[Ocular localization appearing in a child with acute lymphoblastosis after hematological remission for 18 months].

Case report of a 4-year-old girl in whom a unilateral ocular localization appeared after 1 1/2 year of maintained remission of acute lymphoblastic leukaemia. The evolution of this localization can be divided in three phases: 1. During the first 17 months, an apparent simple relapsing conjunctivitis of the left eye. 2. During the following 5 months, repeated episodes of the left iritis with mydriasis and discolouration of the iris. 3. Finally, left uveitis involving the anterior chamber, with hypopion, hyphaema and glaucoma; the clinical presentation at this stage was suggestive of ocular lymphoblastosis. Radiotherapy to the lesion (total dose, 1 200 rads) was quickly followed by resolution without sequelae. No relapse was observed during the following 30 months, till death. The ocular disease remained the only manifestation of leukaemia during its evolution. A haematological relapse occurred 11 months after cure of the iritis, followed 1 year later by a second, treatment-sensitive relapse, and finally by a third, fatal relapse. The patient survived 5 years 9 months.

[Medical applications of cyclotrons (author's transl)].

Isochronous cyclotrons used to accelerate different charge particles (protons, deuterons, alphas...) at variable energies, have important medical applications, for neutron teletherapy, in vivo or in vitro activation analysis or production of short-lived radioisotopes for nuclear medicine. The characteristics of the cyclotron presently available are described for these three applications (low energy "compact" cyclotrons, cyclotrons of intermediate and high energies), and their advantages are discussed from the points of view of the medical requirements, the financial investments and the results obtained.

[Plasma prolactin levels in normal children from birth to adolescence].

Plasma prolactin levels were determined by an homologous radio-immuno assay in normal children: in cord blood, at the first day of life, during childhood and along puberty. 1. In both sexes, there is a very important secretion of prolactin during the neonatal period. 2. Longitudinal studies make obvious a different pattern of plasma prolactin in boys and in girls at puberty.

[Biological and hormonal balance during oral nutrition, exclusive perenteral and mixed nutrition in children].

Exclusive parenteral nutrition and mixed feeding provided by the infusion of a solution of amino-acids, glucose and trace elements by a drip method, result in changes in biological and hormonal balance which have been compared with balance in the child on normal oral feeding. The 24-hour cortisol cycle tends to be attenuated. Blood glucose and, above all, blood insulin levels show major variations. Plasma free fatty acid levels, stable in exclusive parenteral nutrition, in general vary with mixed nutrition. On the basis of these findings, a constant flow should be ensured by use of a pump during exclusive parenteral nutrition. In mixed feeding, meals should be frequent and energy supplies given in the form of slowly absorbed carbohydrates.

[Fatal emboligenic ulcerative endomyocardial fibrosis in filariasis].

A man of 54 had been treated over a 15 month period for pulmonary tuberculosis when aged 41, and had lived on the Ivory Coast and then in the Cameroons for 20 years. Eye signs were discovered in January 1971, and these, together with an eosinophilia, pointed to a possible filariasis which was later confirmed by immunofluorescence and the fixation of complement. Three months later, the patient developed congestive cardiac failure. On the 4th day of treatment with Notizine, multiple systemic emboli occurred, leading to death. Autopsy showed endomyocardial fibrosis with multiple ulceration of the left ventricle, the site of the emboli. The aetiology from filariasis and the mechanism of ulceration of the parietal endocarditis are discussed.

[Diffuse hamartomatous tumor of the cerebellum and brain stem in children].

A child presented in the first year of life with a slowly evolving cerebellar syndrome which resulted in death at the age of 4 1/2 years. A peculiar tumour involving the cerebellum and brain stem and destroying the cerebellar anatomy is described. This was considered to be a form of gangliomatous hamartoma.

[Localized hemangioma of the bronchial trunk of the left lung in a 2-month-old infant].

A 2 month-old infant was admitted because of respiratory distress and thoracic assymetry. X-ray films revealed a distension of the left lung. A pneumo-angiography showed a cardiac silhouette displaced to the right and a normal vascular tree of the left lung. The rapid impairement of the clinical picture led to an exploratory thoracotomy. A tumor at the level of the left hyparterial bronchus was found. The histologic characteristics of the tumor were those of a hemangioma. Within the spectrum of broncheal tumors, hemangioma belong to benign mesenchymatous tumors and have a good prognosis providing that their removal is complete.
