Apert syndrome associated with septo-optic dysplasia.

Apert syndrome and septo-optic dysplasia are rare congenital structural disorders which have not been associated previously; we report a female infant with this association. Brain sonography and computed tomography revealed agenesis of the septum pellucidum; optic hypoplasia was demonstrated by funduscopic evaluation. We postulate that an embryopathic factor, which manifests at 5-6 weeks gestation, may result in this new association. Transverse craniectomy was performed in the hope of preserving brain development. Neither hypopituitarism nor adrenal insufficiency was observed; however, clinical manifestations and laboratory data provided evidence of inappropriate antidiuretic hormone syndrome.

Intestinal malrotation and gastric perforation in a newborn with tetralogy of Fallot: report of one case.

Gastric perforation with gastrointestinal anomaly is rare in newborns. A case of Tetralogy of Fallot with both malrotation and gastric perforation is reported. Before operation, malrotation was diagnosed from the x-ray films. Laparotomy disclosed a linear perforation on the posterior wall of the gastric body. Mechanical obstruction might have played a role in this case.

The neonatal morbidity and mortality of low birth weight neonates.

During the period between January 1, 1987 and March 31, 1991, a total of 454 low birth weight neonates, classified if below 2,500 grams, admitted to this hospital, 450 of them could be evaluated retrospectively. Among the 450 cases, 41 (9.1%) were transferred from outside hospitals within 24 hours of birth; and 409 were original hospital inpatients. The sex distribution was quite even (M/F = 227/223), but more than half of them were small-for-date (56.4%). Multiple pregnancy accounted for 22.9%, and toxemic mother, for 11.6%. Neonatal mortality rates were 23.9% for those with birth weight below 1,500, and 45.5% for those with birth weight below 1,000 grams, respectively. Respiratory distress syndrome occurred in 15.9% of the low birth weight neonates, and the incidence increased stepwise as birth weight decreased. Symptomatic patent ductus arteriosus could be detected in 11%. Fundoscopical evaluation, a routine for all with birth weight below 1,500 grams or below 2,000 grams with prolonged ventilator support, demonstrated retinopathy of prematurity in 5.6% of all cases or 50% in those below 1,000 grams. Routine brain echo done in all those below 2,000 grams within the first three days of age, and at least once every week thereafter, revealed intracranial hemorrhage in 10.2%, but only 2.4% belonged to grade III-IV. Respiratory distress syndrome and outborn were the two major determinants affecting the severity and occurrence of intraventricular hemorrhage significantly.(ABSTRACT TRUNCATED AT 250 WORDS)

Late metabolic acidosis in low-birth-weight babies: report of four cases.

Four low-birth-weight babies, though fed an adequate amount of formula, failed to regain their birth weight within ten days. Blood gas studies revealed metabolic acidosis in all. Their body weight gain improved dramatically after adding sodium bicarbonate to the milk. Late metabolic acidosis was presented, even using special premature formula, in three of them.

Time of the first urine and the first stool in Chinese newborns.

A total of 1,000 consecutive newborns were studied for the time of passage of the first urine and the first stool. Delayed passage (greater than 24 hours) of the first urine was noted in 4% of 920 adequate birth weight infants (birth weight greater than 2,500 grams) and 3.8% of 80 low birth weight infants (birth weight less than or equal to 2,500 grams). Whereas, delayed passage of the first stool was noted in 2.0% of adequate birth weight infants and 11.3% of low birth weight infants. By the age of 48 hours, all infants had passed their first urine, while, 99.9% of adequate birth weight infants and 97.5% of low birth weight infants had passed their first stool. There was significant difference in the gestational age, birth weight, presence of respiratory distress syndrome and delay in the first enteral feeding between the delayed and nondelayed stool passage groups.

Urinary N-acetyl-beta-glucosaminidase in normal Chinese children and children with pyelonephritis.

Urinary excretion of N-acetyl-beta-glucosaminidase (NAG) was measured in random urine as the ratio of NAG to grams of urinary creatinine in 120 normal Chinese children. The results showed that the NAG was elevated in newborn babies and infants, and decreased to adult level after two years of age. Thirteen children with urinary tract infection, who were clinically suggested or proved to have pyelonephritis, were evaluated for their urinary NAG excretion. The level of enzymuria at diagnosis in these patients was significantly higher than that of normal children for age. Eleven longitudinal follow-up data including levels at diagnosis, end of treatment, and two to four weeks after treatment showed that the mean urinary NAG at diagnosis was 109.82 +/- 87.30 u/g creatinine, compared with 75.26 +/- 48.82 u/g creatinine at the end of treatment and 15.37 +/- 9.69 u/g creatinine after recovery. These data suggest the presence of renal tubular cell injury in upper urinary tract infection. Thus urinary NAG may play a role in differentiating lower from upper urinary tract infections.

Isolated thrombocytopenia after bone marrow transplantation in Cooley anemia: report of one case.

A nine-year-old boy, victim of Cooley anemia, received allogeneic bone marrow transplantation with his younger brother serving as the histocompatible donor. The myeloid and erythroid series recovered very quickly, but his platelet count never reached 10(5)/mm3 until the 220th day. Platelet-associated IgG (PAIgG) was checked on the 137th and 305th day with the level of 31.75 fg/pl and 6.84 fg/pl, respectively. Immune mediated process was considered to be the cause of his prolonged thrombocytopenia.

Clover-leaf nucleus of atypical lymphocytes in CMV-mononucleosis syndrome: report of one case.

Atypical lymphocytes with clover leaf nucleus are a frequent finding in adult type T-cell leukemia, and are occasionally seen in an overnight blood sample. This type of cell has apparently never previously been reported in cytomegaloviral infection. The case of a thirty-seven-day-old female infant presented with cytomegalovirus induced mononucleosis syndrome; her blood smear revealed atypical lymphocytes with clover leaf nucleus.

Diagnostic value of arterial hematocrits in neonatal polycythemia.

In order to study the correlation among arterial, capillary and venous hematocrits, sixty newborn babies delivered in our hospital were chosen after receiving their parents' consents for hematocrits study within their first eight hours of life. The venous hematocrits of them (59.77 +/- 14.15%) correlate with arterial hematocrits (57.84 +/- 14.36%) very well (r = 0.95, p less than 0.001). Whereas, the venous and arterial hematocrits correlate with capillary hematocrits (66.63 +/- 15.56%) also significantly (r = 0.72, p less than 0.001 for both). Arterial hematocrits of more than 62% is proportionate to venous hematocrits of 64% and above. The relationship between arterial and venous hematocrits does not change even after we divide the subjects into those venous hematocrits above 64% and those below 64%. Furthermore, the capillary hematocrits of above 70% can be used as the criteria of polycythemia in the neonatal screening. If the capillary hematocrit is above 70%, then venous hematocrit should be checked for the confirmation of polycythemia.

Moyamoya disease manifested initially by repeated attacks of adversive seizure: report of one case.

A nine-year-old Chinese boy had suffered from recurrent episodes of adversive seizure for about a year, seizures which affected the left upper limb, with his head and eyes turning toward the left. An acute attack of left upper-limb weakness and central-type facial palsy occurred before his consultation. Cranial computed tomography (CT) revealed infarction of the right frontoparietal area, and increased density in the head of the left caudate nucleus. Cerebral angiographic study demonstrated a typical pattern of moyamoya disease with bilateral stenosis of the supraclinoid portion of internal carotid arteries. Encephaloduroarteriosynangiosis with encephalomyosynangiosis were performed twice, with resulting good patency of blood flow observed in the follow-up angiographic study.

Salmonella meningitis complicated with subdural empyema, brain abscess and purpura fulminans: report of one case.

There exists high incidence of bacteremia, sepsis and meningitis in young infants with Salmonella infection. However, focal intracranial abscesses due to Salmonella infections are rare. We reported a 2-month-old male baby presenting salmonella infection with brain abscess and purpura fulminans. The patient's clinical course was fulminant. He was admitted due to fever, irritability, anemia and leukopenia. He developed cardiac arrest, shock and skin diathesis on his second hospitalization day. After resuscitation he became comatous and showed acrocyanosis and gangrenous skin over the hands, feet and left ear lobe. Both blood and cerebrospinal fluid cultures were Salmonella Group B. The patient got worse rapidly in spite of vigorous treatment. Subdural empyema, ventriculitis and later brain abscess were detected by serial brain sonograms. He died of central nervous system failure, gastrointestinal bleeding and renal failure on the eighteenth hospitalization day.

Neonatal hypermagnesemia: report of one case.

A female baby was born prematurely to a pre-eclampsic mother at our hospital. The mother had received fifty grams of magnesium sulfate intravenously within the forty-six hours prior to the delivery. This neonate suffered from severe respiratory distress, hypotonia, and hyporeflexia immediately after birth. She recovered completely after receiving low setting ventilatory support for twelve hours and intravenous calcium gluconate supply. All laboratory data were within normal limits except transient metabolic acidosis and elevated serum magnesium level to 2.28 mmol/L of the cord blood. The serum magnesium level dropped to 1.17 mmol/L at 12 hours of age. She tolerated oral feeding within the first day with no obervable neurological sequelae at the follow-up examination. Hypermagnesemia was judged to be the cause of the newborn's clinical presentation.

Stable microbubble test for predicting the occurrence of respiratory distress syndrome.

To predict the development of respiratory distress syndrome (RDS) in premature neonates who might benefit from early administration of surfactant, the clinical applicability of stable microbubble (SM) test was prospectively evaluated. Fifty-nine preterm neonates with the gestational ages of less than 37 weeks were studied. SM test was performed on gastric aspirates obtained within 30 minutes after birth. The number of microbubbles was correlated with later development of RDS. With a cut-off value of less than 10 microbubbles/mm2, the SM test showed a positive predictive value of 76.0% (19/25) and a negative predictive value of 97.1% (33/34), while sensitivity and specificity were 95.0% (19/20) and 84.6% (33/39), respectively. The overall accuracy was 88.1% (52/59). It was concluded that use of the SM test on gastric aspirates immediately after birth is a rapid, simple and reliable procedure for identifying those neonates with surfactant deficiency who may benefit from prophylactic surfactant therapy.

Cytomegalovirus-induced persistent mononucleosis in an infant.

Chronic mononucleosis has recently gained much attention in the literature. Almost all cases of this new syndrome have been ascribed to persistent Epstein-Barr virus infection. However, a case presented with infantile chronic mononucleosis syndrome caused by persistent cytomegalovirus infection. Transient clonal chromosomal change was noted during follow-up.

Transient leukemoid reaction in an extremely premature neonate.

A female newborn was delivered prematurely at gestational age of 26 weeks with birth weight of 1,000 grams. Her initial white blood cell count was 68,900/mm3, which increased to 207,000/mm3 on the fifth day of life with a remarkable shift-to-left pattern. The most immature myeloid series in the peripheral blood smear was promyelocyte; and the promyelocyte, myelocyte and meta-myelocyte accounted for 38% of the peripheral white blood cells. Neither hepatosplenomegaly nor skin rash was noted, but the platelet count dropped to 86,000/mm3 transiently on Day 8. Cytogenetic study for the bone marrow aspiration revealed normal results. The leukemoid reaction recovered within three weeks without specific treatment. Antenatal usage of steroid to promote lung maturation was considered to be one possible cause of the transient leukemoid reaction.

Intrauterine growth of twins in Taiwan.

During the period between January 1, 1980 and December 31, 1991, there was a total of 21,348 live born neonates delivered in our hospital. Among them, there were 368 pairs of twins and 18 sets of triplets. The incidences of twin and triplet pregnancy were 1.76% (one pair in every 57.8 live deliveries) and 0.086% (one set in every 1,164 live deliveries), respectively. The mode of gestational age for all twins in this study was 36.6 +/- 3.2 weeks (36.6 +/- 3.6 weeks for vaginal delivery and 36.7 +/- 2.8 weeks for cesarean section, respectively). Among twins delivered by cesarean section, the birth weight of the first born twin was significantly heavier than the second born twin; however, this difference was not observed for those twins delivered vaginally. Also, the birth weight between male and female was not significantly different. However, for those twin pairs of different sexes, the birth weight for the male twin was significantly heavier than the female co-twin. In those cases the mean birth weight increased steadily as the gestational age increased and plateaued at about the 40th week. Compared to the intrauterine growth curve of the Chinese singleton, the difference started from the 33rd gestational week and reached a significant level at about the 37th gestational week. This may indicate the incipient of the placental insufficiency for multiple pregnancies. The intrauterine growth curve for twin pregnancies may serve as guide for studying the mortality and morbidity of twins in the future.

Trismus-pseudocamptodctyly syndrome: report of one case.

Trismus-pseudocamptodactyly syndrome is a rare neuromuscular disorder that manifests as a combination of hand, foot, and mouth anomalies. Limited range of motion for hands, feet, and mouth are typical presentations. A male term newborn baby was noted to have trismus, ulnar deviation of both wrist joints, limited stretch of digits during hand dorsiflexion but with full digital extension during volarflexion, and talipes equinovarus of the right foot. These anomalies were not seen in his parents. Hecht syndrome, or trismus-pseudocamptodactyly syndrome, was diagnosed with a negative result of electromyographic study.
