Development of squamous cell carcinoma of the esophagus after endoscopic variceal sclerotherapy.

We describe the case of a 45-yr-old white male with portal hypertension and presumed Laennec's cirrhosis who developed squamous cell carcinoma of the esophagus 8 months after completion of a course of endoscopic variceal sclerotherapy. The epidemiology and natural history of esophageal cancer and their relationship to our patient are analyzed. This report emphasizes that squamous cell carcinoma of the esophagus should be considered in the differential diagnosis of postsclerotherapy dysphagia. Further studies will be required to determine whether or not esophageal variceal sclerotherapy is associated coincidently or causally with the development of squamous cell carcinoma of the esophagus in patients at increased risk for this condition.

Hepatobiliary complications of inflammatory bowel disease.

A variety of biliary and hepatocellular diseases occur with increased incidence in patients with inflammatory bowel disease. These include fatty infiltration of the liver, cholelithiasis, pericholangitis-primary sclerosing cholangitis, cirrhosis, chronic active hepatitis, liver abscess, amyloidosis, granulomatous hepatitis, and bile duct carcinoma. Radiography is essential in accurate diagnosis.

Clinical application of in vitro studies for barium-enema examination following colorectal biopsy.

A total of 150 colorectal biopsies were performed in 71 patients and correlated with the barium-enema examination (BE). The investigation was divided into two parts. In the first part, involving 19 patients, the depth of each biopsy specimen was determined histologically prior to BE and potential complications related to the biopsy-BE sequence were evaluated. In the second part, involving all 71 patients, the depth of biopsy specimens taken from various colon lesions was determined histologically without regard to the timing of BE. The results confirm previous in vitro findings in pigs and dogs, namely: (a) BE may be performed without delay following a superficial biopsy, but should not be performed until at least 6 days after a deep biopsy; (b) transcolonoscopic biopsies are likely to be superficial, while transproctoscopic biopsies have the potential to be deep; and (c) BE should be delayed at least 6 days following polypectomy or polyp biopsy performed with electrosurgery.

Radiologic features of multinodular lymphoma of the colon.

The colon is frequently involved in patients with systemic malignant lymphoma and may be the site of initial clinical presentation. Multiple nodules are one of the manifestations of colonic lymphoma. In an effort to determine the most useful and characteristic features, the radiographic findings in 23 patients with multinodular colonic lymphoma were reviewed. The most characteristic radiographic features were identified: (1) variably sized, smooth, sessile nodules with an average diameter of 7 mm; (2) a cecal mass larger than 3 cm; (3) distortion of the haustra ; (4) incomplete evacuation of barium; and (5) involvement of the stomach, small bowel, or spleen. Less common were irregular, pedunculated, or filiform nodules and occasional umbilication . The differential diagnosis includes gastrointestinal polyposis, benign lymphoid hyperplasia, and inflammatory bowel disease. The radiologist may be the first to suggest the correct diagnosis when lymphoma patients present with multiple colonic nodules.

Papillary carcinoma of the thyroid and familial polyposis coli.

A 22-year-old white woman in whom multicentric papillary carcinoma of the thyroid developed two years after prophylactic colectomy for intestinal polyposis is reported. This association has been observed by others. Patients with familial polyposis coli are at risk for a variety of malignancies other than colonic, and careful life-long surveillance is necessary.

Value of pneumoperitoneum in the diagnosis of visceral perforation.

The presence or absence of pneumoperitoneum may not be as reliable an indicator of visceral perforation as commonly thought. Visceral perforation as commonly thought. Visceral perforation resulted in pneumoperitoneum in only 51 percent of patients in this study. Pneumoperitoneum occurred in 14 percent of patients in whom the extraalimentary intraperitoneal air had sources other than a perforated viscus. These patients' clinical findings were often indistinguishable from those of patients with a perforated viscus, and three patients underwent celiotomy unnecessarily. The routine use of the left lateral decubitus film to detect pneumoperitoneum and the judicious use of gastrointestinal contrast studies should be part of the optimal management of patients with suspected visceral perforation.

Work in progress: transverse striations of the esophagus: association with gastroesophageal reflux.

Transverse mucosal ridging of the esophagus may be demonstrated on double-contrast esophagrams as transverse striations. A case review of 44 patients who had transverse striations was performed to evaluate the significance of the striations with respect to gastroesophageal reflux. Gastroesophageal reflux was significantly more common among patients who had transverse striations than it was in the general population who had been referred for the barium meal. This does not imply that all patients with transverse striations or with reflux at fluoroscopy have esophagitis. However, recognition of transverse striations should prompt careful evaluation for reflux and associated complications in symptomatic patients.

Barium enema examination following biopsy of the rectum or colon.

A four-component study was designated to evaluate the possible risk of perforation during barium enema examination following biopsy of the rectum or colon. The study included a survey of gastrointestinal radiologists, an in vitro study of pig colons after biopsy, colon pressure measurements in human subjects, and histological analysis of human colonoscopic biopsy specimens. The results suggest that superficial biopsy of the rectum of colon performed with small colonoscopic forceps is a safe procedure and no waiting period is necessary before performing a barium enema study. However, biopsy of the colon or rectum with proctoscopic forceps appears to have greater potential for perforation and a waiting period between biopsy and barium enema study is recommended. Radiologists should distinguish between proctoscopic and colonoscopic biopsy procedures before determining when a barium enema study will be performed following biopsy.

Gastroesophageal deformities of left ventricular-abdominal aortic conduit.

Left ventricular apical-abdominal aortic valved conduit has been a successful procedure for relief of severe left ventricular outlet obstruction. Postoperatively, unusual deformities of the gastric fundus and cardia may be seen on the upper gastrointestinal series and should not be mistaken for pathologic mass lesions. Three deformities are characteristic: anterior impression on the gastric fundus, medial impression on the gastric fundus, and narrowing of the distal esophagus. Occasional gastrointestinal complications including dysphagia and gastric erosion may occur.

Preliminary abdominal films for gastrointestinal examinations: how efficacious?

A prospective study to evaluate the efficacy of preliminary abdominal films for colon and upper gastrointestinal examinations was carried out on 733 patients. The preliminary film was found to be cost effective and useful in predicting adequacy of patient preparation for colon examinations but not for upper gastrointestinal studies. Significant diagnostic information that could potentially contribute to the patient's care was present in 8.4% of the cases. No contraindications to contrast examinations were found.

Gastric polyps in familial polyposis coli.

Familial polyposis coli has been considered a disease in which polyps are confined to the colon and rectum. The authors recently saw 3 cases in which either adenomatous or hyperplastic polyps were also present in the stomach and duodenum. The polyps were detected only by endoscopy or air-contrast radiographic examination. These cases and other recent studies indicate that gastric and duodenal polyps are more common in familial polyposis coli than previously recognized and should be considered an integral part of the syndrome.

Adenocarcinoma of the stomach in association with Menetrier's disease.

Menetrier's disease is an uncommon lesion which may have malignant potential. This report documents a case of gastric malignancy arising in a patient with long-standing Menetrier's disease and emphasizes that close follow-up of patients with this condition is necessary because of the possible development of gastric malignancy.

Preliminary abdominal films in oral cholecystography: are they necessary?

A survey of 45 radiologists with a special interest in gastrointestinal radiology showed that only 8% used a preliminary abdominal film for oral cholecystography. Review of 467 cases of oral cholecystography with preliminary films over a 1 year period disclosed 88 confirmed cases of cholelithiasis. Of these 88 cases, 27% had opaque gallstones visible on the preliminary film. In one-third of the cases, the opaque gallstones had a classic diagnostic appearance, while the remainder required oral cholecystography for confirmation. Opaque calculi were obscured by oral contrast material in 2% of cases. Other significant pathology on the preliminary films was found in 5.5% of the 467 cases.
