Tuberculosis in ulcerative colitis: bird in the bush.

Records of one hundred and two patients with idiopathic ulcerative colitis were retrospectively studied to estimate the prevalence of associated tuberculosis. There were 49 males and 53 females with a mean age of 33 +/- 13 years. Seven patients (3 pulmonary and 4 intestinal) had associated tuberculosis. All patients with intestinal tuberculosis were females and had small bowel involvement. Patients with co-existing tuberculosis were on steroids at the time of presentation and responded well to anti tubercular treatment with gradual weight gain and remission of diarrhoea and fever. We conclude that in India, the prevalence of tuberculosis is high in patients with idiopathic ulcerative colitis on steroids and a high degree of suspicion is required to detect these cases.

Epidemiology of coronary heart disease in a rural Punjab population--prevalence and correlation with various risk factors.

An epidemiological study to find out the prevalence of coronary heart disease (CHD) and the influence of risk factors on the prevalence of CHD in a total rural community of Punjab was conducted in Pohir, situated near Ludhiana. A total of 1100 individuals (623 males and 477 females) out of a possible 1617 individuals (> 30 yrs) living in 3 villages were studied. In each case a detailed history, physical examination and a 12 lead electrocardiogram (ECG) were recorded. Samples for blood sugar and serum cholesterol were taken. By Epstein's criteria of ECG (using the Minnesota coding), the prevalence of CHD was 30.8/1000, being higher in women (37.7/1000) than in men (25.6/1000). By a clinical judgement method considering history, ECG and treadmill testing (TMT) collectively, prevalence was 31.8/1000, being still higher in women (33.5/1000) than in men (30.5/1000). The prevalence of various risk factors like hypertension, smoking, hypercholesterolemia and diabetes was found to be 14.5%, 8.9%, 7.0% and 4.6% respectively. Of the various risk factors tested, hypertension, hypercholesterolemia and a positive family history showed an association with CHD. Only 38% of patients with CHD, 37% of the hypertensives and 52% of the diabetics were aware of its presence. The knowledge in the general population about risk factors causing CHD is poor.

Course of severe ulcerative colitis in northern India.

Thirty patients with severe ulcerative colitis were studied prospectively. Sixty percent (18/30) of severe ulcerative colitis were in remission after mean duration of 9.2 days (range 2-20 days) of intensive intravenous therapy without major side effects of steroids. Factors predicting poor response to medical therapy on admission are: stool frequency > or = 9 per day, pulse rate > or = 120/minute, temperature > or = 38 degrees C, Albumin < or = 2 gm, mucosal tags on plain x-ray abdomen and pancolitis.

Effect of endothelial cells on the lymphoproliferative response in Takayasu's arteritis.

Takayasu's arteritis, also known as 'non-specific aortoarteritis' is an inflammatory disease of the aorta and its major branches. It also involves the pulmonary artery. The aetiology of the disease is not known so far. Abnormalities of the endothelial cells in terms of their structure and function are seen in the pathology of a number of diseases affecting the blood vessel wall. However, involvement of the endothelial cells in non-specific aortoarteritis is not known. In an effort to identify the role of endothelial cells in the pathogenesis of Takayasu's arteritis, peripheral blood lymphocytes isolated from the blood of patients suffering from Takayasu's arteritis were cultured in the presence of endothelial cells alone and in the presence of mitogens concanavalin-A and phytohaemagglutinin-P. The peripheral blood lymphocytes of patients with Takayasu's arteritis showed a significantly decreased blastogenic response to the mitogen concanavalin-A when cultured in the presence of endothelial cells. Our result thus suggests that endothelial cells may probably induce an inhibitory effect on the lymphocytes in patients with Takayasu's arteritis.
