Heterozygous mutations at the mut locus in fibroblasts with mut0 methylmalonic acidemia identified by polymerase-chain-reaction cDNA cloning.

Genetic defects in the enzyme methylmalonyl CoA mutase cause a disorder of organic acid metabolism termed "mut methylmalonic acidemia." Various phenotypes of mut methylmalonic acidemia are distinguished by the presence (mut-) or absence (mut0) of residual enzyme activity. The recent cloning and sequencing of a cDNA for human methylmalonyl CoA mutase enables molecular characterization of mutations underlying mut phenotypes. We identified compound heterozygous mutations in a mut0 fibroblast cell (MAS) line by cloning the methylmalonyl CoA mutase cDNA by using the polymerase chain reaction (PCR), sequencing with internal primers, and confirming the pathogenicity of observed mutations by DNA-mediated gene transfer. Both mutations alter amino acids common to the normal human, mouse, and Propionibacterium shermanii enzymes. This analysis points to evolutionarily preserved determinants critical for enzyme structure or function. The application and limitation of cDNA cloning by PCR for the identification of mutations are discussed.

Diabetes expert systems: planning for long term use.

Expert systems may play an important role in the future in assisting diabetics to control their disease. However the data available suggest that expert systems are difficult to build and more difficult to maintain over a long period. An examination of the problems in maintaining one expert system, GARVAN-ES1, suggests that the problems arise because experts never report on how they reach a decision, rather they justify why the decision is correct. These justifications vary markedly with the context in which they are required, but in context they are accurate and adequate; the difficulties arise in taking them out of context. It is suggested that expert system building techniques must be able to capture knowledge in context and tools must be available to flexibly change the context in which an expert system knowledge base is examined. Two implementations of such strategies, "ripple down rules" and a "knowledge dictionary", are outlined.

In vivo effects of combination treatment with recombinant interferon-gamma and -alpha in metastatic melanoma.

The toxicity and therapeutic efficacy of the combination of recombinant interferon gamma (rIFN-gamma) and alpha (rIFN-alpha) was investigated in 15 patients with metastatic melanoma. Patients were treated with an escalating dose of rIFN-gamma and a fixed dose of rIFN-alpha administered s.c. 3 times a week. The maximum dose was well tolerated. The median survival time of the patients was 7 months; no clinical remissions were observed. In the majority of cases, expression of HLA class-I and -II antigens on the patients' peripheral blood lymphocytes and monocytes increased markedly during treatment. An increase in HLA-DR expression of peripheral blood T lymphocytes was correlated with a longer survival time. This suggests that activation of T lymphocytes may have a favourable influence on the course of metastatic disease. The in vitro anti-proliferative activity of IFNs on melanoma cell lines isolated from melanoma metastases during treatment of 3 patients was determined. In contrast to the lack of in vivo anti-tumour effect in patients, both rIFN-gamma and rIFN-alpha inhibited DNA synthesis of these melanoma cell lines in vitro, combined IFNs acting synergistically. Anti-proliferative activity observed in vitro occurred at IFN concentrations below the peak serum levels achieved in vivo.

Differential effects of follicle-stimulating hormone, insulin, and insulin-like growth factor I on hexose uptake and lactate production by rat Sertoli cells.

The stimulatory effects of follicle-stimulating hormone (FSH), insulin, and insulin-like growth factor I (IGF-I) on lactate production and hexose uptake by Sertoli cells from immature rats were studied. The time-courses and the maximal stimulatory effects of FSH, insulin, and IGF-I on lactate production were virtually identical. When Sertoli cells were incubated in the presence of FSH in combination with insulin or IGF-I (submaximal doses), additive but no pronounced synergistic effects were observed. The stimulatory effects of FSH and insulin were not dependent on the presence of extracellular calcium. 2-Deoxy-D-glucose (2-DOG), an analogue of D-glucose, was used to investigate the hexose transport system of Sertoli cells. Uptake of 2-DOG was linear in time and virtually all of the intracellular 2-DOG was phosphorylated up to 30 min of incubation; 2-DOG uptake was inhibited by cytochalasin B, but not by cytochalasin E. D-glucose, but not D-galactose, appeared to be an effective competitor of 2-DOG uptake. The Km of 2-DOG uptake was not influenced by FSH, insulin, and IGF-I. FSH had no effect on the Vmax of 2-DOG uptake, whereas insulin and IGF-I caused a 30% stimulation of the Vmax. It is concluded that FSH, insulin, and IGF-I stimulate lactate production by cultured Sertoli cells, but that only insulin and IGF-I stimulate hexose transport. The insulin-like effect of FSH on Sertoli cells may principally involve stimulation of glycolytic enzyme activities.

Cloning of full-length methylmalonyl-CoA mutase from a cDNA library using the polymerase chain reaction.

The polymerase chain reaction was used to clone a full-length human methylmalonyl-CoA mutase cDNA from a human liver library by priming with sequences from the 5' end of a partial cDNA and sequences in the phage vector. The amino acid sequence predicted from the cDNA corresponds to the authentic amino acid sequences of peptide fragment from purified methylmalonyl-CoA mutase. The open reading frame of the cDNA encodes 742 amino acids (82,283 Da) comprising a 32 amino acid mitochondrial leader sequence and a mature protein of 710 amino acids (78,489 Da). The use of the polymerase chain reaction to "screen" the cDNA library represents a novel application of this technique. The full length will enable analysis of mutations underlying inherited methylmalonic acidemias caused by deficiency of the methylmalonyl-CoA mutase apoenzyme.

A comparison of patients and patient complaints at six chiropractic college teaching clinics.

A cooperative study was undertaken by six chiropractic colleges for the purpose of studying similarities and/or differences among the patients and patient complaints at the college outpatient (teaching) clinics. There were some notable differences among the clinics with respect to the standard demographic variables of age, education, employment and income. The sociodemographic characteristics of patients appeared to be different to the extent that the characteristics of the neighborhoods in which the clinics were located were different. Patients referred to the clinics by chiropractic student/interns were more likely to attend for routine physical exam than patients referred by other sources. Although marked differences were observed in patient attendance for routine physical examination, the health problems for which patients sought treatment were very similar among all the clinics. Low back complaints were the most frequently reported health complaint. The characteristics of the low back complaints were very similar at all six sites.

Endemic Acinetobacter anitratus in a surgical intensive care unit: mechanical ventilators as reservoir.

Colonization and infection with an endemic multiresistant strain of Acinetobacter calcoaceticus variety anitratus had been observed in the surgical intensive care unit of a university hospital since 1982. An outbreak of infection with this endemic, multiresistant Acinetobacter anitratus strain occurred between January and September, 1984. After initial attempts at identification of environmental reservoirs had been unsuccessful, risk factors for the acquisition of Acinetobacter anitratus were investigated by comparing the epidemiological characteristics of patients who became colonized or infected with those of control patients without colonization. The results of this case-control study and of the ensuing specific cultures indicated that ventilators in use in the unit were the reservoirs of Acinetobacter anitratus, resulting in frequent nosocomial respiratory tract infections. After modification of the mechanical ventilators, colonization and infection rates with Acinetobacter anitratus decreased. Since January 1985, no new cases of colonization or infection with this endemic strain of Acinetobacter anitratus have been recorded.

The clinical impact of in-vitro fertilization. Part 1. Results and limitations of conventional reproductive medicine.

Conception is a matter of chance. A couple's monthly probability of conceiving naturally is known as their fecundability. The average "normal" fecundability (as a proportion) is approximately 0.2. This means that most young couples can expect to have children and will conceive within 12 months of attempting pregnancy. This article examines the causes of infertility and the results that can be achieved by conventional treatment among couples with a single cause for their infertility. The difficulties that are faced when conventional infertility treatment is applied to infertility with multiple causes or with no apparent cause are explained.

The clinical impact of in-vitro fertilization. Part 2. Regulation, money and research.

Assisted conception with in-vitro fertilization and related reproductive interventions is the only practical way of treating many infertile couples. This article examines the cost-effectiveness of in-vitro fertilization procedures, the importance of not isolating these procedures from the general management of infertility, and the urgent need to clarify the ethical justification for research on the early human embryo.

The sorangicins, novel and powerful inhibitors of eubacterial RNA polymerase isolated from myxobacteria.

A new antibiotic, sorangicin, was isolated from the culture supernatant of the myxobacterium, Sorangium (Polyangium) cellulosum strain So cel2. It is a macrocyclic lactone carbonic acid and is produced in two structural variants, sorangicins A and B. In addition small quantities of the respective glycosides, sorangiosids A and B, may be found. The antibiotic acts mainly against Gram-positive bacteria, including myocobacteria, with MIC values between 0.01 and 0.1 microgram/ml, but at higher concentrations (MIC 3 approximately 30 micrograms/ml) Gram-negatives are also inhibited. Yeasts and molds are completely resistant. The new antibiotic is a specific inhibitor of eubacterial RNA polymerase which it blocks, however, only if added before RNA polymerization has started.

The characteristics, perceptions and personalities of women seeking a reversal of their tubal sterilization.

The characteristics, perceptions and personalities of women who had sought a reversal of a tubal sterilization procedure and those who had not were studied. Women were more likely to request reversal of a tubal sterilization if they were less than 30 years old at the time of the operation or if they had changed their marital status since sterilization. Women who had undergone a tubal sterilization and later sought a reversal stated that at the time of the procedure they did not believe that their family was complete; they had not had sufficient time to consider the implications of undergoing a tubal sterilization; they felt that they had been "pressured" by their partner to be sterilized; and they perceived the potential to have a child as important, even if they did not wish for further children. Women who had had a tubal sterilization performed within a few days of childbirth for "health or medical reasons" and sought a reversal no longer believed that the reasons were valid. In spite of the inclusion of women who had changed their minds about tubal sterilization, no differences were detected in psychometric measures of personality and mental health between our groups. The findings of the survey confirm that a woman should make up her own mind whether or not to undergo a tubal sterilization, and the operation should not be available "on demand". Ways of overcoming the problem are suggested.

Effect of glucose on ATP dephosphorylation in rat spermatids.

Round spermatids were isolated from rat testes and the effects of different energy-yielding substrates on the cellular ATP content were estimated. The ATP content was constant and high (6-8 nmol/10(6) cells) during metabolism of exogenous lactate. During incubation for 30 min in the absence of exogenous lactate, there was a remarkably slow decline of the ATP content, indicating ATP production from other substrates. It was shown that this could reflect beta-oxidation of fatty acids, but not the mobilization of an endogenous pool of acetylcarnitine. Glucose metabolism in the absence of exogenous lactate resulted in a rapid decline of the ATP content. This effect of glucose was correlated with a high fructose 1,6-biphosphate content (6-7 nmol/10(6) cells) and could be prevented by the addition of lactate. It is suggested that metabolism of glucose (and also mannose and fructose, but not galactose) in the absence of exogenous lactate can result in ATP dephosphorylation.

Metabolism of radiolabelled energy-yielding substrates by rat Sertoli cells.

The rates of metabolism in vitro of 3H- or 14C-labelled glucose, pyruvate, glutamine and leucine by Sertoli cells from immature rats were estimated. The overall rate of glucose utilization exceeded by far the rates of oxidation of pyruvate (derived from glucose) via the citric acid cycle and glucose metabolism via the oxidative branch of the pentose phosphate pathway. This pattern of glucose metabolism was not markedly altered after stimulation of glucose metabolism by FSH. The rate of oxidation of exogenous pyruvate indicated that the energy yield from glucose metabolism by Sertoli cells could be dependent on the extracellular concentrations of pyruvate and lactate. There is no evidence that a high rate of aerobic glycolysis is of vital importance for Sertoli cells. In medium containing glucose and all amino acids, 14C-labelled glutamine and leucine were converted to 14CO2 at considerable rates. It was calculated that the oxidation of glutamine and leucine in addition to glucose and fatty acids can yield much of the required energy of Sertoli cells.

Intercellular pathway of leucine catabolism in rat spermatogenic epithelium.

A unique intercellular pathway of leucine catabolism was observed in vitro in rat spermatogenic epithelium. Sertoli cells convert leucine via transmination into 4-methyl-2-oxovalerate, and spermatocytes and spermatids reduce exogenous 4-methyl-2-oxovalerate to 2-hydroxy-4-methylvalerate, which is then released by the spermatogenic cells. The NADH-dependent reduction of 4-methyl-2-oxovalerate could be catalysed by the male-germ-cell-specific lactate dehydrogenase isoenzyme LDH-C4 in the cytosol of the spermatogenic cells, concomitant with the NAD+-dependent conversion of exogenous lactate into pyruvate.

Protein synthesis by isolated pachytene spermatocytes in the absence of Sertoli cells.

Isolated rat pachytene spermatocytes were incubated in chemically defined medium supplemented with pyruvate and lactate, which are known to be essential energy substrates for these germ cells. Protein synthesis by the isolated cells was investigated by means of two-dimensional polyacrylamide gel electrophoresis. The electrophoretic patterns of (35S)-labeled proteins, synthesized by the pachytene spermatocytes during incubation in the presence of (35S)methionine either from 0-2 h or from 24-26 h after isolation, were almost completely identical. The patterns of newly synthesized proteins of freshly isolated spermatocytes and spermatids, however, showed several stage-specific proteins in addition to many proteins common to both spermatogenic cell types. Hence, it was concluded that a stage-specific pattern of protein synthesis can be maintained by pachytene spermatocytes during incubation for a period of 24 h in the absence of Sertoli cells but in the presence of a proper energy source.

Neutralizing antibody to hepatitis A virus in immune serum globulin and in the sera of human recipients of immune serum globulin.

We determined the titer of neutralizing antibody to hepatitis A virus in five lots of immune serum globulin and in the sera of human recipients of immune globulin using a new and sensitive procedure, the radioimmunofocus inhibition assay. The neutralizing antibody titer of four immune globulin lots ranged from 1:170,000 to 1:406,000, and was approximately 100-fold the antibody titer determined by radioimmunoassay. The neutralizing antibody titer of a Bureau of Biologics reference immunoglobulin was 1:794,000. Sera collected from 18 healthy men who had undergone prophylaxis with immune globulin (0.02 ml/kg body wt) were negative when tested by radioimmunoassay. However, 2 of 18 sera collected before administration of immune globulin and sera from all 18 men collected 3 and 55 days later contained detectable neutralizing antibody. The measurement of neutralizing antibody should prove useful for standardizing passive immunoprophylaxis against hepatitis A as well as for evaluating the potential efficacy of new hepatitis A virus vaccines.
