[Indications for preventive regional lymphadenectomy in primary melanoma of the skin of the head and neck].

The authors have conducted a retrospective analysis of results of the surgical treatment of primary melanomas of the skin in the area of head and neck in 86 patients. Five-year survival of the patients made up 53.5 +/- 5.4%. On the basis of studying the dependence of incidence of regional metastases of primary tumors on most important clinico-morphological factors the indications were developed for performing prophylactic cervical regional lymphadenectomy. The authors consider that the selective prophylactic lymphadenectomy will contribute to the improvement of long-term results of treatment of patients with primary melanomas of the skin of head and neck.

[Survival of patients with lung cancer (based on data of the All-Union Center for Cancer Therapy Research)].

The report discusses the results of a sampling survey conducted by the All-Union Center for Research in Efficacy of Neoplasm Treatment. The study was based on the data on 7173 inpatients with lung cancer treated at 38 oncological establishments in the USSR in 1974-1980. Survival was evaluated versus sex, age, stage and treatment modality. Average 5-year survival was 9%, relative survival rate--10%. The scope of therapeutic effort needed to assure maximum survival for each stage was established. The available resources for stepping up the effectiveness of medical help were identified on the basis of relative survival levels.

[Incidence of metastatic lesion of different groups of the intrathoracic lymph nodes in lung cancer (based on research data on surgical preparations)].

The study was concerned with the frequency of metastatic spread of lung cancer to intrathoracic lymph nodes versus lung lobe distribution of malignancy in 299 cases of surgical treatment. In cases of primary tumor in any lobe of both lungs, metastases were detected primarily in the pulmonary and bronchopulmonary lymph nodes. A decreasing frequency of metastases was observed from the intrapulmonary lymph nodes to those of the root of the lung and mediastinum.

[Current possibilities of detecting and diagnosing the early forms of lung cancer].

A long-term experience with oncopulmonary examinations points to their efficacy in detecting lung cancer, early forms included. As a result, the number of primary patients operated on for lung cancer or undergoing lung resection has grown in Leningrad for the past 17 years from 15.6 to 22% and from 13.2 to 19%, respectively. The best diagnostic results for central cancer have been obtained in application of bronchoscopy and with transcutaneous needle biopsy--for peripheral cancer. (The rate of morphological verification of cancer was over 96% for either procedure). The following patterns of early forms of central cancer of the lung have been identified: Ca in situ, microinvasive cancer, incipient invasive cancer and advanced invasive cancer.

[Extent of the surgical intervention in regional metastases of melanomas of the head and neck].

A retrospective analysis of results of surgical treatment of regional metastases of melanomas of the head and neck skin in 64 patients has been made. Five year survival was 26.5%. The authors recommend to extend the typical radical cervical lymphadenectomy at the expense of removal of the subaural, mastoid and intraglandular lymph nodes depending on the localization of primary melanomas and their regional metastases.

[Indications for preventive axillary lymphadenectomy in melanoma of the skin of the upper extremity].

Based upon clinico-morphological analyses of data of 79 patients with primary melanomas of the upper extremity skin the authors have found 3 risk factors of the regional metastasing process. It was shown that in patients having these risk factors there is 50% higher probability of regional metastases of melanomas which may be taken as indications for performing prophylactic axillary lymphadenectomy in such patients.

[Surgical results in lung cancer in women].

The paper deals with the immediate and long-term results of treatment of 173 female and 1,330 male patients with lung cancer operated on at the Department of Lung and Mediastinal Tumors of the Institute in 1960-1979. The results of radical surgery were better in females: 5-year survival rate was 1.5 times that in males (44.4% vs 31.4%) whereas at 10 years it was twice as high (35.5 vs 17.2%).

[The role of intra-operative biopsy in the diagnosis of regional metastases of lung cancer].

The paper discusses the results of a histologic study of intrathoracic lymph nodes removed during surgery for lung cancer in 621 cases at different stages of metastatic spread. The gross appearance of some lymph nodes did not match their real histologic pattern: about 36% of grossly-identifiable metastatic nodes were microscopically shown to be intact whereas about 12% of grossly normal lymph nodes proved to be involved, particularly in cases of adenocarcinoma. Most of the enlarged and compact lymph nodes in cases of low-differentiated lung carcinoma revealed metastases. Intraoperative biopsy of intrathoracic lymph nodes followed by microscopy serves as a highly-predictive procedure in assessing the extent of regional dissemination of lung cancer.

[Relation between the prognosis of skin melanomas and the timing of appearance of regional metastases].

As a result of an evaluation of the data on 319 primary patients with stage II skin malignant melanoma (after Sylven), it was found that among cases of skin melanoma metastases into regional lymph nodes patients with synchronous primary tumor and its metastases have a worse prognosis than those suffering from metachronous involvement of regional lymph nodes. A statistically significant direct correlation was established between the period from primary tumor excision to regional metastasis development and rates of 5-year survival. It was found in 92.9 +/- 2.3% of skin melanoma patients that regional metastases developed within the first 3 years after primary tumor excision. The so-called "metastasis-free" 3-year interval is the border line between favorable and unfavorable prognosis.

[Indications for prophylactic lymphadenectomy in skin melanoma of the lower limbs].

The paper discusses the influence of the 12 most important clinico-morphological factors on the incidence of regional lymph node metastases development after surgical removal of primary tumor in 229 cases of skin melanoma of the lower extremities. The following factors of risk for regional metastases development were identified: primary melanoma thickness not less than 3.0 mm after Breslow; depth of primary tumor invasion into underlying tissues after Clark et al.--IV-V; localization on foot; age of 40-59 years, and male sex. A combination of 3 or more risk factors makes the case for extensive dissection of primary tumor involving prophylactic lymphadenectomy.

[Indications for preventive regional lymphadenectomy in primary melanoma of the skin of the trunk].

A retrospective analysis of clinico-morphological data of 222 patients with primary cutaneous melanoma of the trunk was made. General results of 5-year survival of the patients after a radical surgical treatment was 56.3 +/- 3.3%. Five clinico-morphological risk factors of the regional metastazing of the primary cutaneous melanoma of the trunk were established. Prophylactic regional lymphadenectomy along with a wide dissection of the primary tumor is recommended by the authors for patients with primary melanomas of the trunk skin having 3 and more risk factors of regional metastazing.

[Possibilities of increasing the number of patients with lung cancer undergoing radical treatment].

The report deals with certain factors preventing radical treatment of lung cancer as well as recommendations with respect to cutting down the number of totally-untreated patients and those who are not treated radically. Out of the total of 11,110 patients, radical treatment was carried out in 2,664; far-advanced cases--4,124; 2,527 patients refused to be operated on: surgery was contraindicated because of concomitant diseases in 840, and surgery was refused in consideration of advanced age in 1,157 cases. It was shown that the proportion of radically-operated cases can be increased by 176,2% (2,264 surgical cases taken as 100%) provided that patients' mistrust of surgery leading to refusal is overcome, contraindications for surgery due to concomitant diseases are established on a more realistic basis and radical treatment is not contraindicated for the mere reason of advanced age.

[Place of surgical intervention in the treatment of small cell lung cancer].

On the basis of evaluation of their own findings (162 cases) and data available in the literature the authors suggest that surgery be used as a component of combined treatment for small cell lung cancer. Preoperative assessment of tumor process expansion appeared to be unreliable. Pneumonectomy was found to remain an operation of choice although application of extended pneumonectomy to deal with tumor expansion should not be recommended because the end results do not differ from those of conservative (chemoradiation, polychemotherapy) treatment.

[Results of radiotherapy of lung cancer in women].

Radiation therapy was provided to 38 (11.5%) of 331 lung cancer female patients at the N. N. Petrov Research Institute of Oncology, USSR Ministry of Health. The combination of radiation- and chemotherapy was used in 9 patients. The Rokus, Rad gamma-therapeutic units or a linear accelerator were used for radiation therapy. Daily single focal doses were 1.2-2.0 Gy, the summary dose was 40-60 Gy. Two-five irradiation fields with the choice of optimum conditions on a computer using the linear programming method were used. The lymph outflow tracts were irradiated from isolated fields and in central lung cancer the tumor and mediastinal region (the root, bifurcation and paratracheal lymph nodes on the affected side) were incorporated in the irradiated zone. Five women received split course irradiation. An analysis of the long-term results of conservative therapy of lung cancer in women has shown that radiation therapy or its combination with chemotherapy is the most effective. These results were slightly better than those in men.
