[Change in portal system and liver hemodynamics under the influence of operations on the gastrointestinal tract organs and the basic principles of its correction].

A comprehensive study of hemodynamic indices of the portal system and liver (portal pressure, linear rate of portal blood flow in the liver, rheohepatography) as well as protein-formation function of the liver, its enzymatic activity, in the complex with histomorphology of hepatic tissue and comparison of the data, obtained by some functional tests of general hemodynamics (AP, VP, CVP, linear rate of total blood flow, the porto-caval gradient, TCB), rendered it possible to reveal significant disturbances in hepatic circulation and this organ functioning with satisfactory indices of general hemodynamics. Taking into account the data obtained, the complex therapy for the changes observed was elaborated by the authors; its main principles being as follows: the maintenance of general hemodynamics, restoration of TCB and its components deficiency, the provision of the organ (hepatic) blood flow, the correction of metabolic changes in the liver intra-and postoperatively. The entire complex of drugs was infused in the umbilical vein.

[Anesthesiologic protection of the mother and fetus during abdominal delivery].

Central hemodynamics at different stages of anesthesia, changes in hexenal concentration in the mother's body, adequacy of anesthesia and hexenal effect on the condition of the fetus and newborn have been studied in 40 pregnant women without any accompanying extragenital pathology subject to planned cesarean section performed under combined general anesthesia. Normodynamic type of circulation has been established. The analysis of blood catecholamine changes confirmed the adequacy of the anesthesia. The studies of hexenal concentration have demonstrated the absence of negative effect on the newborn delivered by cesarean section.

[Factors affecting the permeability of cellular and vascular membranes and changes in the enzyme spectrum of the blood in patients having undergone critical states and resuscitation].

126 patients recovered after critical conditions and resuscitation have been examined. Activation of lipid peroxidation and an increase in biogenic amine level, leading to marked hyperenzymemia, have been established. The activity of blood enzyme markers reflecting the severity of brain damage has been studied. It has been established that the more severe encephalopathy, the higher the activity of blood creatine phosphokinase. 2,3-cyclonucleotide-3-phosphohydrolase (CNPH), reflecting the damage of myelin membrane of nervous fibers, has been studied. The appearance of myelin-CNPH marker in the blood confirms demyelinization of the central nervous system (CNS). The more severe encephalopathy, the more marked the process of CNS demyelinization, the higher the activity of CNPH in the blood serum. The highest CNPH activity is observed in patients with eclampsia and in dead patients with postsystolic syndrome.

[The efferent therapy of the critical state due to status asthmaticus].

A total of 200 patients with severe infection-induced bronchial asthma (BA) admitted to hospital with a diagnosis of status asthmaticus (SA) have been subjected to complex examination. It has been demonstrated that SA is a severe combined toxemia that leads to insensitivity of bronchial receptor apparatus to broncholytic effects and requires efferent therapy. 50 out of 130 patients with I to II and III degree SA were resistant to conventional intensive care procedures. The patients were successfully treated with urgent hemodynamic low-volume fraction extracorporeal hemosorption (EHS). EHS ensures positive hemodynamic and detoxicating effects, removing "critical" concentration of toxic ligands that block broncholytic adrenoreception. The use of efferent therapeutic techniques in SA makes it possible to reduce the mortality rate, promote the condition arrest, relieve the critical condition, reduce the doses of broncho- and spasmolytics, glucocorticoids, facilitate future drug withdrawal, and reduce the duration of the patients' stay in intensive care units.

[Metabolic changes in patients operated on in gynecologic diseases under ketamine anesthesia].

Changes in biochemical parameters have been studied in 58 patients during short-term gynecological surgery performed under ketamine or ketamine-diazepam anesthesia. Surgery under ketamine monoanesthesia was accompanied by a considerable activation of sympathoadrenal and hypophyseal-adrenal systems, which was regarded as inadequate anesthesia. A significant decrease in plasma histamine level has been observed.

[A clinico-experimental study of lymphogenous detoxification in acute renal and kidney insufficiency].

Lymphogenous methods for detoxification were employed in 23 patients with acute concurrent renal and hepatic failure of various etiology. Pronounced changes in the composition of lymph and moderate to severe hyperhydration are indications for combined application of lymphosorption (LS) and lymphodialysis (LD). The combined use of LS and LD makes it possible to reduce the number of required hemodialyses three times as much as that of hemodialysis alone and to make the time of a hemodialysis shorter by 1.5-2 hours.
