[A rare etiology of portal hypertension in an adult: congenital hepatic fibrosis. Review of current literature in connection with an observation (author's transl)].

On the basis of a personal observation the authors review the current literature on the hypertensive forms of congenital hepatic fibrosis found in an adult subject during G.I. haemorrhage through the rupture of gastro-oesophageal varices. These observations seem to be exceptional and 17 cases were recorded between 1956 and 1976. A complete hemodynamic portal study was carried out only in six cases. It shows a presinusoïdal portal hypertension in three cases, post-sinusoïdal or sinusoïdal portal hypertension in two cases, mixed hypertension in one case. Our observation is outstanding because of the existence of a rise of the wedged hepatic venous pressure varying from one hepatic vein to the other. The prognosis seems to be relatively bad for the short-term as the mortality rate of 2 years reaches at least 29%.

[Surgical correction of interventricular communication with pulmonary stenosis in a hemophiliac].

Open heart surgery was performed in a 5 year-old boy with severe hemophilia A and large ventricular septal defect with pulmonary stenosis. High doses of Factor VIIIC superconcentrates delivered as small transfusion volumes allowed the use of extracorporeal circulation with heparin. Transfusions began 5 hours before surgery and were stopped 21 days later. No adverse reaction was observed. Thus major surgery is possible in severe hemophiliacs provided strict rules for replacement therapy are followed.

Arterial emboli of the upper extremity: a persisting problem.

From 1969 to 1980, 154 emboli of the upper extremities (UEE) were managed in our institution. They occurred in 151 patients, 66 males (44%) and 85 females (56%) aged ranged from 23 to 87 years (mean 68.9). Obstruction level was in the subclavian artery (12 cases), the axillary artery (46), the brachial artery (74 cases), the radial or ulnar artery (22 cases). Cardiac disease was present in 119 cases. Ischemia was severe in 47 cases (30%), and partial in 104 (67%). Peripheral gangrene was already present in 2 cases. 106 patients were operated on within 10 hours after the occurrence of UEE (69%). Associated systemic emboli was noted in 10 cases. The treatment consisted in 140 primary embolectomies and in 11 cases of conservative therapy (heparin 10, thrombolysis 1). Reoperation was done in 11 patients (7%) because of recurrence of UEE and in 4 cases (2%) for post-operative thrombosis. Results were good in 128 patients with clinical improvement and arterial patency.

The pig as a model in liver xenotransplantation.

Xenotransplantation is considered to be the best solution to the critical shortage of human donors. Despite its phylogenetic distance, the pig appears to be the most appropriate source of organs for transplantation in humans. Its anatomy and physiology are similar to that of man, it can be raised in specific pathogen free environments and it is available in large quantities. The immunological barrier remains to be overcome, however. Considerable progress in the pathogeny of xenogenic rejection has led to the development of therapeutic strategies and the prospect of clinical xenografting appears realisable in the near future.
