Gastrointestinal involvement in non-Hodgkin's lymphoma.

A total of 813 patients admitted to Roswell Park Memorial Institute from 1963--1972 with non Hodgkin's lymphoma (NHL) were reviewed for gastrointestinal (GI) involvement. Primary involvement was found in 71 and secondary involvement in 31 patients. Occult GI involvement was detected in 46% of the autopsy cases. The median survival time after the diagnosis of secondary GI involvement was nine months. The occurrence of primary GI-NHL was: 33 in the stomach, 18 in the small intestine, 14 in the ileocecal area including appendix, and 6 in the large intestine. Retrospective staging according to the Ann Arbor staging classification showed 24 to have presented as Stage I, 30 as Stage II, 4 as Stage III, and 13 as Stage IV. The primary diagnostic and therapeutic approach was operative, except in 2 patients with rectal lymphoma. Resection of the principally involved site was carried out in 42 patients. The remainder had palliative procedures or biopsy examinations only. Postoperative radiation therapy was given to 38 patients. Prognostically important features for primary GI-NHL were: stage; histologic type; site of the primary disease; and whether or not radiotherapy was administered. The age of the patient, size or degree of local extension, and type of operative procedure were prognostically of no importance. The results of this study would indicate that in Stage I and II primary GI-NHL, elective resection is not necessary prior to radiation therapy and that resection alone cannot be considered adequate treatment. A modified staging classification is proposed.

[Radiotherapy of primary, locally circumscribed non-Hodgkin's lymphoma of the stomach (author's transl)].

Different methods of treatment were compared in 24 patients with primary, locally circumscribed non-Hodgkin's lymphoma of the stomach. Prognosis was significantly better (P less than 0.01) for patients who had received radiotherapy as part of their primary treatment, than those who had received only surgical treatment. Two of ten patients whose tumour had been resected and who had received postoperative radiotherapy had a recurrence after 5 and 14 years, respectively, and four patients who had received radiotherapy alone for a non-resectable tumour are free of recurrence. On the other hand, a recurrence occurred in six of ten patients treated by surgery alone. The results indicate that radiotherapy is the most important method in the primary treatment of locally circumscribed gastric lymphoma.

Conversion of bacteriophage fd into an efficient single-stranded DNA vector system.

Single-stranded DNA vectors were constructed in vitro by insertion of various DNA fragments into the Intergenic Region of the single-stranded DNA phage fd. These inserts introduce into the phage genome unique cleavage sites for restriction nucleases which are suited for sticky joining in cloning experiments. Since these sites are usually located within genes coding for antibiotic resistance, inactivation of a resistance gene by insertion can be used as a marker for the successful cloning of a DNA fragment. Resistance genes also allow to select for recombinant DNA phages and to minimize the loss of DNA inserts which otherwise becomes significant above an insert size of about one kb. Cloning of several DNA fragments is described and strand separation of double-stranded DNA fragments by means of cloning into fd DNA is given as an example for application of single-stranded DNA vectors.

Bioinstrumentation for evaluation of workload in payload specialists: results of ASSESS II.

ASSESS II (Airborne Science/Spacelab Experiments System Simulation) was a cooperative NASA-ESA project which consisted of a detailed simulation of Spacelab operations using the NASA Ames Research Center CV-990 aircraft laboratory. The Medical Experiment reported on in this paper was part of the complex payload consisting of 11 different experiments. Its general purpose was to develop a technology, possibly flown on board of Spacelab, and enabling the assessment of workload through evaluating changes of circadian rhythmicity, sleep disturbances and episodical or cumulative stress. As parameters the following variables were measured: Rectal temperature, ECG, sleep-EEG and -EOG, the urinary excretion of hormones and electrolytes. The results revealed evidence that a Spacelab environment, as simulated in ASSESS II, will lead to internal dissociation of circadian rhythms, to sleep disturbances and to highly stressful working conditions. Altogether these effects will impose considerable workload upon Payload Specialists. It is suggested that an intensive pre-mission system simulation will reduce these impairments to a reasonable degree. The bioinstrumentation applied in this experiment proved to be a practical and reliable tool in assessing the objectives of the study.

Detection of type D retrovirus in a human amnion cell line (FL).

This paper describes the detection of viral particles in a human amnion cell line (FL). These particles belong to the group of type D retroviruses, because of their characteristic morphology, their major antigenic determinants, and the presence of particle-associated Mg++-preferring RNA-dependent DNA polymerase. In addition to budding and mature type D virus particles, intracytoplasmic type A structures have also been found. Immunological analysis provided evidence of cross-reactivity between the particles described and the Mason-Pfizer monkey virus as well as type D retroviruses from other human cell lines (PMF, HEp-2). Particles isolated from FL cell medium were shown to infect type D virus-susceptible human TU 197 cells.

[Results of hygienic analyses carried out in galleys, pantries and stock-rooms aboard sea-going vessels (author's transl)].

In the supply area of 120 German and foreign sea-going vessels scratch samples were taken from fixtures and examined bacteriologically. Only a third of the samples was free from E. coli in 1 g sample material. Pathogenic germs were found only rarely.--No distinct differences were detected between German and foreign ships.--Wooden working surfaces were compared with plastic surfaces and it was found that the E. coli count of the wooden surfaces was only half as high as that of the plastic surfaces.--The external impression of cleanliness of the objects examined furnished no clues as to the bacteriological findings.

Neurological complications and myelographic features of achondroplasia.

The neurological complications of achondroplasia are discussed based on the literature and two of our cases. Thoracolumbar kyphosis, hypertrophic osteoarthritis, and intervertebral disc herniations may cause early neurological disturbances to adult achondroplasts owing to the spinal stenosis. The symptoms range from nerve root compression syndromes to complete transverse myelopathy. The radiographic features are characteristic, but the value of myelography for etiological clarification and location of the pathological process is limited. The significance of early symptoms and signs must be stressed and early decompression performed, as results are very disappointing once extensive neurological damage has occurred. An extensive laminectomy is usually indicated.

[Drug-induced readjustment of therapy-resistant hypertension].

By a combined vasodilator-[diisopropylamine (disostat), dihydralazine (depressan)] furosemide- (furesis-) infusion therapy was tried to achieve a new stabilisation of the blood pressure of therapy-refractory patients with hypertension. Propranolol (Obsidan) was additionally administered for the suppression of a sympathetic counter-regulation. On 3 patients is demonstrated how by a ten days' intensive decrease of the blood pressure and decrease of the extracellular fluid volume the therapy resistance may be interrupted and a repeated response to an oral antihypertensive standard therapy may be achieved.

Transposition of a DNA sequence determining kanamycin resistance into the single-stranded genome of bacteriophage fd.

Derivatives of bacteriophages fd which transduce kanamycin resistance were selected after growth of the phage in an E. coli strain that carried transpoon 5 (Tn5). Different clones of transducing phage and their DNAs were characterized by gel electrophoresis, electron microscopy, and by their ability to multiply in the absence of helper phage. Integration of the intact transposon into the full size phage genome was correlated with an increase in size of the phage particle from 0.95 mu to 1.7 mu, and with the appearance in the phage DNA of the stem loop structure characteristic for single-stranded Tn5 DNA. In non-defective phages the site of insertion was mapped by heteroduplex analysis within the intergenic region of the phage genome. Defective transducing phages were characterized as an insertion of Tn5 into a phage gene, and/or as a partial deletion or duplication of phage and transposon DNA. The size of the transducing phage from different defective clones varied from 0.6 mu to 3.0 mu and was directly proportional to the DNA content. These results demonstrate that filamentous bacteriophage are highly capable to replicate and package very different amounts of foreign DNA.

Methodical investigation of the production of antibodies towards 3,4-dimethoxyphenylethylamine.

In this study the synthesis of various antigens from 3,4-dimethoxyphenylethylamine (3,4-DMPEA) is described. Antigen A was formed by coupling the acylated side chain of 3,4-DMPEA to the protein, while antigen B was synthesized by introducing an acylated amino group to the benzene ring and linking this reactive group to the protein. With antigen A an antiserum with a titer of 1 : 16000 could be raised, while with antigen B coupled to human albumin and bovine gamma-globulin the antiserum had a titer of 1 :1000 and 1 : 4000. Antibodies formed by immunization with antigen A exhibited a high specificity for substituents of the benzene ring, but were less specific for substituents of the side chain. Antibodies formed by treatment with the antigen B were specific for both kinds of substituents. So far, it has not been possible to harvest any specific antibodies towards the corresponding antigens of dopamine.

[Radial hemolysis in the diagnosis of influenza virus infections].

Some of the parameters of the radial haemolysis test were studied, and the test was used to determine antibody titres of sera obtained from patients involved in the epidemic outbreak of influenza-A/Port Chalmers, 1975. The sensitivity of the method was found to depend on the degree of erythrocyte sensitisation. An enlargement by 40 per cent of the haemolytic halo diameter should be considered and treated as antibody rise, if the reaction was to used in serological diagnosis under the conditions tested. The findings were in agreement with the complement fixation reaction in 48 per cent of all cases and with the haemagglutination inhibition test in 53.8 per cent.

[Demonstration of IgM antibodies in influenza virus infections].

Forty-Four sera of patients with confirmed influenza-A infection were titrated in parallel with and without 2-mercaptoethanic treatment against influenza-A by means of passive haemagglutination. Reduction of titre by at least two levels was detectable in 21 cases. Gradient centrifugation was additionally undertaken of 23 patient sera and three serum samples obtained from intact probands who had produced a titre against influenza-A in passive haemagglutination. Antibody against influenza-A was present in both the IgM and IgG fractions of the patients' serum samples. Yet, in the three control sera of intact probands antibody against influenza-A was recordable only from the IgG fraction. IgM antibody could not be detected by 2-mercaptoethanoic treatment alone unless the 7s-antibody portion in a given serum sample was smaller than the 19s-antibody portion.
