Children's attentional skills and road behavior.

Switching attention and concentration, 2 skills expected to be used by skillful pedestrians, were studied. A sample of 160 children (aged 4 years 3 months-10 years) played a computer game involving attention switching. To examine concentration, a subset of the children was distracted with a cartoon video while they attempted a difficult task that required matching familiar figures. The same subset was also observed crossing roads. Older children switched faster and were less distracted. Children who were better at switching were more likely to show awareness of traffic when about to cross a road. Children who maintained concentration when challenged by a distracting event crossed the road in a less reckless manner. Parents and educators designing safety programs should take into account the development of these skills.

Basal expression of bradykinin B1 receptor in peripheral sensory ganglia in the rat.

The bradykinin B1 receptor is thought to be induced by tissue injury and inflammation. In the present study, we have investigated whether there is a basal expression of B1 receptor in dorsal root ganglion (DRG) and trigeminal ganglion neurons in rats. A substantial number of neurons in both DRGs and trigeminal ganglia were found to be B1-immunoreactive in rats. Both small and medium-sized DRG neurons were B1-immunoreactive, suggesting that they are likely to be Adelta- or C-fibre neurons which are involved in nociceptive transmission. These results support a possible role for B1 receptors in the modulation of nociceptive sensory transmission.

The intrinsic function of a motor system--from ion channels to networks and behavior.

The forebrain, brainstem and spinal cord contribution to the control of locomotion is reviewed in this article. The lamprey is used as an experimental model since it allows a detailed cellular analysis of the neuronal network underlying locomotion. The focus is on cellular mechanisms that are important for the pattern generation, as well as different types of pre- and postsynaptic modulation. This experimental model is bridging the gap between the molecular and cellular level to the network and behavioral level.

Warm-coding deficits and aberrant inflammatory pain in mice lacking P2X3 receptors.

ATP activates damage-sensing neurons (nociceptors) and can evoke a sensation of pain. The ATP receptor P2X3 is selectively expressed by nociceptors and is one of seven ATP-gated, cation-selective ion channels. Here we demonstrate that ablation of the P2X3 gene results in the loss of rapidly desensitizing ATP-gated cation currents in dorsal root ganglion neurons, and that the responses of nodose ganglion neurons to ATP show altered kinetics and pharmacology resulting from the loss of expression of P2X(2/3) heteromultimers. Null mutants have normal sensorimotor function. Behavioural responses to noxious mechanical and thermal stimuli are also normal, although formalin-induced pain behaviour is reduced. In contrast, deletion of the P2X3 receptor causes enhanced thermal hyperalgesia in chronic inflammation. Notably, although dorsal-horn neuronal responses to mechanical and noxious heat application are normal, P2X3-null mice are unable to code the intensity of non-noxious 'warming' stimuli.

Sustained kilowatt lasing in a free-electron laser with same-cell energy recovery

Jefferson Laboratory's kW-level infrared free-electron laser utilizes a superconducting accelerator that recovers about 75% of the electron-beam power. In achieving first lasing, the accelerator operated "straight ahead" to deliver 38-MeV, 1.1-mA cw current for lasing near 5 &mgr;m. The waste beam was sent directly to a dump while producing stable operation at up to 311 W. Utilizing the recirculation loop to send the electron beam back to the linac for energy recovery, the machine has now recovered cw average currents up to 5 mA, and has lased cw with up to 1720 W output at 3.1 &mgr;m.

NK1 (substance P) receptor antagonists--why are they not analgesic in humans?

Tachykinin NK1 receptor antagonists have failed to exhibit efficacy in clinical trials of a variety of clinical pain states. By contrast, in preclinical studies in animals NK1 receptor antagonists have been shown to attenuate nociceptive responses sensitized by inflammation or nerve damage, although they exhibit little effect on baseline nociception. Other agents with this profile of activity in animal tests, typically nonsteroidal anti-inflammatory drugs (NSAIDs), are analgesic in humans. Thus, NK1 receptor antagonists appear able to block behavioural responses to noxious and other stressful sensory stimuli at a level detectable in animal tests but fail to provide the level of sensory blockade required to produce clinical analgesia in humans.

Increased lymphocyte replicative index following 2,4-dichlorophenoxyacetic acid herbicide exposure.

OBJECTIVE: Evaluate peripheral blood lymphocyte proliferation (replicative index:RI) and micronuclei frequency (MF) among 2,4-D herbicide applicators. METHODS: Twelve applicators spraying only 2,4-D provided a blood and urine specimen upon enrollment, several urine samples during the spraying season, and a blood specimen at the study's end. Nine controls provided blood and urine specimens upon enrollment and at the study's end. Gas chromatography/tandem mass spectroscopy determined urinary 2,4-D levels and standard in-vitro assays determined RI and MF scores. Applicator RI and MF were compared before and after spraying and with controls. RESULTS: Applicators contributed 45 urine specimens with concentrations ranging from 1.0 to 1700 (microg 2,4-D/g creatinine/L urine) that logarithmically (In) increased as spraying time increased. Applicator RI increased after spraying (p = 0.016), independent of tobacco and alcohol use, and demonstrated a weak dose-response with increasing urinary 2,4-D levels (p = 0.15). Among 2,4-D applicators, pre-exposure complete blood counts and lymphocyte immunophenotypes were not significantly different from post-exposure measurements. CONCLUSION: Urinary 2,4-D concentration, an exposure biomarker, may be associated with lymphocyte replicative index, a cell proliferation biomarker.

The EEG consistency index as a measure of ADHD and responsiveness to medication.

The primary diagnostic procedure for Attention-Deficit/Hyperactivity Disorder (ADHD) is the clinical interview, because psychological, neuropsychological, and neurological tests to date have not had sufficient specificity. Currently, there is no objective means to measure severity of ADHD, or the extent to which it is benefited by various dosages of medication. We recently reported that a certain EEG profile, the Consistency Index, occurring during the transition between two easy cognitive tasks clearly differentiated ADHD from non-ADHD boys between the ages of 8 and 12. The current study replicated this with older males (19-25) using different tasks, and a double blind, placebo versus Ritalin controlled crossover design. Seven ADHD subjects were found to have a significantly lower Consistency Index than 6 non-ADHD males while transitioning from 2 Simple tasks during placebo condition, while only the ADHD subjects demonstrated a significant improvement in their Consistency Index while on Ritalin. Similar but nonsignificant trends were observed while transitioning across Hard tasks.

Relationship between fascicle size and perineurial collagen IV content in diabetic and control human peripheral nerve.

AIM: The relationship between perineurial collagen IV content and fascicle size was determined in diabetic and control human peripheral nerve. METHODS AND RESULTS: Age-matched diabetic and control sural nerve samples were immunostained using antibodies to collagen IV. The number of cell layers and the perimeter of the fascicle were measured and the collagen IV content of the perineurium determined. Using this method, a comparison could be made of collagen IV content in the perineuria of fascicles of different size. A positive linear relationship was found between fascicle size and the amount of collagen IV per unit of perineurium. The number of perineurial cell layers and the collagen IV content of the diabetic nerve did not differ from control values. CONCLUSIONS: The linear relationship between fascicle size and perineurial collagen IV content per unit of perineurium underlines the importance of taking fascicle size into account when determining changes in basement membrane components associated with neuropathy. The results indicate that increased deposition of collagen IV is not involved in the early changes in the perineurial cell basement membrane and is thus not the primary factor involved in altered nerve function associated with diabetic neuropathy.

Does increased cement pressure produce superior femoral component fixation?

A new system of femoral cement pressurization is presented that attempts to produce sustained and elevated cement pressure. Five paired cadaver femora were pressurized with Simplex cement, and PFC femoral components were inserted. One of each pair was pressurized with the new system, and 1 was pressurized with an existing device. Pressure was recorded at the proximal and distal levels. After curing, all 10 specimens were sectioned at similar levels and subjected to push-out testing to failure. Specimens that achieved higher pressures and longer duration of pressures tended to show higher levels of failure (not statistically proven on 10 specimens). Cementing techniques that use higher pressurization of cement are recommended. It is mandatory, however, that the femur be thoroughly cleaned of fat before the application of these techniques to avoid a fat embolism syndrome.

The locus of a novel gene responsible for arrhythmogenic right-ventricular dysplasia characterized by early onset and high penetrance maps to chromosome 10p12-p14.

Arrhythmogenic right-ventricular dysplasia (ARVD), a cardiomyopathy inherited as an autosomal-dominant disease, is characterized by fibro-fatty infiltration of the right-ventricular myocardium. Four loci for ARVD have been mapped in the Italian population, and recently the first locus was mapped in inhabitants of North America. None of the genes have been identified. We have now identified another North American family with early onset of ARVD and high penetrance. All of the children with the disease haplotype had pathological or clinical evidence of the disease at age <10 years. The family spans five generations, having 10 living and 2 dead affected individuals, with ARVD segregating as an autosomal-dominant disorder. Genetic linkage analysis excluded known loci, and a novel locus was identified on chromosome 10p12-p14. A peak two-point LOD score of 3.92 was obtained with marker D10S1664, at a recombination fraction of 0. Additional genotyping and haplotype analysis identified a shared region of 10.6 cM between marker D10S547 and D10S1653. Thus, a novel gene responsible for ARVD resides on the short arm of chromosome 10. This disease is intriguing, since it initiates exclusively in the right ventricle and exhibits pathological features of apoptosis. Chromosomal localization of the ARVD gene is the first step in identification of the genetic defect and the unraveling of the molecular basis responsible for the pathogenesis of the disease.

Macronutrient intake and whole body protein metabolism following resistance exercise.

UNLABELLED: The provision of carbohydrate (CHO) supplements following resistance exercise attenuated muscle protein (PRO) degradation (Roy et al. J. Appl. Physiol. 82:1882-1888, 1997). The addition of PRO may have a synergistic effect upon whole body protein balance by increasing synthesis (Biolo et al. Am. J. Physiol. 273:E122-E129, 1997). PURPOSE: To determine if the macronutrient composition of a postexercise beverage would alter muscle anabolism and/or catabolism following resistance exercise. METHODS: We provided isoenergetic CHO (1 g x kg(-1)) and CHO/PRO/FAT supplements and placebo (PL) immediately (t = 0 h) and 1 h (t = + 1 h) following resistance exercise (9 exercises/3 sets/80% 1 RM) to 10 young, healthy, resistance-trained males. Whole body leucine turnover was determined from L-[1-13C]leucine kinetics at approximately 4 h postexercise. RESULTS: No differences were observed for urinary 3-methylhistidine or urea nitrogen excretion between the trials. Leucine flux was significantly elevated at approximately 4 h postexercise for both CHO/PRO/FAT (177.59+/-12.68 micromol x kg(-1) x h(-1)) and CHO (156.18+/-7.77 micromol x kg(-1) x h(-1)) versus PL (126.32+/-10.51 micromol x kg(-1) x h(-1)) (P < 0.01). Whole body leucine oxidation was elevated at approximately 4 h for CHO/PRO/FAT (29.50+/-3.34 micromol x kg(-1) h(-1)) versus CHO (16.32+/-2.33 micromol x kg(-1) x h(-1)) (P < 0.01) and PL (21.29+/-2.54 micromol x kg(-1) x h(-1)) (P < 0.05). Nonoxidative leucine disposal (NOLD) was significantly elevated at approximately 4 h for both CHO/PRO/FAT (148.09+/-10.37 micromol x kg(-1) x h(-1)) and CHO (139.86+/-7.02 micromol x kg(-1) x h(-1)) versus PL (105.03+/-8.97 micromol x kg(-1) x h(-1)) (P < 0.01). CONCLUSIONS: These results suggest that consumption of either CHO or CHO/PRO/FAT immediately and 1 h following a resistance training bout increased NOLD as compared with a placebo.

Evaluation of treatment efficacy of Raynaud phenomenon by digital blood pressure response to cooling. Raynaud's Treatment Study Investigators.

Our previous studies have suggested that digital blood pressure response to cooling could provide a measure of the efficacy of treatments that are administered to patients with Raynaud phenomenon (RP). This method was used on 158 primary RP patients participating in a multicenter, randomized clinical trial that compared the efficacy of sustained-release nifedipine with temperature biofeedback in the treatment of RP. A pill placebo and electromyography served as controls. The response to local finger cooling was measured at 30 degrees, 20 degrees, 15 degrees and 10 degrees C in a temperature-controlled room under standardized conditions. The results showed that, at the 15 degrees C and 10 degrees C local cooling temperatures, the patients in the nifedipine group had a higher mean digital systolic blood pressure, a higher relative digital systolic blood pressure (RDSP), a smaller proportion of subjects with RDSP < 70% and a smaller proportion of subjects with a zero reopening pressure than the patients in the three other treatment groups. These results were statistically significant at 10 degrees C, the nifedipine group being significantly different from all others (p < 0.05); no significant difference was found between the three other treatment groups.

'Platonic jealousy': a conceptualization and review of the literature on non-romantic pathological jealousy.

Romantic jealousy has long been of interest to psychodynamically oriented clinicians. More recently empirical investigations have emerged into the causes and treatments of romantic jealousy. What has not kept pace with this interest is a wider research agenda into non-romantic forms of jealousy. While work has appeared in relation to specific groups or topics, such as sibling rivalry, no attempt has been made to draw together the material on non-romantic jealousy. In this paper the theoretical and empirical material on non-romantic jealousy is reviewed in order to answer two fundamental questions. Firstly, is non-romantic jealousy necessarily pathological? Secondly, to what extent is it helpful to subsume all forms of non-romantic jealousy under one term? It is suggested that while non-romantic jealousy is not always pathological, the term non-romantic jealousy may be a useful differentiating term, not least in highlighting an important area for future research.

A retrospective study of percutaneous endoscopic gastrostomy in ALS patients during the BDNF and CNTF trials.

Percutaneous endoscopic gastrostomy (PEG) provides a reliable route for nutrition and hydration in ALS patients with dysphagia. We performed a retrospective analysis of the CNTF and BDNF databases to determine the clinical status of ALS patients within 30 days preceding PEG insertion. This analysis revealed an approximately 50% reduction of function across multiple measures of ALS disease status. A trend to earlier intervention with PEG was apparent upon review of published studies and the CNTF and BDNF studies. By comparing the rate of decline pre- and post-PEG, nutritional supplementation via PEG stabilized the weight loss experienced by patients. Death within 30 days post-PEG was associated with a marked reduction in forced vital capacity (FVC) and identified a group of ALS patients in whom PEG should be cautiously performed. These data emphasize the importance of sequential measurement of FVC in managing ALS patients to guide the timing of nutritional intervention with PEG.

Desmin mutation responsible for idiopathic dilated cardiomyopathy.

BACKGROUND: Idiopathic dilated cardiomyopathy, of which approximately 20% of cases are familial (FDCM), is a primary myocardial disorder characterized by ventricular dilatation and impaired systolic function. It is a common cause of heart failure and the need for cardiac transplantation. Although 6 chromosomal loci responsible for autosomal dominant FDCM have been mapped by linkage analysis, none of these genes have been identified. By use of the candidate-gene approach, actin was identified recently as being responsible for dilated cardiomyopathy. Considerable evidence suggests desmin, a muscle-specific intermediate filament, plays a significant role in cardiac growth and development. METHODS AND RESULTS: To determine whether a defect of desmin induces dilated cardiomyopathy, 44 probands with FDCM underwent clinical evaluation and DNA analysis. Diagnostic criteria, detected by echocardiography, consisted of ventricular dimension of >/=2.7 cm/m(2) with an ejection fraction </=50% in the absence of other potential causes. After amplification by polymerase chain reaction, the exons of the desmin gene were sequenced. A missense desmin mutation, Ile451Met, which cosegregates with FDCM without clinically evident skeletal muscle abnormalities, was identified in a 4-generation family but was not detected in 460 unrelated healthy individuals. CONCLUSIONS: A novel missense mutation of desmin, Ile451Met, was identified as the genetic cause of idiopathic dilated cardiomyopathy. This finding is of particular significance because this is the first mutation detected in the desmin tail domain, and the function of the desmin tail remains unknown. Because this mutation leads to a restricted cardiac phenotype in the family studied in the present report, it suggests that the tail of desmin plays an important functional role in cardiac tissue.

Identification of sonic hedgehog as a candidate gene responsible for the polydactylous mouse mutant Sasquatch.

The mouse mutants of the hemimelia-luxate group (lx, lu, lst, Dh, Xt, and the more recently identified Hx, Xpl and Rim4; [1] [2] [3] [4] [5]) have in common preaxial polydactyly and longbone abnormalities. Associated with the duplication of digits are changes in the regulation of development of the anterior limb bud resulting in ectopic expression of signalling components such as Sonic hedgehog (Shh) and fibroblast growth factor-4 (Fgf4), but little is known about the molecular causes of this misregulation. We generated, by a transgene insertion event, a new member of this group of mutants, Sasquatch (Ssq), which disrupted aspects of both anteroposterior (AP) and dorsoventral (DV) patterning. The mutant displayed preaxial polydactyly in the hindlimbs of heterozygous embryos, and in both hindlimbs and forelimbs of homozygotes. The Shh, Fgf4, Fgf8, Hoxd12 and Hoxd13 genes were all ectopically expressed in the anterior region of affected limb buds. The insertion site was found to lie close to the Shh locus. Furthermore, expression from the transgene reporter has come under the control of a regulatory element that directs a pattern mirroring the endogenous expression pattern of Shh in limbs. In abnormal limbs, both Shh and the reporter were ectopically induced in the anterior region, whereas in normal limbs the reporter and Shh were restricted to the zone of polarising activity (ZPA). These data strongly suggest that Ssq is caused by direct interference with the cis regulation of the Shh gene.
