This multicenter double-blind study of 157 patients was designed to evaluate the safety and efficacy of a new dosage regimen of oral acyclovir (800 mg twice a day for 5 days) for the treatment of recurrent genital herpes. The new regimen was compared with the standard dosage of 200 mg given five times a day to patients treated for three consecutive episodes (one episode with physician-initiated therapy and two with patient-initiated therapy) and followed up for three untreated episodes. The new regimen was well tolerated and effective. In male patients it may be more effective than the standard regimen of 200 mg given five times a day to treat vesicles that are already present.
Thirty-five of 44 nonunited scaphoid fractures that were at least 6 months old healed in a mean time of 4.3 months during pulsed electromagnetic field (PEMF) treatment using external coils and a thumb spica cast. The mean time from the onset of the fracture to treatment was 40 months. No concurrent operation was performed. Follow-up time averaged 8.4 months. Eight of nine fractures with avascular necrosis healed. Five of eight fractures in the proximal third healed. Twelve (75%) of 16 patients treated in short-arm thumb spica casts and PEMF healed versus 22 (92%) of 24 patients treated initially in long-arm thumb spica casts and PEMF. We have found PEMF to be a reliable alternative method of treating nonunited scaphoid fractures. Because of the low risk, simplicity of use, and reliability, we recommend its consideration in the treatment of undisplaced, nonunited fractures without carpal instability less than 5 years after the injury. Treatment should initially begin with a long-arm cast.
Abnormalities in type I collagen have been recognized in a number of connective tissue disorders. In the Marfan syndrome, an autosomal dominant condition producing a generalized abnormality in connective tissue, no consistent abnormality has been identified, although one individual has been found to have an elongated pro alpha 2(I) collagen chain [Byers et al, Proc Natl Acad Sci USA 78:7745, 1981]. To determine the nature of the alteration in the gene that produced this abnormality, we studied the pro alpha 2(I) gene from this individual by genomic blotting and gene cloning. Genomic mapping studies detected no abnormalities. However, analysis of the cloned segment of the pro alpha 2(I) collagen gene from the Marfan individual indicates that the gene contains a 38 base pair insertion in an intron near the collagenase cleavage site. Although the relationship of this insertion to the protein abnormality is unclear, it may be a useful marker for the diagnosis of the Marfan syndrome.
Septic olecranon bursitis developed in three patients with systemic lupus erythematosus. Although promptly treated with systemic antibiotics, the clinical course in two patients became prolonged and eventually required bursectomy to eradicate the infection. The occurrence of septic bursitis was a manifestation of the general susceptibility of SLE patients to infections.

This report extends the association of herpes simplex virus type 2 (HSV2) with cervical cancer to vulvar carcinoma. In addition to seroepidemiologic observations linking prior HSV2 infection to genital cancer, the new evidence involves the neoplasms themselves. Biopsies of cervical and vulvar cancers were found to contain HSV2-specified antigens. The virus-induced antigens are nonstructural DNA-binding proteins normally present in the nuclei of infected cells; however, in the cancer cells they were found in the cytoplasm. Whole virus structural antigens were not detected in the biopsies. In recent years there has been a parallel rise in prevalence of HSV2 infections and vulvar carcinomas, particularly in women under 40 years of age, reaffirming the association of the virus infection and the neoplasms.

The modalities used to treat 27 male patients with metastatic breast cancer at Memorial Hospital from 1957--1979 are reviewed. The overall objective orchiectomy response rate was 48% (11/23). The median response was 11 months (6--96 months). The median survival in responders was 58 months, whereas the median survival in orchiectomy failures was 24 months. Four patients had estrogen receptor protein determinations (ERP). Two positive ERP patients and one borderline ERP patient responded (18--34 months). One ERP-negative patient failed orchiectomy. The number of major ablations in this series (two responses/two adrenalectomies, two failures/two hypophysectomies) does not permit conclusions, although review of the literature suggests that major ablation may be valuable in patients responding to orchiectomy. Two of three patients who refused ablative therapy responded (three months, ten months) to single agent provera. Administering stilbesterol to the ten-month responder and to the patient who failed provera resulted in two additional responses (four months, continuing, and 22 months). Of six patients who failed orchiectomy and subsequently received chemotherapy, four patients responded (7--40 months). The median survival in these patients was 40 months (33--44 months). This review supports the importance of orchiectomy in the treatment of metastatic male breast cancer. Although the numbers are small, these data suggest that estrogen receptor determinations may be useful in selecting patients for orchiectomy. Selective additive hormone therapy may be useful in patients who refuse orchiectomy. Chemotherapy may provide worthwhile palliation in patients who fail orchiectomy.
The quantitative tissue specificity of the high mobility group (HMG) chromosomal proteins was investigated. Perchloric acid (PCA) extracts of four different chicken tissues and erythrocytes contained three proteins which comigrated on NaDodSO4-polyacrylamide gels with the HMG's 1,2, and E from erythrocyte nuclei. These three HMG's from embryonic skeletal muscle and erythrocytes also comigrated on two-dimensional gels, employing an acid-urea system in the first dimension and an NaDodSO4 system in the second. Interpretation of the two-dimensional gels suggested that the two low molecular weight proteins of this triplet arose from the HMG 2 band of the acid-urea gels. These have been designated HMG 2A and HMG 2B. Three proteins of similar molecular weights were also found in the PCA extracts of calf thymus. They were arranged in a similar but not identical pattern on two-dimensional gels. Thus, these three HMG's appear to be neither tissue nor species specific. In addition, the 2.0% PCA extracts of all chicken tissues examined contain a 38 000-dalton (38K) nuclear protein which coisolates with the HMG's. These four proteins are found in different relative amounts in each of the four chicken tissues and erythrocytes. They are found in the same relative amounts, however, in embryonic skeletal muscles from different chicken strains with widely different highly repetitive sequence content, suggesting that none of these individual proteins is selectively localized to constitutive heterochromatin. The quantitative tissue specificity of the HMG's and the 38K protein, however, suggests that they may participate in regulating cell-specific gene expression.

A patient who had been treated for Hodgkin's disease was infected with herpes zoster. Shortly thereafter, glomerulonephritis mediated by anti-glomerular basement membrane (anti-GBM) antibody occurred. The diagnosis was confirmed by the presence of circulating anti-GBM antibody, immunofluorescence, and elution. Results of immunofluorescence of a lymph node disclosed deposits of IgG on the vascular and stromal connective tissue, in a pattern similar to that seen when heterologous anti-GBM antibody reacted with the node. Because of the known nephritogenic potential of solid lymphoid tissues, this finding suggests a possible relationship between his tumor and his glomerular disease. The role of zoster infection remains undefined.
