Differential diagnosis importance of the creatine kinase isoenzyme pattern in delirium tremens.

Total CK and CK-MB (by the inhibition test, Merck-1-CK-MB) were measured in 33 patients which had to be admitted to the department of psychiatry due to acute withdrawal symptoms (predelirium, delirium) caused by chronic alcohol abuse. The results were evaluated together with the routinely performed laboratory determinations as well as clinical examination and morphologic investigations (ECG, X-ray examination of the thorax, EEG, CT). The results show that the CK-MB/total CK relationship represents also in these patients the most reliable parameter to discriminate elevated total CK values. In no but one case a substantial CK-BB release from the central nervous system could be demonstrated by the inhibition test. In 83% of the patients with elevated total CK activities it was possible to exclude an affection of the heart muscle (CK-MB portion below 6%). The CK-MB/total CK-quotient was clearly superior to the total CK/GOT relationship, by which a myocardial affection could be excluded in less than 40%. In four patients the CK-MB portion was above the critical decision limit of 6%: three obtained between 6% and 7%, one even 17%. The results suggest that the CK-MB/total CK-quotient may represent the most sensitive and reliable parameter of myocardial affection in patients with an alcohol intoxication and/or a delirium tremens.

Further evidence for the superiority of a 3-compartmental model for distribution analyses of i.v. injected bile acids in men: studies in patients with liver diseases.

In 15 patients (n = 6 without and n = 9 with liver diseases) we measured the plasma disappearance curves after single injections of C 14-glycocholate, calculated the distribution and excretion kinetics on the basis of a 2- and 3-compartmental model and compared the calculated values for the maximal C 14-accumulation within compartment 2 that should correspond to the liver in both compartmental models. The results demonstrate that only the 3-compartmental model reproduces the pathophysiological situation in the patients with liver injuries, namely a decrease of the maximal C 14-accumulation within the liver and an increased C 14-accumulation within the extrahepatoplasmatic compartment. The results therefore confirm our previous studies in healthy patients even for the patient with liver diseases.

Effects of cholecystokinin-pancreozymin on bile flow and biliary bile salt secretion in the cholecystectomized animal.

The effects of an intravenous infusion of cholecystokinin-pancreozymin on bile secretion were studied in 13 unanesthetized and cholecystectomized piglets. The results demonstrate that CCK-PZ induces a significant increase of bile flow and bile salt excretion. The excretion patterns for bilirubin and Ioglycamide, however, showed a slight but not significant decrease. Peripheral hemodynamic parameters remained unchanged. A detailed analysis of these results suggests that CCK-PZ induces an increase of biliary bile salt secretion by stimulation of intestinal motility and enhancement of bile salt circulation within the enterohepatic circulation (EHC). In combination with previous studies on the effects of spasmolytics on biliary bile salt excretion these results therefore emphasize, that the intestinal motility and transit time might play a role in the physiology and pathophysiology of biliary bile salt secretion.

[Dose-effects of the contrast medium Iotroxinate on biliary excretion of contrast media during infusion cholegraphy (author's transl)].

In the anaesthetized pig we studied the influence of the dosage of the contrast medium Iotroxinate on the following parameters of bile secretion: bile flow, bile salt excretion, concentration, and excretion of Iotroxinate. The results demonstrate that high doses of Iotroxinate inhibit the bile salt independent fraction of bile flow in a dose-dependent manner, by analogy to organic anions like Rose Bengal, Indocyaningreen, Ioglycamite. The biliary concentration of Iotroxinate however increases slightly. That means, that high doses of Iotroxinate do not impair the quality of opacification of bile ducts in x-ray examinations, as the contrast medium concentration represents the decisive parameter for a valid cholegraphy.

Inhibition of bile salt excretion and bile flow by spasmolytic drugs. An additional therapeutic effect in the treatment of biliary colics?

The influence of the spasmolytic drugs hyoscin-N-butylbromide (HBB, Buscopan) and Extractum opii (E. opii) on bile flow, bile salt and bilirubin excretion is investigated in the cholecystectomized, unanaesthetized pig and compared with the results after acute, quantitative interruption of the enterohepatic circulation (EHC) by biliary drainage. The results demonstrate that HBB and E. opii induce a significant decrease in bile flow and bile salt excretion. A further analysis suggests that this effect is due to an inhibition of intestinal motility and transit time, followed by a delay of the EHC of bile salts with a decrease in biliary bile salt excretion and bile salt dependent bile flow. The results point out a second mechanism that might be responsible for the effectiveness of spasmolytic drugs in the treatment of biliary colics: the lowering of intraluminal pressure by inhibition of bile salt dependent bile flow.

[Caffeine and bile secretion. Experimental studies in the unanaesthetized piglet (author's transl)].

We investigated the distribution and excretion of 8-14C-caffeine as well as the effects of caffeine on bile secretion in the unanaesthetized piglet after intravenous and intraduodenal application. The findings confirm a) data known from other species concerning the intestinal absorption, distribution and excretion of 8-14C-caffeine, b) the delayed plasma disappearance of 8-14C-caffeine known in the pig following an initial rapid disappearance rate and c) a hydrocholeresis, so far only known from rats. However, the increase in bile flow accompanied by an increase of urine flow was noticed not before 3--4 h after caffeine application. Therefore the question arises whether this increase is caused by caffeine itself or by its metabolites. On the basis of our results and those of the literature it is attempted to answer this question.

[Studies on the biliary excretion of the hepatotropic contrast media ioglycamate and iotroxinate in pigs and men (author's transl)].

The distribution and biliary excretion of two hepatotropic contrast media, Ioglycamate and Iotroxinate, has been studied in the anaesthetized pig in infusion experiments. The new substance Iotroxinate exceeds the Ioglycamate in two of the investigated parameters: it was excreted faster and reached a higher biliary concentration. The biliary transport maximum, however, was unchanged. On the basis of these experiments the new contrast medium Iotroxinate was tested in men.

[Cholangiography and diuretics (author's transl)].

Contrast media intended for use in cholangiography are partly eliminated heterotropically via the kidneys. This can create difficulties in properly evaluating a cholangiogram. To overcome this problem, other protections are used, especially tomography (2, 3, 4, 5). This paper discusses of a third possibility, namely, whether the contrast medium in the renal calyces can be diluted by means of a diuretic, and so eliminated as a disturbing factor. The condition for this is, however, that excretion of the contrast medium via the bile ducts is not influenced in any way. This problem is investigated both experimentally and by means of a clinical study.

Necessity of a 3-compartmental model for analyzing the distribution and excretion kinetics of i.v. injected bile acids in man.

The distribution and excretion of i.v. injected c14-glycocholate (n = 8) and c14-cholate (n = 5) was measured in the serum, duodenal juice and urine in 13 patients in order to examine the possibilities to reporduce the measured data by a multi-compartmental analysis. The results demonstrate that a 3-exponential fitting and therefore a 3-compartmental model represents at present the most appropiate compartmental model for analyzing the distribition and excretion kinetics of the injected bile acids by mathematical procedures. In extending the previously described 2-compartmental model it encloses the 3-compartments plasma, liver and extrahepatoplasmatic space and allows a precise calculation of important physiological parameters, for example of the hepatic (V1/V2-clearance) and extrahepatoplasmatic (V1/V3-clearance) clearance, the biliary excretion and the distribution between the 3 compartments.

[Progress in the differential diagnosis of myocardial infarction by determining isoenzyme CK-MB with inhibitory antibodies (author's transl)].

Sensitivity and value of the quantitative measurement of the isoenzyme creatine kinase-MB by means inhibitory antibodies were studied in a number of experiments. The results indicate that concentrations of more than 3 U/1 are abnormal: they were found only in patients with myocardial cell damage but not after intramuscular injection or physical exercise. Determining CK-MB apparently offers a better way of assessing low or moderate increases of total CK activity than, e.g., the CK/GOT ratio. The method should therefore be introduced into clinical diagnosis. But the question of whether CK-MB measurement can differentiate between myocardial infarction and reversible myocardial cell damage remains unanswered: it is not 100% specific for myocardial infarction.
