The glycoproteins of secretory cells in airway epithelium.

Quantitative histochemical methods demonstrate a variety of glycoproteins within mucus-secreting cells of airway epithelium. A single cell may synthesize one or a combination of four major types--(i) neutral glycoprotein, (ii) and (iii) sialylated either sensitive or resistant to sialidase and (iv) sulphated. In human airway disease, or in experimental response to inhalation of an irritant, there is mucus cell hyperplasia and change in the proportion of cells synthesizing the various types. Experimental studies show how speedily these changes occur. In rats exposed to tobacco smoke changes are found within 20 h of the first exposure. Only in the extrapulmonary epithelium is there discharge of the secretions, with an apparent fall in cell number. Modification of glycoprotein may occur with an unchanged or increased cell number, suggesting that it occurs in existing and newly appearing secretory cells. Modification of the contents of the granule occurs toward the cell apex. Modification of glycoprotein synthesis towards the normal is also the most sensitive and earliest sign of recovery.

Comparative biochemistry of mammalian epididymal plasma.

1. A comparison has been made of the chemical composition of epididymal plasma from the cauda epididymidis of nine mammalian species. 2. Results have shown that epididymal plasma contains low concentrations of inorganic ions and high levels of several unusual organic constituents, among which may be mentioned glycerylphosphorylcholine, hypotaurine, carnitine and several glycosidases and phosphatases. 3. The influence of this milieu on the motility and survival of spermatozoa is discussed.

A study of waste gas scavenging in operating theatres.

Measurements of atmospheric nitrous oxide concentrations were made in eight hospitals, using 30 anaesthetic machines and locations, during 41 anaesthetic administrations or simulations. All areas were air conditioned, two by laminar flow devices. All anaesthetic machines and ventilators were fitted with commercially available gas collector (scavenger) valves. In all areas except one, venturi suction was used to exhaust gases. Pollution levels during endotracheal anaesthesia did not exceed the recommended 30 ppm. except where leaking anaesthetic machines or nitrous oxide supplies were used. In two instances where paediatric anaesthesia was administered through uncuffed endotracheal tubes nitrous oxide levels were also excessive. Of nine anaesthetics administered through face masks, only in one was the ambient nitrous oxide concentration acceptable. During induction of anaesthesia using nitrous oxide, unacceptable peaks of concentration were encountered. In two air conditioned recovery rooms tested, nitrous oxide concentration was acceptable. The collector valves performed their function satisfactorily, but laminar flow air conditioning was insufficient in itself to maintain acceptably low nitrous oxide concentration without the use of scavenging.

Asymptomatic viral hepatitis types A and B in an adolescent population.

Sera from 95 adolescents were examined for markers of hepatitis B virus (HBV) infection and hepatitis A virus (HAV) infection. HBV markers were found in eight adolescents (8%) and evidence of previous HAV infection was found in 18 adolescents (19%); none had a history of clinically recognizable hepatitis. These findings support the growing evidence that HBV and HAV infections are diseases of the pediatric age group, and that testing of HBV vaccines when they become available for patient use will have to include a pediatric population.

Rupture of the plantar fascia in athletes.

Symptoms resembling those of plantar fasciitis were seen in six athletes who were thought to have a partial rupture of the plantar fascia. Treatment, which included the use of crutches, anti-inflammatory agents, strapping of the arch, and ice packs, was successful in all but one patient who had a painful mass in the area of the previous rupture. After surgical excision of the painful mass and release of the fascia, he recovered. Five of the six athletes had been previously treated with repeated local injections of steroid.

The anti-reproductive pharmacology of LH-RH and agonistic analogues.

LH-RH and three particular ("super") analogues were evaluated for agonistic (ovulation-induction and short-term uterotrophic properties) and postcoital contraceptive activity in rodents. Additionally, LH-RH and/or a representative analogue (D-Ala6-des Gly10-Pro9-LH-RH ethylamide) were tested for postcoital contraceptive/vaginal smear/return to fertility effects, precoital contraceptive activity, and effects on puberty in the immature female. All compounds induced ovulation and uterotrophic effects and terminated pregnancy when administered either pre- or post-implantation. LH-RH and the representative analogue, while terminating pregnancy postcoitally, produced an associated break in the characteristic leucocytic vaginal smear of pregnancy to one of cornification by day 12; at this time mating and insemination were reestablished and all rats carried to normal term. Precoitally, LH-RH administered to nembutalized (but not to unblocked) rats produced a 50% reduction in the pregnancy rate and a 38% decrease in the number of viable pups delivered. In immature rats, the representative analogue delayed puberty (i.e. vaginal canalization) and retarded the growth of the ovaries, uteri, and anterior pituitary gland. The collective data strongly support the concept that LH-RH and agonistic derivatives, in spite of their putative pro-fertility classification, are characteristically antifertility by nature. Since the latter effect appears to be the paradoxically dominant one, it is suggested that LH-RH agonism is synonymous with contraception. Furthermore, such peptides may represent a new potential approach to fertility control.

Hb Potomac (101 Glu replaced by Asp): speculations on placental oxygen transport in carriers of high-affinity hemoglobins.

Blood from a woman with unexplained erythrocytosis had increased oxygen affinity, but no abnormality could be detected by electrophoresis or chromatography of her hemolysate. Separation of the tryptic peptides of her beta chains disclosed two half-sized peaks in the regions of beta T-11. The faster of these was abnormal, with the structure beta 101 Glu replaced by Asp. The new hemoglobin was called "Potomac." Three of the proband's four surviving siblings and both of her children were carriers. Differences in the ratio of carrier: normal children born to male of female carriers of 23 other high-affinity hemoglobins were not significant. The high proportion of carriers in this kindred was probably due to chance alone, and not because high maternal oxygen affinity interfered with oxygen transport to fetuses with normal hemoglobin.

Splenectomy for Felty's syndrome. Clinicopathological study of 27 patients.

The major clinical and pathological features and the long-term follow-up of 27 patients with Felty's syndrome who were treated with splenectomy for sever granulocytpenia and for acute, chronic, or recurrent infection were studied. Granulocyte counts rose within days in most patients, although slow responses and transient granulocytopenia did occur; only 12% of the patients had persistent or recurrent granulocytopenia. Infections resolved promptly in 77% of the patients, more slowly in the remainder, and only one patient had new problems of infection after aplenectomy. Splenic enlargement, present in all but one case, was attributable to expansion of the sinusoidal pulp. The most substantial pathological features of immune stimulation included germinal center hyperplasia and prominent clusters of plasma and preplasma cells within sinuses.

Visual evoked response as a function of grating spatial frequency.

Transient visual evoked responses (VER's) to the appearance-disappearance of sinusoidal gratings have been investigated for a range of spatial frequencies. Contrary to the results of previous studies, the results show that the transient VER consists of a relatively simple waveform that is most easily characterized by the initial negative peak (N1) whose latency and amplitude vary with the contrast and spatial frequency of the grating. At spatial frequencies less than 3 cycles/degree (c/d) an additional short latency component appears in the response. This component is maximum at 1 to 2 c/d, saturates at low contrast, and is insensitive to the precise position of the grating on the retina. The results are related to the properties of transient and sustained channels assumed to exist in the human visual system.
