Correlations between intraocular pressure and resulting scleral indentation after detachment surgery.

The correlation of IOP, cerclage diameter and cerclage buckle height was examined experimentally for different silicone band profiles. The smallest cerclage diameter was found at the lowest IOP whereas a maximal buckle height was reached with an IOP between 10 and 15 mm Hg. For clinical application a maximal vitreous traction relief is expected with minimal IOP. The maximal buckle height in order to reach an optimal drainage barrier occurs with an IOP between 10 and 15 mm Hg.

[Some correlations between ultrasonographic and histopathologic findings in malignant melanoma of the choroid (author's transl)].

The ultrasonographic findings in 39 cases of malignant melanoma of the choroid has been correlated with histopathological results. Tumors with hemorrhages and/or necrosis showed a lower reflectivity than those with a higher amount of connection tissue. B-scan ultrasonography was found to be reliable for the measurement of the extent of the tumor in all three dimensions.

[First experiences with serial computed tomography in choroidal melanomas (author's transl)].

22 patients with extending intraocular lesions are examined by serial computed tomography after a bolus injection of contrast medium. The changes of X-ray density of the tumor areas are compared with those of the vitreum and of brain tissue. Choroidal melanomas exceeding 2 mm in prominence are identified as highly vascularized areas in which contrast enhancement according to certain brain tumors takes place. A differentiation from large pseudotumors seems to be possible. The value of this method for the differential diagnosis of choroidal melanomas is discussed.

[A model to measure the volume of choroidal melanomas (author's transl)].

A formula to calculate the volume of biconvex-shaped malignant melanomata of the choroid is presented. The axial length of the eye, whatever the refractive error, has nearly no influence on the results. A computer printout of the range of results is added for clinical use. The reliability of contact B-Scan ultrasonography to obtain those parameters is examined using various tumor models of defined sizes.

Efficiency of pupillary stretching in cataract surgery.

PURPOSE: To assess the efficiency of pupillary stretching with iris hooks in poorly dilatable pupils during phacoemulsification. METHODS: Forty eyes with maximal preoperative mydriasis less than 4.5 mm (mean 3.9 mm S.D. 1.0) received bimanual pupillary stretching according to the technique of Miller and Keener (1994). RESULTS: Small pupils were mainly caused by long-term pilocarpine therapy (n = 21), posterior synechia (n = 11) or diabetic iridopathy (n = 7). With multidirectional stretching, the pupil could be enlarged to mean 6.6 mm (S.D. 0.66), lasting throughout phacoemulsification. The effect of stretching was significantly greater in eyes with synechias, but no significant differences in post-operative pupil motility was observed between the three subgroups. Six eyes with pronounced postoperative inflammation responded well to antiinflammatory treatment. CONCLUSIONS: Done with routine surgical instruments, bimanual pupillary stretching seems to be an easy, safe and effective procedure for enlarging small pupils by an average of 2.7 mm during phacoemulsification, especially in eyes with posterior synechias.
