Pharmacotherapy practice in a rehabilitation hospital.

A description of a pharmacotherapy practice in a rehabilitation hospital is presented. Patient pharmacotherapy assessments were classified using the pharmaceutical care model as a typology. Pharmacotherapy log-sheets for a 2-year period were reviewed and categorized. The most frequent type of observed medication related problem encountered in the practice was "drug induced medical problem," followed by "duration or dose of medication too great" and "indication without drug therapy." The most serious type was "duration or dose of medication too great." "Drug-food interactions" were not examined during the course of the practice. The particular medications found to be frequently associated with medical problems in the rehabilitation population were those affecting 1) bowel and bladder training, 2) cognitive retraining, and 3) gait and strengthening. Those in the "duration or dose of medication too great" category included ciprofloxacin, histamine-2-antagonists, phenytoin, and benzodiazepines. The report provides additional evidence of the practical use of the theoretical model of pharmaceutical care in the approach to pharmacotherapeutic management of patients.

Ohio hospital formularies: an analysis of structure, process, and outcome.

A cross-sectional survey of Ohio hospital formulary systems conducted in the fall of 1982 is presented. The study describes the current status of formulary systems and formularies and analyzes the relationships between structure, process and outcome variables related to formularies and 21 randomly selected Ohio Department of Public Welfare (ODPW) Drug Efficiency Study and Implementation (DESI)-listed drugs. Fifty-five per cent of the state's hospitals reported having a formulary. Hospitals with formularies appeared to have a lower ratio and number of ODPW-DESI agents in stock. However, two variables maximally discriminated the avoidance of ODPW-DESI drugs: 1) the number of drugs in inventory or on formulary, and 2) the existence of a formulary. The results suggest that having a formulary assists in the avoidance of ineffective drug products, but the study's process variables could not predict formulary existence or positive outcomes.

Cancer chemotherapy and contraceptive counseling.

Contraceptive counseling for patients receiving antineoplastic agents is illustrated by the case of a patient with Hodgkin's disease. After the patient began treatment, he married his girlfriend and desired to have children in the near future. Clinicians are alerted to the sexual needs of the cancer patient and the need for contraception assessment in patients of reproductive years. An outline for informing cancer patients about contraception is provided.

Antineoplastic drug-use process at a rural hospital.

This article describes the antineoplastic drug-use process and monitoring activities performed by the Department of Pharmacy at a 225-bed rural hospital. Documents developed and used in the process assist the pharmacist in assessing drug-related toxicity, in making recommendations for laboratory or system studies, and for organizing a therapeutic rationale for inpatients and out-patients. Involvement of pharmacists in oncology in rural or small institutions can be essential for maximal therapeutic monitoring and quality patient care. The importance of monitoring antineoplastics on a per patient basis is highlighted.

Behavioral management concepts with application for home parenteral nutrition patients.

The concept of symptom-avoidance behavior is discussed, with particular reference to home intravenous hyperalimentation (HIVH) patients. Symptom-avoidance behavior is explained as a combination of a health behavior model and an expectancy motivational theory leading to a basic outcome, which is satisfactorily avoiding the onset of symptoms. A systematic behavioral assessment method is presented, based on anecdotal cases; it is experimental in nature and can serve as a guide for practitioners who wish to initiate HIVH.
