One hundred and fifty cases of Kala-azar were studied for evidence of hepatic involvement. The hepatic function was mildly affected in 25 cases and 3 cases had fulminate hepatitis. Most of the cases were cured after anti-Kala-azar therapy except 2 cases, who died of hepatic failure. This study suggests that fulminant hepatitis may be the outcome of Kala-azar, itself.
Six different secretory proteins of molecular weights (15, 26, 30, 41, 55 and 70 kDa) were isolated from 8-day-old culture filtrate of Mycobacterium tuberculosis H37Ra using different column chromatography techniques. These proteins were further examined for their ability to induce cell mediated (T-cell proliferation assay) and humoral immune response (ELISA) in mice immunized with total culture filtrate proteins. Out of six proteins, three proteins showed good reactivity. However, the activity was at a maximum with 30 kDa antigen. The immune response induced by 30 kDa antigen emulsified in Freund's incomplete adjuvant (FIA) was investigated and was found to be dose dependent. The T-cell response induced by this protein was skewed towards T-helper (Th1) cells as determined by the pronounced secretion of interleukin-2 (IL-2) and gamma-interferon (IFN-gamma). The protective activity of the 30 kDa protein was also evaluated and compared with reference to Bacillus Calmette Guerin (BCG) vaccine in the mice challenged with virulent M. tuberculosis H37Rv. The degree of protection afforded by the 30 kDa antigen on the basis of mortality and the significant decrease in c.f.u.'s recovered from different organs (lung, liver, spleen) after 30 days of challenge with LD50 of M. tuberculosis H37Rv was significantly higher in comparison to BCG vaccinated animals. However, the degree of immunity induced by this antigen decreased with time (when challenged 8 and 12 weeks post-immunization) but it was still comparable with BCG. These findings suggest that 30 kDa secretory protein of M. tuberculosis is the key immunoprotective antigen and may be a suitable candidate for the development of an alternative subunit vaccine against tuberculosis.
Free and ethidium bromide (EB) complexed alkali denatured T7 DNA molecules were renatured at 58 and 62 degrees C respectively for 1--3 h. The structures of the renaturation products of the free candidates were as usual showing native like, branched and unrenatured DNA whereas the structures of the ethidium bromide complexed one were somewhat different, showing non-renatured loop-like and entangled regions present inbetween the renatured segments. On the basis of the linear base sequence of T7 DNA, these non-renatured parts are indicative of the inhibition of renaturation by the complexed EB molecules. Mapping of the non-renatured regions showed that they were present at some specific sites, which inturn suggested that the EB-binding has some base sequence specificity.
Success of Universal Salt Iodization (USI) programme depends on availability of iodised salt to consumers, which should be reflected in their body iodine status. From a monitoring study in Calcutta, it was found that all packed salts were iodised and most of them (98.1% at household level and 93.6% at retailers' outlets) had iodine at a level of > or = 15 ppm. Of the loose salts, 34.6% at household level and 19.9% at retailers' outlet had iodine level < 15 ppm. A few number (0.5% at household level and 1.0% at retailers' outlets) of salts had no iodine. To ascertain the impact of consumption of iodised salt iodine excreted in urine (UIE) was measured in school children of age between 8-12 years of south, east, west, north and central parts of Calcutta. 22.95% of male children and 31.81% of female children had urinary iodine level less than 50 micrograms/l, which is cut off figure of public health concern. Children from poor slum areas were found to be at greater risk.
The study of hydration power of stratum corneum of lesions show highly significant poor water-uptake at low temperatures (p less than 0.001), a defect not recorded after removal of water soluble fractions. Secondly, the lesions show significant poor water-diffusive power (p less than 0.001). The findings suggest a qualitative alteration in the stratum corneum of leprosy patients, probably in its water-soluble protein fraction.
Two cases of leprosy--one Tuberculoid and the other Borderline are reported. The Tuberculoid patch shows sharply defined hyperpigmented border whereas in the Borderline patient, diffuse hyperpigmentation is present around the lesion. The cases are reported for documentation due to rare clinical pressentation.
