The metabolism of floctafenin in man and rodents.

Floctafenin (FFn), 2,3-dihydroxypropyl--N--(8--trifluoromethyl--4--quinolyl) anthranilate, a new nonnarcotic analgesic drug, was studied in man, mice, and the isolated perfused rat liver. In all species the drug is rapidly hydrolyzed to floctafenic acid (FFa). In seven volunteer subjects who each received a single oral dose of 400 mg floctafenin on an empty stomach, the blood concentration of FFa usually reached a maximum between 1 and 2 hours (mean 1.57 +/- 1.28 microgram/ml at 1.5 hours) and declined over the next 6 hours. Eight hours after drug administration the mean concentration of FFa in the blood of the volunteers was 0.1 +/- .05 microgram/ml. Approximately 25 per cent of the administered dose of floctafenin was recovered as FFa and hydroxy-FFa in the urine collected from each subject for 48 hours after drug administration. In mice each having received a single intraperitoneal dose of floctafenin (2 mg), the concentration of floctafenin declined by about 50 per cent in 15 minutes, and this decline was accompanied by a rise in the concentration of FFa that remained constant for 3 hours. The analgesic effect observed after administration of floctafenin to humans is likely to be mediated by its major metabolite, FFa. In these volunteers no free floctafenin was detected in the blood.

Unusual silica granulomas of the skin with massive involvement of axillary lymph nodes.

Silica granulomas are rare in the skin. Involvement of the regional lymph nodes seems to be unusual. The case of a 35-year-old man with massive diffuse fibrosis of the axilla with multiple foreign body granulomas and involvement of axillary lymph nodes is presented. The lymph nodes exhibit multiple epithelioid cell granulomas and silicotic nodules which are indistinguishable from similar nodules in mediastinal lymph nodes in pulmonary silicosis. It is assumed that the numerous silica particles which were identified by X-ray microanalysis under the scanning electron microscope and recovered from the mineral ash after incineration were accidentally implanted into the skin at the time of an injury 26 years previously. The long delay between silica exposure and granuloma development is a typical feature of these lesions. The possible mechanisms operative in this process are discussed.

Surface investigations of oxide layers on cobalt-chromium-alloyed orthopedic implants using ESCA technique.

The uppermost monolayers of a metal implant are far more important for the corrosion resistance than the alloy itself. Using electron spectroscopy for chemical analysis (ESCA) technique these oxide layers on Vitallium specimens were investigated regarding thickness, elemental composition, and valence state of the elements. The oxide layers on metal implants are not very strong, as they can be easily damaged mechanically. According to environmental conditions, oxide layers of different elemental composition can be grown on the same specimen.

Clostridial myonecrosis ('gas gangrene') during cephalosporin prophylaxis.

Four cases of clostridial myonecrosis that developed in open fractures were treated with surgical debridement and with intravenous cephalosporins as antibiotic prophylaxis. All patients recovered following amputation of the involved extremity, and treatment with high-dose penicillin in three cases, and erythromycin plus high-dose cephalothin in the fourth. This complication was not seen in patients with similar injuries that were managed surgically but used other antibiotics, usually penicillin, as prophylaxis. In vitro susceptibility tests of clostridia to cephalothin were performed; the results demonstrated that nearly 50% of clostridia tested were resistant to cephalothin.

[Investigations of oxide layers on pure metals using ESCA technique (author's transl)].

The principals of the ESCA technique are described in details. Oxide layers on pure metals are investigated regarding composition and growth velocity. The valence state of a metal and the thickness of its oxid layer at the time of implantation into human body tissue are highly responsible for the corrosion resistance of this metal. Compared with Iron, Cobalt and Nickel and oxid layers of pure Titanium, Chromium and Molybdenum did not show further growth of thickness after several days in air. Immediately after removal of the passive layer only the stable and not the reactive oxids of the metal were restored.

Anal carcinoma: a clinical approach to p53 and RB gene proteins.

The role of p53, retinoblastoma (RB) and c-erbB-2 gene proteins in the pathogenesis of anal carcinomas has been examined. A total of 97 patients were included in the study. c-erbB-2, RB and p53 proteins were abnormal in 0, 5 and 34% of the cases, respectively. No correlation was observed between p53 and age, sex, tumour stage, histopathological tumour grading, human papilloma virus (HPV) affection or prognosis. It is concluded that the c-erbB-2 gene is not involved in the development of anal carcinomas; the role of the RB gene is somewhat uncertain, while p53 may be involved in about 1/3 of the cases. However, p53 is not correlated with HPV infection or clinical or histopathological data.

Ultrastructure and electron immunocytochemistry of insulin-producing B-cell tumors from transgenic mice: comparison with counterpart human tumors.

This paper reports on an ultrastructural and electron-microscopic immunocytochemical study of pancreatic B cells from normal mice, pancreatic B cells and derivative tumors from transgenic mice, and tissue from human pancreatic B-cell tumors. In normal and neoplastic B cells from both species, typical immature and mature beta-granules (with spherical cores of variable density) were observed, whereas typical beta-granules with a crystalloid core were only present in human B cells (normal and tumor). A small number of atypical granules were found in distinct neoplastic cells which contained no typical beta-granules. The atypical granules were smaller (100-200 nm diameter) than typical beta-granules (250-450 nm diameter) seen in other cells. Immunoreactivity for proinsulin was localized only to immature granules, whereas insulin and C-peptide immunoreactivities were demonstrated in atypical, immature, and mature granules. In transgenic mouse and human B-cell tumors, insulin immunoreactivity was consistently weaker than the immunostaining for C-peptide. An intragranular, topographic segregation of immunoreactive C-peptide was observed in a population of transgenic tumor cells. Our results showed similarities in antigenic distribution and only slight differences in morphology between human and mouse B cells. Therefore, the transgenic mouse system may prove to be an effective model for studying mammalian B-cell tumorigenesis.

Double-staining methods at the ultrastructural level applying colloidal gold conjugates.

Several double gold-labeling methods have been reviewed, and our own results of applying some of these methods have been included. Of the six double-staining methods tested, four gave satisfactory results (indirect double immunogold staining method, two-face protein A-gold staining method, two-face amplified protein A-gold staining method, and formaldehyde blockade protein A-gold staining method). The other two techniques (sequential double protein A-gold staining method and formaldehyde blockade amplified protein A-gold staining method) showed cross-reaction between the first and second staining sequence.

The c-erbB-2 protein in primary and metastatic breast carcinomas.

Material from 41 patients with primary breast carcinoma and lymph node metastases at the time of primary surgical intervention was immunostained for c-erbB-2 protein, neuron-specific enolase (NSE), and estrogen receptors. Thirty of the primary breast carcinomas were of ductal type. Six were classified as infiltrating lobular carcinomas, 2 were apocrine, 1 was mucinous, and 1 was a tubular carcinoma. One tumor could not be classified as ductal or lobular by light microscopic examination alone. The number of lymph node metastases available varied from 1 to 14 per case (median, 3.9). Nine (22%) of the primary breast carcinomas (8 ductal and 1 apocrine) expressed c-erbB-2 protein and showed c-erbB-2 gene amplification; 12 expressed NSE immunoreactivity. None expressed both markers. Estrogen receptor immunoreactivity was present in 23 of the 41 cases, including 9 of the NSE-positive cases. C-erbB2- protein-positive metastases were present in 18 cases (44%), and in 13 cases all metastases were immunostained. In 5 cases the expression of c-erbB-2 protein varied from metastasis to metastasis. NSE immunoreactivity was expressed in 10 cases, and in 3 cases with minor NSE-positive cell populations the metastatic lesions expressed c-erbB-2 protein as well. All 9 primary breast carcinomas expressing c-erbB-2 protein had lymph node metastases with c-erbB-2-immunoreactive tumor cells. Eight of the 9 c-erbB-2 protein-negative primary tumors with metastases expressing c-erbB-2 protein showed no amplification of the c-erbB-2 gene.(ABSTRACT TRUNCATED AT 250 WORDS)

Proliferative activity of synovial sarcoma: an immunohistochemical evaluation of Ki-67 labeling indices of 52 primary and recurrent tumors.

The relationship between cell differentiation and proliferation in biphasic synovial sarcoma (BSS) and the connection between the two components of these tumors and monophasic synovial sarcoma (MSS) are still unknown. In this study we specifically compared the Ki-67 labeling indices (Ki-67-LI) in the solid/glandular and spindle cell components of primary BSS (n = 8) as well as the values thus obtained in primary BSS both outside clear-cut biphasic areas (n = 7) and in primary MSS (n = 23). We also compare the Ki-67-LI of primary tumors (seven BSS, seven MSS) and their respective first recurrences. We observed that the cells of the solid/glandular component of BSS proliferate significantly faster (p = 0.007) than those of spindle cell areas. No significantly different Ki-67-LI were observed in the comparison of the solid/glandular areas of BSS with the spindle/epithelioid component of BSS without clear-cut biphasia and MSS. No significant differences of Ki-67-LI were observed in the comparison of primary tumors of both subtypes with their respective first recurrences. The implications of the low proliferative activity of the spindle cell component of BSS exhibiting clear-cut biphasia, which was also confirmed in a concurrent study using anti-proliferating cell nuclear antigen/Cyclin PC10, remains to be clarified.
