[Celioscopic salpingectomy. Apropos of 31 cases].

In this article we describe our experience with the first cases on which we have carried out laparoscopic salpingectomy. We have done this in 31 cases for, in 25 cases, tubal pregnancy and, in 6 cases hydrosalpinx. We describe the technique and results. The laparoscopic technique seemed to us to be especially indicated in cases of ectopic pregnancy when the decision to carry out salpingectomy was taken particularly when it was a second pregnancy in the same tube, or when the pregnancy occurred in a pathological tube or had already been operated on or where the tubal pregnancy had burst. It is also, whatever the reason for the salpingectomy, an alternative treatment of choice in the sterile woman who wants later to have IVF because it is less likely to cause parietal and pelvic adhesions.

[Prenatal diagnosis of limb and digital abnormalities. Evaluation of the activity of the Port Royal University Clinic from 1979 to 1983. Apropos of 30 cases].

Since 1979, thanks to high level ultrasonography systematic ultrasonic examinations between the 18th and the 22nd week of pregnancy have permitted to prediagnose limb and extremity abnormalities. Embryoscopy and foetoscopy can be used as additional techniques. This paper reports the activity results of the Port Royal Maternity hospital, between 1979 and 1983. In this hospital's Prenatal diagnosis Center, ultrasonographists, obstetricians, geneticists, surgeons, pediatricians and foetopathologists work together fruitfully. 16 limb abnormalities and 14 extremity abnormalities were detected there. We describe the circumstances of their detection, the diagnostic methods used, and the obstetrical attitude chosen.

[Microsurgical treatment of tuboperitoneal lesions. I. Results of distal tuboplasties: apropos of 135 cases].

135 patients were operated on using a microsurgical technique for distal plastic repairs between April 1978 and April 1983. Recurrences and cases with two lesions were excluded. The patients were classified in the group which corresponded to the tube that was less affected when the lesions were not symmetrical according to the I.F.F.S. classification. Out of 59 operations for fimbrioplasty, 31 had an on-going pregnancy, which is 52.5%, and 8 had an extra-uterine pregnancy, which is 13.5%. Of 76 cases that had neosalpingostomies, 28 had an on-going pregnancy, which was 36.8%, and 17 an extra-uterine pregnancy, which was 22.3%. When we looked at the results as a function of the initial lesion (the 9 cases of mixed lesions being excluded), 39 out of 83 cases of phimosis had an on-going pregnancy (47%) and 15 an extra-uterine pregnancy (18%). Out of 43 cases of hydrosalpinx 17 had an on-going pregnancy (39.5%) and 5 an extra-uterine pregnancy (41.6%). The levels of success obtained after operation on distal tubal lesions according to the criteria of definite selection are relatively satisfactory if one compares the present results with those obtained for in vitro fertilisation. It is therefore still correct to operate on these lesions. All the same, distal reparative surgery does give rise to a high risk of extrauterine pregnancy, which is nearly 20%. In our series the basically more significant rate at p less than 0.05 was shown after making neosalpingostomy in cases of phimosis. The consequences of this complication can now be limited by the progress that has been made in laparoscopic surgery.

[Microsurgical treatment of tuboperitoneal lesions. II. Results of the release of adhesions: apropos of 46 cases].

Between April 1978 and April 1983 46 patients were operated on for division of adhesions of a first and second degree according to the Madrid classification. They were operated on by laparotomy. 36 patients, which was 78.3%, had an on-going pregnancy and 3 had an extra-uterine pregnancy, which was 6.5%. We give three reasons to explain these results: antibiotic and anti-inflammatory treatment before the operation. This is important because of the high rate of infection and in particular chlamydia trachomatis. This is on-going in these patients. A technique for division of adhesions that was carried out according to a precise and stereotyped protocol used by all three operators. a follow-up of 43 out of these 46 patients that allowed them to be treated in the months following surgery to deal with other factors contributing to lowered fertility.

[Significance of the diamine-oxidase test in the diagnosis of spontaneous rupture of fetal membranes].

The authors report on their experience collected over a year of the use of the diamine-oxydase activity test after having collected the vaginal secretions on strips of Whatman paper placed in the vagina when they were trying to recognise premature rupture of the membranes. They studied: a control series of 90 patients (30 where the rupture was certain, 60 where it was definite that there was no rupture). They point out how reliable the method is at all stages of pregnancy and how simple and easy it is to carry out; a series of 89 consecutive patients whose clinical picture suggested premature rupture of the membranes; the diagnosis was shown to be wrong 48 times and confirmed 41 times (either immediately in 38 tests using DAO, or 48 hours after carrying out the first test in 3 cases). If this test is used to confirm that the membranes have really ruptured and to say when they have ruptured diamine-oxydase can be a very valuable factor when it is associated with the clinical picture and the ultrasound picture in leading to a rational handling of the case.

[Biopsy of chorionic villi by forceps under ultrasonic control: technic and results].

We describe an original technique for villi sampling using a rigid forceps under ultrasound control. 120 chorion biopsies were performed, the success rate of sampling is 100%, the samples are pure without any maternal contamination in 90% of cases. The fetal loss rate needs further evaluation because the number of continuing pregnancies is still too small to allow enough accurate assessment. No miscarriage was observed among our last 70 patients. At the beginning of our experience we preferred to start our diagnoses only for high genetic risk diseases: haemoglobinopathies, hereditary metabolic diseases, lethal X-linked diseases. We insist on the efficiency and the quickness of this method and on the quality of the samples which makes the diagnosis safer.

[Prevention of Chlamydia trachomatis infections in women].

Over the past several years, the incidence of chronic salpingitis has increased relentlessly, with its accompanying pelvic pain, ectopic pregnancies, and tubal sterility. Chlamydia are a frequent etiologic agent. Preventing these infections is difficult, nevertheless proper care will be given by recalling four key words: avoidance, diagnosis, education, and treatment. It is important to discourage early sexual activity and multiple sexual partners, to encourage the use of condoms, and avoid the use of an intrauterine device in adolescents. Atypical presentations of salpingitis must be recognized and treated effectively, thus preventing the spread of infection. Education on venereal diseases should be presented in secondary schools, during military service, and other similar groups, but especially during physician visits for contraception.

[Racket-shaped tubal-uterine anastomosis. Technic and results].

Proximal tubal lesions that are discovered when a case of sterility is being worked out are polymorphic in nature and they are variable in extent. Fortunately it is rare that a proximal tubal lesion should involve the whole interstitial portion and the uterine os of the tube. This is why many surgeons have taken up the method of tubo-uterine implantation advocated by Palmer. Salat-Baroux and Cornier published a paper on microsurgical tubo-interine implantation in 1979. We since 1978 have been carrying out a technique of tubo-uterine anastomosis in raquette shape (mostly isthmo-uterine). This is described in detail in this article. The first results on 32 anastomose carried out in 23 patients who were operated on of whom 17 cases had pure cornual lesions and 6 cases bipolar lesions seem to be encouraging with a patency rate 6 months later of 69% and a rate of intra-uterine pregnancy of 23.2% (0.10-0.41) at 12-18 months and 31.1% (0.14-0.56) at 18-24 months (using an actuarial method). Up to now we have not had an extra-uterine pregnancy near the site of suture. Our technique seems to us to give a worthwhile alternative microsurgical method to implantation in interstitial lesions that are widespread and deep.

[Should one operate on bifocal tubal lesions in 1984? Apropos of 54 cases].

We have analysed 54 cases where the patients had two lesions in the tubes and were treated with microsurgery. The results of histological examinations show that chronic salpingitis in the major lesion. Infection was the aetiological cause in 76% of the cases. The results of microsurgical treatment are poor with a low rate of intra-uterine pregnancy: [19.4% (0.08-0.42) at 24-30 months for pregnancies that were continuing and for abortions, 11.7% (0.04-0.33) at 24-30 months using actuarial techniques for estimating pregnancies that were continuing]. There was a high risk of extra-uterine pregnancy [35% (0.18-0.58) at 24-30 months]. These results make us wonder whether microsurgery or in vitro fertilisation should be the treatment of preference in these cases where the tubes are affected in two areas.

[A new prostaglandin E1 analog in late interruption of pregnancy, in utero deaths and very premature ruptures of the membranes. Apropos of 117 cases].

The authors have used suppositories containing 1 mg of an analogue of PGE1 in 117 cases. The indication was fetal abnormality or fetal disease in 85 cases, fetal death in utero in 24 cases and very premature rupture of the membranes in 8 cases. The pessaries have a number of advantages over the techniques that were employed previously; intra-amniotic injection and intra-cervical perfusion of PGF2. The method is very efficacious: 98.3% successes. There was no failure in the series of intra-uterine fetal deaths. Placing the pessaries in the posterior vaginal fornix is easy and the technique does not consist of any manipulation. Not having to put any laminary tents in and no early rupture of the membranes has made it possible to avoid infection except in 1.7% of cases. The incidence of heavy bleeding was exceptionally low: 0.8%. As far as the dangers of rupture of the uterus or of tears in the cervix, this was minimal. Picking cases for correct indications, using doses adapted to each patient and careful observation by a team who are used to dealing with terminations of pregnancy and their techniques should lower the risk. The time spent in hospital is lessened: on an average 3 days. The fetus that is expelled is never macerated, which is important in order to be able to find out all the facts necessary to give good genetic counselling to a couple who have been profoundly traumatised.(ABSTRACT TRUNCATED AT 250 WORDS)

Prenatal diagnosis of sickle-cell anemia in the first trimester of pregnancy.

To investigate the usefulness of chorionic biopsy for prenatal diagnosis of sickle-cell anemia by restriction-endonuclease analysis of fetal DNA, we studied 30 pregnancies before elective abortion. When the reproducibility of the technique for obtaining adequate DNA samples was established, we successfully applied the test to five pregnancies at risk for sickle-cell anemia. In two cases, sickle-cell disease of the fetus led to a decision to terminate the pregnancy. In three other cases, a normal or AS genotype was demonstrated. One normal infant has been born, and one other pregnancy is continuing normally. In one case in which fetal death was observed three weeks after sampling, placental abnormalities found on histologic examination were compatible with a chromosomal aberration. Our study shows that chorionic biopsy is feasible for the prenatal diagnosis of sickle-cell disease before the 10th gestational week. If subsequent experience demonstrates this technique to be safe enough for mother and fetus, the ability to test in early pregnancy may make prenatal diagnosis acceptable to more couples at risk for serious genetic disorders.

[Determination of the acetylcholinesterase activities of the amniotic fluid on a centrifugal analyzer].

Amniotic fluid cholinesterase (ChE) activity measurements are realized to detect pre-natal neural tube defects. Their determinations on centrifugal analyzer are described. Total ChE activity is assayed at 412 nm with dithiobis-(nitrobenzoic-acid). Acetylcholinesterase activity is measured in the presence of ChE inhibitor, 14 X 10(-6)M lysivane; our results about the inhibitor concentration confirm the previous studies of Dale (Clin. Chim. Acta, 1977, 77, 407-413, Lancet, 1980, ii, 975) in opposition to those of Brock (Lancet, 1981, i, 95). Non NTD amniotic fluids from 92 women between the 17th and 20nd weeks of pregnancy were used to establish normal values. The technique proposed is rapid, accurate and needs only 40 microliters sample.

[Preconceptional selection of sex using the ionic method. Dietary regime. Results of a 2 years' prospective clinical study].

45 women out of a total of 58 patients who followed a dietary regime appropriate to the sex they had chosen for their babies were successful in delivering a baby of the wanted sex. This result gives a success figure of 77.6% (the mean between 67 and 89%: p less than 0.001) and from the statistical point of view this difference is significant as compared with the natural sex ratio. The diets were overall well tolerated and the children at birth weighed normal weights. The parents seemed to request approximately equal numbers of girls as of boys--a balanced request. A high number of patients abandoned the method (75%). This is explained by the restrictions that are imposed by the diets and because the method would still appear to be experimental. It does seem that the ability to influence the sex ratio of the future offspring is possible when the ionic balance of the diet is changed. The study should be continued.
