Serum pseudocholinesterase: high density lipoprotein cholesterol ratio as an index of risk for cardiovascular disease.

A significant increase in the ratio of serum pseudocholinesterase/high density lipoprotein cholesterol (the Complementary Risk Factor ratio) was found in individuals classified as high risk for cardiovascular disease on the basis of the ratio of total cholesterol/high density lipoprotein cholesterol (the Established Risk Factor ratio) compared to the individuals with average and low risks. This Complementary Risk Factor ratio also showed good correlation with serum low density lipoprotein, triglycerides and the Established Risk Factor ratio. These results indicate that serum cholinesterase has a parallel relationship with low density lipoproteins and a reciprocal relationship with high density lipoproteins. We propose that the Complementary Risk Factor ratio may be an additional marker in predicting the risks for cardiovascular disease.

Tissue chromium exchange in the rat.

This study of chromium (Cr) exchange in the rat combined measurements of 51Cr distribution and tissue Cr content to permit an assessment of tissue Cr exchange under control conditions and during varied Cr intakes. Liver Cr was found to be 50 to 100% exchangeable whereas pancreas Cr was only 34% exchangeable. In kidney, the specific activity of 51Cr exceeded that of serum by more than 100%, indicating a complex type of exchange involving both a rapidly exchanging Cr pool and an "inner" Cr pool with "sink-like" characteristics. Chromium deprivation of moderate degree reduced serum Cr and tissue exchangeable Cr pools but did not change total tissue Cr, 51Cr distribution, or glucose tolerance. Chromium supplementation and Cr overload increased serum Cr and total, exchangeable, and nonexchangeable tissue Cr pools but did not alter 51Cr distribution. These results indicate that the merging of tracer techniques with tissue Cr analyses yields a more complete view of Cr exchange than 51Cr distribution alone. The data support the hypothesis that specific transport characteristics exist in tissues that may regulate the biological role of Cr.

Disseminated intravascular coagulation in pregnancy. The role of heparin therapy.

Treatment of the underlying cause and supportive care constitute the basic principles of management of disseminated intravascular coagulation. The role of anticoagulation with heparin is controversial in the absence of any controlled studies. This case report describes two patients with acute obstetric disseminated intravascular coagulation in whom the use of heparin resulted in marked clinical improvement. Treatment with heparin may be of help in situations where there is a delay in elimination of the underlying cause.

Radiochromium distribution in hypophysectomized and adrenalectomized rats.

The effects of hypophyseal and adrenal ablation (HYPOX, ADX) on trivalent radiochromium (51Cr) distribution and of GH and T4 replacement in HYPOX rats were determined. Hypophysectomy increased body retention of 51Cr by 20--35%, and hormonal replacement restored body retention to normal. Serum 51Cr was increased 300--800%, resulting in depressed tissue to serum 51Cr ratios. GH replacement partially restored the 51Cr distribution abnormalities in HYPOX rats to normal. The combination of T4 with GH enhanced the restoration of normal 51Cr distribution, and tissue to serum 51Cr ratios returned to baseline levels. ADX had no effect on body retention of 51Cr but had a small effect on increases in serum and tissue 51Cr levels. As a result, tissue to serum 51Cr ratios did not change. These data indicate that in the HYPOX state there is a marked disturbance in Cr metabolism which is due to deficiencies of both GH and T4. Whether or not adrenal hormones are envolved in 51Cr distribution will require further study.

Iatrogenic pancreatitis. A fatal complication in the induction therapy for acute lymphocytic leukemia.

A patient with acute lymphocytic leukemia developed acute pancreatitis during induction therapy with corticosteroids and asparaginase. Postmortem examination showed changes consistent with hemorrhagic pancreatitis and fat necrosis over the pancreas and omentum. Corticosteroids were thought to be responsible for pancreatitis in this patient since "hemorrhagic pancreatitis" has not been previously described with asparaginase. We recommend careful follow-up of serum and urinary amylase values in patients receiving induction therapy with these agents.

Growth and cytopathic effect of rubella virus in primary and progeny guinea pig kidney cells.

A culture system useful for the demonstration of cytopathology of rubella virus in guinea pig kidney cells is described. Not only were cytopathic rubella infections dependent on cell type (primary lung cells were not responsive), but infection early in primary passage was also necessary. Secondary-passage kidney cells were unresponsive. The cytopathic phenomenon did not appear to depend on any special medium or on inhibition of the normal interferon response.

Acute renal failure in the neonate.

With advances in diagnostic technology, the diagnosis of renal failure from prerenal and renal causes is being made more frequently. With better clinical understanding of etiologic causes, early prevention and diagnostic and therapeutic measures can be taken. Proper and controlled use of fluids, calories, and drugs is essential. Because of its relative ease, peritoneal dialysis is usually preferred over hemodialysis when renal failure cannot be handled more conservatively. Although the incidence of renal failure has apparently increased with the advent of neonatal intensive care, survival rates are increasing and the potential for prevention is better.

Sonographic appearances and percutaneous management of primary tuberculous liver abscess.

Primary tuberculous liver abscesses are rare. We report on 3 patients who presented with a nonresolving abscess in the liver. Clinical presentation and sonographic findings in each case were nonspecific. A diagnosis of tuberculosis was established with microbiologic examination of pus in 2 cases and examination of an excised abscess wall in 1 case. Needle aspiration (1 patient) and short-term (72 hours) catheter drainage (1 patient) were unsuccessful, and surgical excision was required in these patients. In the third patient, continuous catheter drainage over 18 days resulted in cure, indicating that long-term catheter drainage with antituberculous chemotherapy may be a viable alternative to surgery in the management of primary tuberculous liver abscess.

Hepatic granulomas due to visceral larva migrans in adults: appearance on US and MRI.

Visceral larva migrans is a syndrome characteristically involving children with a history of pica, and usually presents with fever, abdominal pain, tender hepatomegaly, and hypereosinophilia. Hepatic granulomas of visceral larva migrans are rare in adults. We describe three adult patients with hepatic lesions which on histopathology demonstrated characteristic granulomas of visceral larva migrans. All patients had abdominal sonograms and two had additional MR scans of the liver. Both ultrasound and magnetic resonance imaging demonstrated characteristic appearances which have not been described previously (viz., ill-defined central necrotic areas surrounded by concentric thick walls and perifocal edema in the liver parenchyma).

Co-administration of melatonin reverses the tin-protoporphyrin (SnPP) induced decline of cytochrome P450 content in vivo in rats.

Melatonin (N-acetyl-5 methoxytryptamine) is a low molecular weight antioxidant and is an endogeneous defense system against the deleterious actions of the extremely reactive hydroxyl radical. Among the enzymes that participate in the antioxidant functions is cytochrome P-450, a stalwart of the detoxification system in the body. Our results revealed that tin-protoporphyrin administration brought about a marked decline in cytochrome P-450 levels. This decline was, however, reversed by the coadministration of the antioxidant, melatonin. Thus, the enhanced antioxidant status in melatonin-treated rats may act as a protective mediator of various pharmacological functions altered during tin-protoporphyrin (an antihyperbilirubemenic agent) administration to Wistar rats.

CT portography by direct intrasplenic contrast injection: a new technique.

BACKGROUND: The evaluation of percutaneous contrast injection into splenic parenchyma as an alternative technique for computed tomographic (CT) portography in the preoperative assessment of primary hepatobiliary tumors. METHODS: Thirty-two patients underwent a nonenhanced CT scan of the liver, after which a 19-gauge, 10-cm-long needle was introduced into the splenic parenchyma under CT guidance. One hundred forty milliliters of contrast medium (200 mgI/mL; 28 g/I) were injected through this needle: first, a 20-mL bolus (in 5 s) and then 2 mL/s for 60 s. At the end of the bolus injection (5 s), 8-mm-thick contiguous axial scans of the liver were obtained. RESULTS: The success rate of the procedure was 93.7% (30/32; two technical failures). The average time required for the entire study was 13 min and 50 s (range = 7 min 53 s to 25 min 17 s). Hepatic parenchymal enhancement was good in 24/30 (80%), moderate in 3/30 (10%), and unsatisfactory in caudal sections of the liver in 3/30 (10%). Artifactual perfusion defects were seen in 4/30 (13%) due to inadvertant injection of small quantities of air. Intrasplenic subcapsular contrast accumulation occurred in 56.2% (18/32; minimal 15, moderate 3), extrasplenic contrast leakage in 12. 5% (4/32), and left shoulder pain in 18.7% (6/32). No major complications were observed. CONCLUSIONS: Direct intrasplenic contrast injection for CT portography is a simple, effective, and safe technique with a high success rate and requires significantly less time and lower doses of contrast medium; it also eliminates angiography, indwelling arterial catheters, and patient transfers from angiography to the CT area.

Utility of thin layer chromatography for detection of opioids and benzodiazepines in a clinical setting.

This study examined the utility of thin layer chromatography (TLC) for detection of recent use of opioids and benzodiazepines among drug addicts seeking treatment at the Drug Dependence Treatment Centre of All India Institute of Medical Sciences, New Delhi, India. Over a period of 5 years (1991-1995), 6,055 urine samples were analyzed for opioids (morphine, codeine, buprenorphine, dextropropoxyphene, pentazocine) and benzodiazepines (diazepam, nitrazepam) by TLC. Out of all the drug tests (n = 9,922) carried out, 24% of the drugs had been used during the past 72 hr. Averaged across all drugs, the detection rates corresponding to 24, 48, and 72 hr by TLC were 37%, 36%, and 31%, respectively. A high percentage of negative TLC results was observed in these samples. Moderate sensitivity of the TLC assay procedure, low consumption of drug, short time between drug use and urine collection, overreporting by the subjects, and drug use history of the subject obtained from multiple sources led to high negative results. These findings suggest that all the TLC negative results also need further confirmation by an alternative, more sensitive technique in a clinical setting. This will make the drug abuse testing program more meaningful.

Validity of health status measures in patients with ulnar wrist disorders.

PURPOSE: To assess measurement properties and construct validity of health status measures. METHOD: Forty-three patients with surgically managed ulnocarpal impingement completed a self-report mail survey, including regional (Disabilities of the Arm, Shoulder and Hand [DASH] questionnaire), disease-specific (Brigham Functional Scale), and generic (Short Form 36 [SF-36] Acute Health Survey) health status measures and questions on condition severity and work status. STATISTICAL ANALYSIS: Scores were transformed and frequency distributions constructed to compare the distribution of responses to the measures. Correlation analysis and analysis of variance were applied to assess construct validity. RESULTS: The DASH and Brigham questionnaires had similar distributions of scores, with a slightly greater spread of responses and no ceiling effect on the DASH. Patients appeared slightly less healthy on the basis of the SF-36 scores, which reflected in part the effect of comorbidities. Both the DASH and the Brigham discriminated across levels of severity and work status; the DASH also discriminated on the basis of type of surgery. The SF-36 was able to discriminate some constructs but not as well as the regional and disease-specific measures. CONCLUSION: This study provides evidence of construct validity for the DASH and Brigham questionnaires in patients with ulnar wrist problems in the late post-operative period.
