Status of the rat acrosome during sperm-zona pellucida interactions.

Over the past 40 years evidence from many sources has indicated that the mammalian acrosome reaction occurs within or near the cumulus oophorus. Recently, however, workers investigating in vitro fertilization in the mouse have concluded that in this system the acrosome reaction takes place on the surface of the zona pellucida. We have investigated the interaction of rat spermatozoa and the zona pellucida by using the scanning electron microscope (SEM) and two monoclonal antibodies which are directed to antigens of the rat sperm acrosome. When in vitro inseminated eggs from which the cumulus has been removed are viewed with the SEM some sperm heads on the surface of the zona pellucida appear unaltered whereas others appear to be undergoing changes. In vivo, all displayed altered head morphology. Using immunogold labeling we found that two antibodies employed, 2C4 and 5B1, were directed to acrosomal content and vesiculating acrosomal membranes. Immunofluorescence staining of zonae pellucidae in in vitro fertilization studies revealed numerous small positive regions. These were presumably acrosomal content and membranes which had been left on the zona surface by spermatozoa which had been associated with the zona surface. Our results suggest that the rat acrosome interacts with the zona pellucida. During this interaction some acrosomal content and membranes detach from the spermatozoon and remain on the surface of the zona pellucida.

The DNA sequence of the Rhodobacter capsulatus ntrA, ntrB and ntrC gene analogues required for nitrogen fixation.

We have determined the DNA sequence for the genes nifR1, nifR2 and nifR4 in the photosynthetic bacterium Rhodobacter capsulatus. These genes regulate transcription of the nifHDK operon and so limit the expression of nitrogen fixation activity to periods of low environmental concentrations of both oxygen and fixed nitrogen. The sequences of these three genes are similar to components of the ntr regulation system in Escherichia coli and Klebsiella pneumoniae. The two-component regulatory system of ntrB and ntrC in E. coli is represented by nifR2 and nifR1 in R. capsulatus and nifR4 in R. capsulatus is the equivalent of the E. coli ntr-related sigma factor ntrA.

Use of external shock-wave lithotripsy and adjuvant ursodiol for treatment of radiolucent gallstones. A national multicenter study.

A prospective multicenter trial was performed to evaluate the use of external shockwave lithotripsy (ESL) and adjuvant medical therapy for the treatment of gallstones. A Medstone STS lithotripter was used together with ursodiol. Two hundred twenty-three patients were treated under general anesthesia (75%) or with intravenous analgesia (25%). Initial treatments were on an inpatient basis, but as centers gained experience, outpatient treatments became more common. Stone fragmentation and clearance were greatest in patients with solitary gallstones less than 2 cm in diameter. In this group of patients, stone fragmentation occurred in 97% of patients, and the cumulative stone-free rates at three and six months were 54% and 90%, respectively. These results indicate that fragmentation of gallstones can be achieved by a dry shock-wave lithotripter and that stone clearance is induced more rapidly by external shock-wave lithotripsy and adjuvant ursodiol therapy than by ursodiol therapy alone.

Lymphocyte vaccination against experimental autoimmune encephalomyelitis: evaluation of vaccination protocols.

Repeated vaccination with encephalitogenic but not other T cell lines could effect marked resistance to 'active' experimental autoimmune encephalomyelitis (EAE) induced by injection of GP-BP in adjuvant. Partial resistance to active EAE was observed in rats recovered from 'passive' line-mediated EAE and in rats vaccinated with T cells attenuated by irradiation or ganglioside treatment. However, no resistance was observed in animals given low doses of activated encephalitogenic T cells. Treatment with hydrostatic pressure alone was found to be ineffective as a means of attenuation, and vaccination with pressure-treated encephalitogenic T cells actually induced mild signs of EAE. However, vaccination with cells that were first pressure treated and then irradiated prevented both clinical and histologic signs of active EAE. In contrast, protection against passive EAE appeared to be clonotypic. Lymphocyte vaccination induced delayed type hypersensitivity (DTH) reactions against autologous T cells, mostly to shared antigens, demonstrating the immunogenicity of multiple antigens on the vaccinating cells.

Feed intakes and meal patterns of sheep during pregnancy and lactation, and after weaning.

Four cross-bred ewes were kept in individual pens from the last month of pregnancy until the third week of weaning after a 5-week lactation; lambing occurred in late March. The ewes were offered a complete pelleted feed ad libitum, and every few days the feeding behaviour of each was monitored by continuous recording of the weight of the feed container. The weight and timing of each meal was used to calculate mean meal size and length, intermeal interval, hunger and satiety ratios. The increase in daily intake which occurred after parturition was due to increases in meal size, rate of eating and meal frequency. Intake was lower in the period of the day from 0400 hrs to 0800 hrs than during any of the 4-h periods between 0800 hrs and 2400 hrs, with intermediate values at 2400-0400 hrs. The time of day had no effect on meal size or rate of eating. These results are compared with the predictions of a model designed to predict meal occurrences in sheep and the differences used to suggest improvements to the model.

Mechanistic investigation of species-specific thyroid lesions induced by treatment with the histamine H1 antagonist temelastine (SK&F 93944) in rats.

Temelastine (SK&F 93944), an H1 histamine receptor antagonist, induces thyroid histopathological lesions in the rat, indicative of thyroid follicular cell stimulation, at oral doses of 10-33 mg/kg body weight/day. These changes do not occur in the dog or mouse. Endocrinological short-term studies support an increased thyroid follicular activity (increased radioiodide accumulation) with decreased circulating thyroxine (T4) at oral doses at or above 300 mg/kg body weight/day and increased circulating thyroid stimulating hormone (TSH). This is thought to be responsible for the thyroid follicular stimulation following Temelastine treatment. No direct inhibition of thyroid function occurs. Temelastine produces these species-specific changes by enhancing thyroxine clearance from the circulation in the rat, but not in the dog or mouse. In vitro studies with cultured rat hepatocytes suggest that the mechanism behind these changes is a drug-induced increase of hepatocellular T4 binding and uptake which leads to an enhanced metabolic clearance of the hormone.

Distal anastomotic intimal hyperplasia: histopathologic character and biogenesis.

Although thrombogenicity of the prosthetic graft, progression of the atherosclerotic disease and distal anastomotic intimal hyperplasia are known etiologic factors of late graft failure, its occurrence is frequently encountered in the late graft occlusion. Forth-two canine PTFE iliofemoral grafts (all with end-to-side distal anastomosis) were studied. Computer digitization revealed that distal anastomotic intimal hyperplasia occurred exclusively at the heel and the toe of the graft and the floor of the host artery. The distal anastomotic intimal hyperplasia was 80-130 cells thick. Light microscopy and transmission electron microscopy revealed a similar architecture of interlamination of cellular elements and extracellular matrix in the hyperplastic cells. Transmission electron microscopy further defined a gradual cell transformation and orientation from the graft to the lumen. The cells near the graft were characterized by a gradual reduction of rough endoplasmic reticulum with a concomitant acquisition of myofilaments, transforming ovoid mesenchymoid cells to slender myofibroblasts. The orientation of cells in distal anastomotic intimal hyperplasia was embodied by random cell distribution at the periphery to a well-organized interlamination of myofibroblasts and extracellular matrix near the lumen. Distal anastomotic intimal hyperplasia is a biologic entity with active cellular and subcellular events. Its biogenesis appears to be influenced by the hemodynamics of blood flow at the distal anastomosis.

Life events, vulnerability and illness: a selected review.

In this review we examine the links between life stress and illness and the research approaches and methodologies which have been used to study them. Evidence is presented of causal relationships between adverse life events and illness onset and we discuss ways in which these events may be detected and measured. The notion that 'moderating variables' may affect both an individual's vulnerability to the effects of adversity and the coping strategies adopted to deal with life stress is emphasized. Typically, research has focused on the effect of a single psychosocial variable on the correlation between life events and illness. We conclude that research aimed at eliciting factors associated with successful coping is of more importance if primary care physicians are to identify vulnerable patients and make therapeutic interventions to minimize the damaging effect of stress and adversity.

Identification of carbohydrate-binding proteins in mammalian spermatozoa (human, bull, boar, ram, stallion and hamster) using [125I]fucoidin and [125I]neoglycoprotein probes.

A protein-carbohydrate recognition system is thought to be involved in the early stages of fertilization in mammals. In this investigation carbohydrate-binding proteins have been identified in extracts of human, bull, boar, ram, stallion and hamster spermatozoa using [125I]fucoidin and [125I]neoglycoproteins (BSA-fucose and BSA-mannose) to probe Western blots. Results show that proacrosin is the major protein species recognized in extracts of human, bull, boar and ram spermatozoa. In hamster and stallion spermatozoa, carbohydrate-binding activity was associated with several low molecular weight proteins that may or may not be related to active forms of acrosin. These results support the hypothesis that proacrosin is involved in secondary or consolidated binding of mammalian spermatozoa to the zona pellucida by virtue of its carbohydrate-binding capacity.

Factors associated with severity of symptoms in patients with chronic unexplained muscular aching.

Subjects with chronic, diffuse, unexplained muscular aching were recruited--21 from a primary care practice, nine from a rheumatology practice, and two from a pain clinic. No additional criteria were used to select subjects. Subjects with mild or moderate symptoms differed from those with severe symptoms with respect to the following characteristics: the presence of fatigue on awakening, the number of tender points, difficulty in sleeping, and the degree of tenderness in typical fibromyalgia areas as measured by a dolorimeter. These findings suggest that muscular aching is likely to be of greater severity if other symptoms or signs of fibromyalgia are also present.

Sympathetic vasoconstriction during exercise in ambulatory patients with left ventricular failure.

In patients with heart failure, exercise is thought to increase sympathetic vasoconstrictor tone. To investigate the extent of this sympathetic activation, we studied the effect of maximal exercise on nonexercising vascular beds in 35 patients with left ventricular failure (ejection fraction, 21 +/- 8%; peak exercise oxygen uptake (VO2), 12.3 +/- 3.5 ml/min/kg). In 28 patients, cardiac output and leg blood flow were measured during maximal upright bicycle exercise. Total flow to nonexercising tissue was then calculated as cardiac output--(2 x leg flow). In seven patients and six normal subjects, forearm blood flow was measured during supine bicycle exercise before and after alpha-adrenergic blockade with intravenous phentolamine. Maximal upright exercise increased the vascular resistance of nonexercising tissue from 34 +/- 16 units at upright rest to 45 +/- 25 units (p less than 0.02) but did not affect total flow to nonexercising tissue (rest, 2.9 +/- 1.0; maximal exercise, 2.8 +/- 1.4 l/min; p = NS). Supine exercise had no significant effect on forearm blood flow or vascular resistance in the normal subjects. In the patients with heart failure, supine exercise increased forearm vascular resistance from 45 +/- 17 to 58 +/- 25 mm Hg/ml/min/100 ml (p less than 0.02), again with no change in tissue flow (rest, 2.4 +/- 0.1; maximal exercise, 2.4 +/- 0.9 ml/min/100 ml; p = NS).(ABSTRACT TRUNCATED AT 250 WORDS)

Lung vascular cell proliferation in hyperoxic pulmonary hypertension and on return to air: [3H]thymidine pulse-labeling of intimal, medial, and adventitial cells in microvessels and at the hilum.

The labeling index (LI) of lung vascular intimal (endothelial and precursor smooth muscle, medial (smooth muscle), and adventitial cells (fibroblasts) was in the normal rat and in the rat with pulmonary hypertension caused by breathing high oxygen (87% O2 at normobaric pressure). Cell labeling was assessed during hyperoxia (Days 1, 4, 7, 10, and 28), at the start and end of weaning hyperoxia-adapted rats to air (Days 29 and 35), and 1, 2, and 4 wk after return to breathing air after hyperoxia and weaning (Days 42, 49, and 63). Bursts of intimal, medial, and adventitial cell proliferation different in their timing and extent contribute to lung vascular wall remodelling in these injuries. The Ll of the microvascular fibroblast increased most on Day 4 of hyperoxia (20-fold); it persisted above the normal rate throughout hyperoxia and was high at the start of weaning (greater than 1- less than 2-fold). Two weeks after return to breathing air, it again increased (less than 1-fold). The Ll of the microvascular endothelial cell increased most on Day 7 of hyperoxia (10-fold); it also persisted above the normal rate throughout hyperoxia and at the start of weaning (greater than 2- less than 3-fold) and increased again 2 wk after return to breathing air (6-fold). Labeled precursor cells were not present in the normal lung: they were present on Days 4 and 7 of hyperoxia but not at any other time, including weaning and return to breathing air.(ABSTRACT TRUNCATED AT 250 WORDS)

Natural history vs. surgery for Menière's disease.

Menière's disease has an episodic course, and certain patients undergo spontaneous remission of their vertigo. A retrospective study of patients treated from 1974 to 1983 was undertaken to evaluate the long-term outcome of patients with Menière's disease for whom surgery was recommended, comparing those who had a surgical procedure with a similar group of patients who declined surgery. The surgical procedures performed were endolymphatic subarachnoid shunt (ELS), retrolabyrinthine vestibular neurectomy (RVN), middle fossa vestibular neurectomy (MFVN), and transmeatal cochleo-vestibular neurectomy (CVN). We used a questionnaire, made up according to the 1985 American Academy of Otolaryngology (AAO) criteria, for reporting results for Menière's disease treatment, and compared patients who were offered surgery but declined (N = 50) with those who underwent surgery (N = 83). The data were analyzed statistically. Initial evaluation, which included air and bone conduction audiometry, speech discrimination, electronystagmography (ENG), frequency of vertigo attacks per month, and disability, showed both groups to be comparable at the outset. Of the non-operated group 57% had complete control of vertigo at 2 years; 71% had complete control after an average of 8.3 years. After an ELS, 40% of patients had complete control of vertigo after 2 years; 70% had complete control after an average of 8.7 years. After a neurectomy (RVN or CVN), 93% had complete control of vertigo (average followup, 4.4 years). These results indicate statistically that the ELS procedure does not alter the long-term natural course of vertigo control in Menière's disease, whereas both the RVN and CVN significantly improve the patient's chance of being permanently free of vertigo attacks.

Long-term results of transmeatal cochleovestibular neurectomy: an analysis of 100 cases.

One hundred patients have been treated over a 13-year period with a transmeatal approach to the internal auditory canal for cochleovestibular neurectomy. Ninety-one of these patients have followup of more than 3 months (average, 4.67 years). The most common indication for the procedure was Meniere's disease (71%). Chronic labyrinthitis, usually following stapes, middle ear, or mastoid surgery, was the next most common indication. Patients with the preoperative diagnosis of Meniere's disease had better results in the curing of vertigo (89%) than those having chronic labyrinthitis (68%). Overall, vertigo was cured in 84% of patients and markedly improved in another 15.1%. Tinnitus was relieved or improved in 65% of all patients and in 67% of patients with Meniere's disease. Mild unsteadiness was commonly noticed postoperatively, but only 11% described this as severe. Eighty percent of these latter patients reported unsteadiness preoperatively. Complications were uncommon and temporary: one case of delayed facial paresis that recovered completely, one CSF leak, and one wound infection. There were no cases of permanent facial paralysis or meningitis. The advantages of the transmeatal approach to the IAC for CVN over labyrinthectomy without CVN are assurance of complete labyrinthine denervation, increased likelihood of improved tinnitus, practice at sectioning the posterior ampullary nerve (PAN), and the ability to inspect the internal auditory canal for a small tumor or other pathology. We recommend this procedure for treatment of unilateral vestibular dysfunction in patients with no serviceable hearing.

Small fenestra stapedotomies with and without KTP laser: a comparison.

The results of 33 small fenestra stapedotomies performed using conventional techniques were compared with the results of 33 stapedotomies performed using the argon or KTP laser. The ossicular chain was reconstructed using a Teflon wire piston of 0.6 mm diameter, and follow-up was at least 1 year. Over-closure of the air-bone gap or closure to within 10 dB was accomplished in 91% of the laser-treated group versus 72% of the conventionally treated group (p less than 0.10). The hearing results were statistically better in the laser group (p less than 0.05). Transient delayed vestibular symptoms, lasting from 1 to 3 weeks, were present in 39% of the laser-treated group and in 12% of the patients treated by conventional techniques (p less than 0.05). The KTP laser stapedotomy, using a micromanipulator mounted on the microscope, is a safe, efficient technique that reduces some of the technical difficulties associated with conventional stapes surgery. The main advantage of the laser is that it enables the surgeon to make an atraumatic, bloodless opening in a fixed or mobile stapes footplate without mechanical manipulation of the stapes. Using a lower wattage to vaporize the footplate and waiting several seconds between laser bursts may decrease the incidence of postoperative vestibular symptoms. The use of the KTP laser in stapes surgery represents a major advance in surgery for otosclerosis.

Progesterone concentrations in samples of saliva from adolescent girls living in Britain and Thailand, two countries where women are at widely differing risk of breast cancer.

Menstrual-cycle patterns of salivary progesterone concentration were obtained for 131 and 245 adolescent girls up to 4 years postmenarche living in Britain and Thailand respectively. These patterns were graded on a scale of 1 (little or no activity) to 5 (activity similar to that exhibited by the mature premenopausal woman) and the frequency of these grades within groups of girls from each centre was analysed. The major finding was that British girls exhibited a predominance of higher grades of progesterone activity when compared with their Thai counterparts (n = 2 x 58) when matched for chronological and gynaecological ages (P approximately 0.002). This was particularly so for the girls from these two matched groups in the gynaecological age range 2-4 years (P approximately 0.03). The major contribution to this significant difference between the two groups is attributed to the greater effect of chronological age on progesterone secretion in the British girls (P approximately 0.03) compared with the Thai girls (P approximately 0.29). These findings may have implications for facilitating our understanding of the reason for the differing risk of breast cancer in women in both countries.
