The toxicity of the thermal decomposition products of polyvinyl chloride (PVC) plastic were studied and their pulmonary effects in guinea pigs evaluated. Measures of toxicity included lethality, alteration in lung mechanical function, lung mixed function oxidase enzyme activity and lung organ to body weight ratio. A technical PVC formulation (composition not known) was more lethal than another formulation designed to simulate electrical insulation. Both materials and pure PVC produced highly irritant gases as measured by changes in respiratory rate and compliance (decreased) and total flow resistance (increased). While adverse pulmonary mechanical effects in lung function were observed, lung biochemical parameters were relatively unchanged.
The authors present a case of cholestatic jaundice which developed during total parenteral nutrition. The histology of the liver mimicked the picture of large bile duct obstruction. However, there was no duct obstruction and the symptoms regressed after the caloric input was reduced. At postmortem examination, the portal inflammatory infiltrate had cleared and was replaced by fibrosis. Alterations in bile chemistry could be responsible for the similarity between the lesions induced by parenteral feeding and those classically described in large bile duct obstruction.

Report of a case of aggressive musculo-aponeurotic fibromatosis in a 14-year-old girl with transformation into fibrosarcoma, 10 years later. The tumoral mass initiated its growth in the nuchal region but finally invaded both the cervical and the right retroscapular region. Evidence for muscular, osseous and neurological invasion and destruction was found. The tumor could be controlled neither by surgery, nor by radiotherapy and chemotherapy. This case report is studied in the light of cases reported in the literature.

Twenty-two cases of lymphoid tumours of the stomach were reviewed by application of the Kiel classification for non-Hodgkin's malignant lymphomas. Four cases of pseudolymphomas were found, one of which had been previously misdiagnosed as malignant lymphoma. The remaining cases were all malignant tumours with B-cell lymphoma features. These were divided into seven low grade lymphomas (three immunocytomas and four centroblastic/centrocytic) and II high grade lymphomas (six centroblastic and five immunoblastic lymphomas). No cases of Hodgkin's disease or lymphoblastic lymphoma were observed. The Kiel nomenclature was not only easy to apply, but also helped to differentiate pseudolymphoma from malignant lymphoma. Both the pseudolymphomas and the malignant lymphomas were consistently associated with follicular gastritis. This lesion, while intrinsically non specific, was sometimes accompanied by suggestions of transition between itself and the lymphoma, a fact which at least raises the possibility of a transformation of the former into the latter.
The paper reports a re-evaluation--based on the Kiel classification for non-Hodgkin lymphomas--of a group of cases initially diagnosed as undifferentiated small cell carcinomas or primary lymphomas of the thyroid. Twelve such cases were found among the 155 cases of primary malignant tumours of the thyroid recorded at the Institut Jules Bordet between 1955 and 1975. The review of the clinical charts and the histology showed that all the cases were in fact malignant lymphomas fitting easily into one of the groups described in the Kiel classification. These findings support the growing opinion that undifferentiated small cell carcinoma of the thyroid does not exist as a distinctive clinicopathological entity. Furthermore, the Kiel classification proved to be an excellent prognostic indicator, since all the cases classified as highly malignant were indeed fatal, whereas the surviving cases--three of which had shown tumoral extension beyond the thyroid capsule--fell into the group of low malignancy. Lastly, this study acknowledges the frequently observed association of malignant lymphoma of the thyroid with stigmata of Hashimoto's disease, and thus supports the concept that the continuous antigenic stimulation observed in the latter could trigger the development of a malignant lymphoma.
A woman with a T cell lymphoproliferative malignacy and heterozhgosity at the X chromosome-linked locus for glucose-6-phosphate dehydrogenase (G-6PD) isoenzymes was studied to find the clonal origin of her circulating neoplastic T cells. The red blood cells, polymorphonuclear cells, whole mononuclear cells, and T cell-depleted mononuclear cells contained both A and B isoenzymes of G-6-PD. In contrast, the tumor cells, separated by using their capacity to form rosettes with sheep red blood cells, contained only the B isoenzyme of G-6-PD. This observation strongly suggests the monoclonality of this T cell malignancy.
The authors present the historical evolution of the concepts of non-Hodgkin malignant lymphomas which lead to the new functional classifications of Lukes and of Kiel. These classifications are based on the discoveries of modern immunology and are relying on the marking techniques for the identification of the T, B and U lymphocytes. Arguments are presented why these modern nomenclatures should progressively replace the Rappaport classification which preceded the major developments of modern immunology. The Kiel nomenclature is briefly described.
Clinical, radiological, surgical, and histological features are reported for a patient with a temporal lobe oligodendroglioma that had developed in infancy and was diagnosed and partially resected at age 10. The patient is presently 24 years old and still well. To the best of our knowledge this is the first report of an oligodendroglioma with probable onset at 5 months of life and satisfactory course over 24 years.
This study reports the incidence of HBsAg in a series of chronic lymphoproliferative disorders in which a high incidence of cirrhoses had been previously observed. Twenty-three cases were collected from the necropsy records. The type of lymphoma was reviewed in the light of the new functional classifications of non-Hodgkin malignant lymphomas introduced by Lennert and Lukes. The presence of HBsAg in the liver was investigated by the indirect immunofluorescence technique. Eleven cases showed plasmocytoid features and were considered as immunocytomas. Seven cases showed cirrhosis of the liver; six of them belonged to the immunocytoma group. Four cases were positive for HBsAg. Three of them were found among the group combining cirrhosis and immunocytoma. They presented the abundant nodular distribution of HBsAg typical of inactive cirrhosis.A parallel is drawn between the often reported association of Waldenström's syndrome and cirrhosis and the association of immunocytoma and cirrhosis observed in this study. The analogy is all the more justified since most of the lymphomas associated with Waldenström's syndrome happen to be immunocytomas. Therefore the association between HB virus infection and cirrhosis on the one hand and chronic lymphoproliferative disorders on the other may not be purely coincidental. A chronic antigenic stimulus such as persisting HBsAg could trigger the proliferation of a malignant lymphoid clone.
Non-suppressible insulin-like activity (NSILA-S) was determined in 5 dogs before and after pancreatectomy and again during insulin therapy. All NSILA-S determinations were carried out on serum samples which were passed over Sephadex G-50 columns equilibrated with 1 M acetic acid. The levels of NSILA-S decreased drastically shortly after pancreatectomy and rose slowly after institution of insulin therapy, to normal levels. During the period of severe diabetes after pancreatectomy the concentration of growth hormone was elevated. These findings indicate that 1) the pancreas cannot be the site of synthesis and release of NSILA-S, 2) NSILA-S levels do not always parallel growth hormone levels and 3) the synthesis and secretion of NSILA-S among other factors is under the control of insulin.

Non-suppressible insulin-like activity (NSILA) is a term used for a variety of substances in serum, excluding insulin, which promote glucose uptake of adipose tissue and diaphragm in vitro. NSILA-S is a peptide with a molecular weight of 7000 which is soluble in acid ethanol and which has been purified on a large scale from human serum. This study describes a simple chromatographic one step procedure by which NSILA-S can be extracted and quantitatively measured in individual sera. Using Sephadex G-75 equilibrated with 1 M acetic acid, NSILA-S was detected only in one peak containing small molecular peptides. NSILA-S obtained with this one step chromatographic procedure exerted all the effects of purified NSILA-S including sulphation activity on the rat cartilage. All chromatographic fractions with NSILA-S also had sulphation activity. Both, NSILA-S and sulphation activity were increased in acromegalics and decreased in pituitary dwarfs. It is suggested that one molecule in serum is responsible for both activities which are, at least in part, under the control of growth hormone.
