. (peri-)membranous glomerulonephritis, mesangioproliferative glomerulonephritis) are dependent on the phase of the disease.

Paraurethral leiomyoma--an imitator.

In location and clinical appearance a paraurethral leiomyoma imitated other disease in 2 patients. Correct preoperative diagnosis of these lesions can avert therapeutic error. Diagnostic clues include a solid, noncystic mass that does not communicate with the urethra, is circumscribed, and has essentially normal overlying mucosa. A surgical approach through the anterior vaginal wall avoids urethral injury and enucleation is curative.

Experience with a direct reading dedicated fluorometer for determination of erythrocyte protoporphyrin.

Data are presented for erythrocyte protoporphyrin (EP) concentrations determined by the direct reading tecnhique of Lamola et al. (1) Mean concentration of EP in children whose blood lead concentration was less than 30 mug/dl was 34.7 mug/dl whole blood +/- 13.4 (SD). Mean EP concentration in children with blood lead concentrations of 40-59 mug/dl was 80.0 mug/dl. It was 158 mug/dl. It was 158 mug/dl for children with blood lead concentrations greater than 59 mug/dl. All children in this last group had concordant values for Pb and EP on the first or second test.

Prenatal diagnosis of heterozygous beta-thalassemia.

The parents of a child with homozygous thalassemia of the beta O variety requested prenatal diagnosis during a subsequent pregnancy. At the 19th week of pregnancy, a sample of blood containing fetal cells was obtained by placentocentesis. Radiochromatography of globin chains demonstrated production of a beta-chain with a beta/gamma synthetic ratio of 0.049, which is low for this gestational age. The conclusion that the child would be heterozygous for the beta-thalassemia gene was confirmed after birth.

Ocular alkali burns in a large urban area.

A retrospective study of 63 ocular alkali burn victims revealed that black males in the 30 to 49 year age group were at highest risk, and that the burns were most commonly sustained via assault. One hundred eyes were involved, and in 38 cases there was bilateral involvement. There were 116 hospital admissions and 125 surgical procedures, yet 58% of the injured eyes had a final visual acuity of less than 20/200. Other demographic and socioeconomic factors were characterized to identify this event as a public health problem with well-defined risk factors and unsatisfactory treatment. Primary preventive measures are, therefore, necessary.
