Unstable fractures and dislocations of the pelvic ring--results of treatment in relation to the severity of injury.

Between 1968 and 1984, III patients were treated for unstable fractures of the pelvic ring. Cause, incidence, accompanying local lesions, mortality, Injury Severity Score (ISS) and mode of treatment were retrospectively analysed. ISS and mortality were closely correlated. The overall mortality was 19% and the average ISS 53. The mortality of the conservatively treated patients was 21.8%, for the operative group the mortality was 8.3%, while the ISS was the same in both groups. From these results we conclude that unstable pelvic fractures, which are almost invariably associated with other serious injuries, should be treated aggressively by external and/or internal fixation.

Tendon sheath infections of the hand.

Septic tenosynovitis continues to be a serious condition of the hand. Complications, sometimes necessitating amputation, are not uncommon. We studied the anatomic arrangements of the flexor tendon sheaths and were able to demonstrate different synovial compartments within the one osteofibrous tendon sheath. This has important consequences for surgical therapy. 48 Patients are reported operated on for tendon sheath infection over a 10-year period. All data influencing the end-result are analysed, with particular attention to delay in operative treatment, the results of bacterial cultures, the use of antibiotics and the surgical technic itself. When tenosynovitis is suspected, immediate drainage of all the different compartments of the tendon sheath is required.

Free revascularised periosteum transplantation: an experimental study.

Two different types of revascularised periosteal graft were used to bridge an experimental bone defect and compared with control groups. Revascularised tibial periosteal grafts completely filled the defect with new bone, whereas revascularised rib periosteal grafts failed to do so.

Lisfranc dislocations: fractures and/or dislocations through the tarso-metatarsal joints.

Because of the anatomical configuration of the tarso-metatarsal joints with their strong ligamentous connections, pure dislocations in this region are rare. We describe our experience of 24 cases of Lisfranc dislocation. Classification is according to Wilson. In the long term, functional and radiological results appear to depend on the accuracy of reduction. For good anatomical results, immediate closed or, if necessary, open reduction and stabilization by percutaneous Kirschner wires is required. Because of the severity of soft tissue damage, decompression with dorsal skin incisions should also be considered.

Comparison of saphir infrared coagulation and YAG-laser in the surgery of parenchymatous organs: an experimental study.

In an experimental study the efficiency of infrared coagulation and the neodymium-yttrium-aluminium and garnet laser (YAG-laser) were compared in operations on the liver, spleen and kidney in the dog. In surgery of the liver, both the YAG-laser and infrared coagulation gave good results. In surgery of the spleen, results with infrared coagulation were excellent, allowing extensive transections and partial resections. Kidney surgery was unsuccessful with either method. Our results are consistent with comparable clinical experience, in that infrared contact coagulation is extremely useful in surgery of the spleen.

Revascularized periosteum transplantations.

A controlled experiment was performed in 56 African pygmy goats, analyzing the bone-forming capacity of two different revascularized periosteum grafts in midshaft tibial defects. At radiological examination revascularized tibial periosteum grafts firmly bridged the defect within 8 weeks, revascularized costal periosteum grafts did not.

Early detection of hemorrhagic hypovolemia by muscle oxygen pressure assessment: preliminary report.

Skeletal muscle PO2 was studied during graded hemorrhagic shock in Labrador dogs by means of a polarographic needle PO2 electrode. Compared to hemodynamic, blood gas, biochemical, and hematologic variables, a shift to the left of the cumulative histogram of skeletal muscle PO2 was the earliest and most sensitive indicator of impaired skeletal muscle oxygenation. The mean arterial blood pressure and the mean of the medians of the skeletal muscle PO2, respectively, were as follows: during the control period 125 and 38.1 mm Hg, during the imminent shock period 143 and 24.5 mm Hg, and during the shock period 89 and 5.1 mm Hg. This electrode is suitable for clinical use.
