Gradenigo syndrome: a case-report.

We report a case of sixth nerve palsy as a rare complication of acute otitis media (apical petrositis). The clinical triad of acute otitis media, pain in the distribution of the fifth cranial nerve and sixth nerve palsy is known as Gradenigo syndrome.

Orbital involvement in sinus pathology: often without ocular pain.

Many and diverse sinus pathologies can present with an orbital involvement. Contrary to popular belief, uncomplicated rhinosinusitis causes rarely orbital pain or headaches. However, especially in children, acute ethmoidal rhinosinusitis can be complicated by an extension of the infection into the orbit. Also, isolated sphenoid sinusitis is often associated with significant ophthalmologic or neurological complications. Sinus surgery is another possible source of orbital complications associated with pain. The occurrence of orbital pain in these and other sinus pathologies is discussed, since early recognition and prompt therapeutic action are often necessary in order to avoid devastating sequellae.

[Acquired enophthalmos associated with hypoplasia of the maxillary sinus and asymptomatic chronic maxillary sinusitis].

BACKGROUND: We report a case of progressive enophthalmos, maxillary sinus hypoplasia and radiological signs of sinusitis in a young woman. CASE REPORT: The patient had no history of sinus surgery or facial trauma. There were no signs of sinusitis. Functional endoscopic sinus surgery was proposed to restore the communication between the nose and the antrum. DISCUSS: The particular aspects of asymptomatic maxillary sinus and causes of maxillary sinus hypoplasia are discussed. We suggest that obstruction of the osteomeatal complex causes negative pressure in the maxillary sinus provoking bone alterations. These alterations cause the maxillary sinus hypoplasia and the enophthalmos.

[Preventive cryotherapy of the retina and cataract surgery].

The prevention of aphakic retinal detachment by cryotherapy is controversial. No study up to now has compared results. We performed a retrospective study of two groups of patients: both groups consisted of patients with mature cataract and certain risk factors for retinal detachment (myopia, previous retinal detachment in the eye in study or in the fellow eye etc.) in the first group (n = 299) circumferential cryopexy has been performed prior to cataract extraction while in the second group (n = 149) no prophylaxis has been done. Comparison between the two groups did not reveal any protection afforded by prior cryotherapy: despite the prophylactic cryotherapy, 7 patients (2.3%) in the first group developed a retinal detachment post cataract extraction; two patients (1.3%) in the second group developed a retinal detachment; both were high-risk patients. These results lead us to conclude that prophylactic treatment does not necessarily prevent this complication. The best prophylaxis appears to be careful regularly performed examination of the peripheral retina both pre- and post-operatively.
