Femoral shortening in total arthroplasty for completely dislocated hips: 3-7 year results in 25 cases.

During the years 1988-1991, we performed 25 total hip replacements for completely dislocated hips in 15 women and 4 men with a median age of 54 (17-67) years. In all cases, femoral shortening at the subtrochanteric level was performed to obtain reduction of the hip. The patients have been followed for 3-7 years. 1 patient experienced sciatic nerve palsy, 1 a delayed union and 1 a malunion at the osteotomy site. There were no signs of mechanical failure. All patients were satisfied. According to the Charnley hip score, function was excellent in 15 cases, good in 9 and fair in 1. The median Harris hip score improved from 43 at the time of operation to 93 at follow-up. 7 hips had a positive and 18 a negative Trendelenburg test. Before operation, all patients had a Trendelenburg limp. Our intermediate results indicate that femoral shortening at the subtrochanteric level is a suitable adjunct to total arthroplasty for a completely dislocated hip.

Recombinant interferon alpha-2A combined with prednisone in metastatic renal-cell carcinoma: treatment results, serum interferon levels and the development of antibodies.

Five partial responses were seen in 23 patients with metastatic renal-cell carcinoma (MRCC) receiving interferon-alpha 2a (IFN)+prednisone (P). Four of 24 subsequent patients responded to IFN+P (combined response rate 19%). The median response duration was 8 months (3 to 30 months). The one-year survival for all eligible patients was 52%. Eight of 26 evaluable patients developed antibodies detected by an enzyme immunoassay. In 2 patients, high levels of neutralizing antibodies were also found, together with particularly low IFN levels. In one patient the development of neutralizing antibodies coincided with the loss of initial response. The treatment was well tolerated by most patients. Premature discontinuation of treatment was necessary in only 2 patients. In MRCC, combination treatment with IFN+P is as effective as IFN monotherapy (response rate 19%), with significantly reduced subjective toxicity. The clinical relevance of the development of antibodies against IFN requires further investigation.

Antibody Formation

Recombinant interferon-alpha combined with prednisone in metastatic renal cell carcinoma. Reduced toxicity without reduction of the response rate--a phase II study.

Five responses (lung metastases, three; lymph node metastases, two) were observed in 23 patients with metastatic renal cell carcinoma who received recombinant interferon-alpha-2A (IFN) 18 X 10(6) U in three intramuscular doses each week combined with oral prednisone (10 to 20 mg daily). The response duration was 4+, 4+, 9, 11+, and 15+ months. In general, the combination treatment of interferon and prednisone lead to a significant reduction of the subjective side effects (flu-like symptoms) as compared to a previous experience in patients treated with interferon only. Reduction of the interferon dose or discontinuation of IFN treatment was necessary in only two of 23 patients receiving IFN plus prednisone. Prednisone, however, had little effect on the hepatic toxicity often associated with high-dose IFN treatment. The subjective tolerability of a high dose of IFN is significantly increased if oral prednisone (10-20 mg) is given concomitantly in patients with metastatic renal cell carcinoma without reducing the response rate. Randomized trials will be necessary to confirm the efficacy of the IFN and prednisone combination. In addition, higher doses of IFN combined with prednisone should be evaluated in this malignancy.
