[The effect of combined respiration physiological measures on pulmonary gas exchange and ventilation].

The immediate effect of physiotherapy and the effect of IPPB-inhalations in 18 patients with chronic obstructive lung disease and global respiratory failure have been investigated and discussed. The procedures (physiotherapy or IPPB-inhalations) were tested singly and in combination. Better results are obtained when physiotherapy is performed before IPPB-inhalation. A significant decrease in pCO2 and a significant rise in VC and FEV1 are observed, with unchanged pO2.

[Respiratory insufficiency in acute lung diseases].

The diagnosis of respiratory failure is based on pathological findings in arterial blood gas analysis. Thus, even in emergent cases with acute pulmonary disorders, rapid differentiation of the amount and nature of the respiratory impairment is possible. Furthermore, evaluation of the blood gas analysis may be an essential factor in the choice of therapeutic procedures. Hypoxia should be treated by strictly controlled oxygen administration, to avoid further respiratory depressant effects. Patients with acute and progressive respiratory acidosis usually need artificial ventilation by volume- or pressure-cycled respirator. Some important modifications of ventilation are recommended with a view to improving pulmonary gas exchange and decreasing the incidence of complications, especially during the long-term treatment with respirators.

Influence of lung resection on pulmonary circulation and lung function at rest and on exercise.

Since June 1972, preoperative evaluation of the pulmonary hemodynamics and lung function tests at rest and on exercise were done in patients undergoing thoracotomy with lung resection. 49 patients have been investigated postoperatively, 28 patients had pre- and postoperative rightsided heart catheterization. 80% of the patients were feeling well and more than 50% were working again. The pulmonary hemodynamics showed postoperatively already at rest an increase of the pulmonary artery pressures, the cardiac index reached on exercise no longer the preoperative values and there was an increase of PCV pressures on exercise compared to preoperative. Stroke volume, heart rate, AV difference for oxygen, oxygen uptake, PaO2, V, VD/VT, AadO2, and diffusing capacity for CO steady state on exercise showed no statistical significant changes. The extent of lung resection showed no different influence on the pulmonary circulation for the time period investigated.

Neurolept analgesia for bronchoscopic examinations.

In 38 patients requiring diagnostic bronchoscopy, three different procedures of anesthesia have been investigated. Of the three, neurolept analgesia combined with topical anesthesia of the airway proved by far the best form. It is followed by psychic indifference without lessening the patient's voluntary control, thus facilitating intricate technical manipulations on the bronchial system. No severe side effects arise when suitable equipment for diagnosing and treating respiratory and cardiovascular depression is available.
