Monoclonal gammopathy in beta thalassaemia.

A rare instance of IgG-kappa monoclonal gammopathy in a patient with beta-thalassaemia trait is reported. The patient had a smoldering multiple myeloma with no apparent clinical symptoms pertaining to the disease and was a non-responder to conventional Melphalan and prednisolone therapy. Beta-thalassaemia trait was detected while investigating him for anaemia. A state of altered immunological reactivity, chronic infections and associated biliary tract disease are some of the pathogenetic mechanisms suggested. However in this patient none of these were operative.

LH and FSH responses to GnRH in health and disease.

LH and FSH responses to intravenous bolus GnRH were evaluated in healthy men and women and in patients with various hypothalmo-pituitary disorders. Higher LH FSH responses to GnRH were recorded. In prepubertal girls FSH responses were higher than those of LH; however, while FSH responses remained unaltered through pubertal development stages 2-5, there was a progressive increase in LH response during that period. GnRH test was marginally useful in diagnosis of delayed puberty but more so in hypogonadotropic hypogonadism and true precocious puberty. In experimental hyperprolactinemia, GnRH induced a near normal LH response but caused an exaggerated FSH response, which was normalized after clomiphene treatment. GnRH test was found to be useful to assess pituitary reserve of gonadotropins in a variety of clinical situations involving hypothalamo-pituitary gonadal axis.

Serum thyroxine and triiodothyronine concentrations in rats receiving lithium carbonate.

Circulating thyroxine (T4) and triiodothyronine (T3) concentrations in rats were determined after various periods of lithium administration in diet. No significant change in serum T4 and T3 levels was observed after ten days of lithium treatment. However, lithium treatment for one month, two months and four months showed a significant decrease in serum T4 and T3 levels indicating the adverse effect of chronic lithium intake.

Galactorrhoea amenorrhoea syndrome due to internal carotid artery aneurysm.

A 32 year old female with hyperprolactinaemia-galactorrhoea-amenorrhoea due to a right internal carotid artery aneurysm just before its bifurcation is described. She had two episodes of subarachnoid haemorrhage necessitating an emergency internal carotid artery ligation. She responded to bromocriptine treatment with restoration of her menses, normalization of circulating prolactin and disappearance of galactorrhoea.
