Autoradiographic evidence for the impermeability of mouse peritoneal macrophages to tritiated streptomycin.

Cultured mouse peritoneal macrophages were found to be relatively impermeable to streptomycin. Based on radioactivity measurements and radioautographic evidence, macrophages were impermeable to tritiated dihydrostreptomycin for periods up to 20 hr of incubation. Little or no intracellular streptomycin could be detected even when incubation was carried out in the presence of therapeutic blood levels of carrier dihydrostreptomycin. When the cultured mouse macrophages were allowed to phagocytize staphylococci, yeast cells, or polystyrene latex particles in the presence of tritiated streptomycin, the impermeability of the cells to the antibiotic was not affected. These observations suggested that the process of phagocytosis does not facilitate the intracellular accumulation of streptomycin, as seems to be the case for the fixed phagocytic cells of the liver.

Cost-effectiveness of improved treatment services for sexually transmitted diseases in preventing HIV-1 infection in Mwanza Region, Tanzania.

BACKGROUND: A community-randomised trial was undertaken to assess the impact, cost, and cost-effectiveness of averting HIV-1 infection through improved management of sexually transmitted diseases (STDs) by primary-health-care workers in Mwanza Region, Tanzania. METHODS: The impact of improved treatment services for STDs on HIV-1 incidence was assessed by comparison of six intervention communities with six matched communities. We followed up a random cohort of 12,537 adults aged 15-54 years for 2 years to record incidence of HIV-1 infection. The total and incremental costs of the intervention were estimated (ingredients approach) and used to calculate the total cost per case treated, the incremental cost per HIV-1 infection averted, and the incremental cost per disability-adjusted life-year (DALY) saved. FINDINGS: During 2 years of follow-up, 11,632 cases of STDs were treated in the intervention health units. The baseline prevalence of HIV-1 infection was 4%. The incidence of HIV-1 infection during the 2 years was 1.16% in the intervention communities and 1.86% in the comparison communities. An estimated 252 HIV-1 infections were averted each year. The total annual cost of the intervention was US$59,060 (1993 prices), equivalent to $0.39 per head of population served. The cost for STD case treated was $10.15, of which the drug cost was $2.11. The incremental annual cost of the intervention was $54,839, equivalent to $217.62 per HIV-1 infection averted and $10.33 per DALY saved (based on Tanzanian life expectancy) or $9.45 per DALY saved (based on the assumptions of the World Development Report). In a sensitivity analysis of factors influencing cost-effectiveness, cost per DALY saved ranged from $2.51 to $47.86. INTERPRETATION: Improved management of STDs in rural health units reduced the incidence of HIV-1 infection in the general population by about 40%. The estimated cost-effectiveness of this intervention ($10 per DALY) compares favourably with that of, for example, childhood immunisation programmes ($12-17 per DALY). Cost-effectiveness should be further improved when the intervention is applied on a larger scale. Resources should be made available for this highly cost-effective HIV control strategy.

The first international intercomparison of EPR-dosimetry with teeth: first results.

Intercomparison of EPR-dosimetric techniques using tooth enamel had been performed in order to check whether the results produced by different laboratories are consistent and accurate. Participants were supposed to evaluate doses applied to pulverized enamel samples, using routine techniques from their laboratories. The intercomparison has demonstrated a great variety of methods used for dose reconstruction. Peculiarities of experimental approaches are discussed systematically in terms of procedure for recording the EPR-spectra, determination of the amplitude of the radiation induced signal, determination of the dose, and error propagation.

Association of metabolic gene polymorphisms with alcohol consumption in controls.

The objectives were to study the association between metabolic genes involved in alcohol metabolism (CYP2E1 RsaI, CYP2E1 DraI, ADH1C, NQO1) and alcohol consumption in a large sample of healthy controls. Healthy subjects were selected from the International Collaborative Study on Genetic Susceptibility to Environmental Carcinogens (GSEC). Subjects with information on both alcohol consumption and at least one of the studied polymorphisms were included in the analysis (n=2224). Information on the amount of alcohol consumption was available for a subset of subjects (n=844). None of the studied genes was significantly associated with drinking habits. A significant heterogeneity with age was observed when studying the association between CYP2E1 RsaI and alcohol drinking. CYP2E1 RsaI polymorphism was significantly associated with being a never drinker at older ages (odds ratio [OR] 2.4, 95% confidence interval [CI] 1.2-4.8; at ages above 68 years), while the association was reversed at ages below 47 years (OR 0.5, 95% CI 0.2-1.4). For subjects with detailed information on alcohol intake, no association between alcohol quantity and polymorphisms in metabolic genes was observed; subjects carrying the NQO1 polymorphism tended to drink more than subjects carrying the wild-type alleles. Therefore, no significant association between CYP2E1 RsaI, CYP2E1 DraI, ADH1C, NQO1 polymorphisms and alcohol consumption was observed in healthy controls.

Large infiltrating renal transitional cell carcinomas: CT and ultrasound features.

A series of nine patients with large infiltrating renal transitional cell carcinomas is presented. These large tumors are rare but important in the differential diagnosis of renal masses. The average tumor size was 8.5 cm. In six patients the tumor involved almost the entire kidney. Seven masses were centrally located but two were peripheral. With CT the masses exhibited low attenuation relative to the normal or opposite renal parenchyma. Four masses exhibited necrosis. Lymphadenopathy, venous invasion, calcification, and spontaneous hemorrhage were all seen in this series. The major differential diagnostic possibility was renal cell carcinoma but lymphoma and metastatic disease were included. The diagnosis was confirmed in six patients by fine needle aspiration biopsy because of the presence of metastatic disease or the possibility of lymphoma. In the remaining three patients the diagnosis was made at surgery. The recognition that a large infiltrating renal mass may represent a transitional cell neoplasm is important in the eventual surgical management, and accurate preoperative diagnosis is stressed.

Heart failure practice improvement effort in the outpatient setting: a health care financing administration initiative to improve the care of medicare beneficiaries with heart failure.

This column is the fourth in a series describing Health Care Financing Administration initiatives to improve care for Medicare beneficiaries with heart failure. The first three papers addressed the background, design, and baseline results of the Health Care Financing Administration national initiative to improve quality of inpatient care for heart failure through the activities of each state's Health Care Financing Administration contractor Peer Review Organization. This paper describes a smaller-scale but equally important endeavor: the Heart Failure Practice Improvement Effort, a pilot project to test the feasibility of assessing and improving heart failure care in the outpatient setting. Pain assessment in the elderly.

A major increase in the elderly population presumes a proportionate increase in chronic brain syndrome sufferers, who may have chronic pain. This article discusses the difficulties in assessing pain in this care group, with specific reference to those who are confused and non-verbal. Areas for further research are highlighted.

Sequelae of biochemical events following photochemical injury of rat sensory-motor cortex: mechanism of ganglioside protection.

In culture the protracted and abusive stimulation of glutamate (GLU) receptors results in neuronal death through a mechanism involving the persistent translocation of PKC and the destabilization of (Ca2+)i homeostasis [(Ca2+)i HD]. In contrast, intermittent GLU receptor use elicits a coordinated expression of immediate early genes (IEG) acting as nuclear third messenger. Brain ischemia also is known to result in the paroxysmal abusive stimulation of glutamate receptors. The glutamate receptive elements in turn degenerate largely as a function of their inability to control homeostatic Ca2+ due to the irreversible translocation of PKC. In the present study we employed an in vivo model of focal brain ischemia using the photosensitive dye, Rose bengal. With this model we sought to determine the neuroprotective actions of MK-801, a noncompetitive blocker of GLU at the NMDA-sensitive receptor and of the semisynthetic gangliosides LIGA 4 and LIGA 20 which in vitro have been demonstrated to block PKC translocation. Moreover, we sought to establish whether the persistent stimulation of ionotropic glutamate receptors would led to a change in ionotropic glutamate expression in the focal and perifocal area. Importantly, the perifocal area (i. e., the region surrounding the area of primary insult) is a region in which profound cellular reorganization occurs including neuronal death and glial proliferation and is a key region to target various neuroprotective drugs aimed at ameliorating the neurodegeneration following stroke. Receptor abuse dependent antagonists (RADA) drugs such as gangliosides selectively curtail the amplification steps that specifically differentiate signal transduction following physiological receptor use from that following pathological receptor abuse.(ABSTRACT TRUNCATED AT 250 WORDS)

Nursing student information network: fostering collegial communications using a computer conference.

This article describes the development and implementation of a student-based computer information network in a university setting. A computer conference was established as a pilot project on two campuses in a large multipurpose, multicampus university school of nursing, and it was used to communicate information and facilitate collegial interaction among students, faculty, academic counselors, and administrators. The project demonstrates that computer conferences can be used to disseminate information effectively to diverse and geographically distant student populations. Such networks have the potential to enhance opportunities for collaborative learning experiences and student-faculty communication.

Secretion of bioactive interleukin 1 by rat testicular macrophages in vitro.

The relative capacities for testicular macrophages and resident peritoneal macrophages to secrete the pro-inflammatory cytokine, interleukin 1 (LL-1), in response to stimulation by bacterial lipopolysaccharide (LPS), were compared in vitro. Macrophages were isolated from adult male rat testicular interstitial cells or peritoneal lavage by adherence to glass coverslips or plastic culture dishes. The macrophages were immediately cultured, with or without a maximal dose of LPS (1 mu g/ml), over 24 hours at 32 degrees Celsius. Bioactive LL-1 production was measured by a sensitive thymocyte proliferation bioassay, employing recombinant human LL-1 beta as the reference standard. In comparison with the peritoneal macrophages, testicular macrophages displayed only a very small response to LPS, producing 2.8% of the amount of LL-1 per cell secreted by peritoneal macrophages cultured under identical conditions. Production of authentic LL-1 was confirmed by inhibition of the bioassay response in the presence of human recombinant LL-1 receptor antagonist. A small molecular mass (<10 kDa based on ultrafiltration) inhibitor of LL-1 bioactivity was also present in the medium collected from both cultures, but this inhibitory activity did not account for the differences in activity observed. In cultures of total peritoneal cells under similar conditions, addition of testosterone (10-1,000 ng/ml) did not affect LL-1 production in response to LPS. These data indicate that testicular macrophages have a reduced ability to secrete bioactive IL-1, and they provide further evidence for an altered capacity for immune responses within the testis.
