[The ventricular pressure-volume ratio, an index of left ventricular performance in man].

The left ventricular systolic pressure-volume diagram has been established before and after infusion of Nitro-prusside, Angiotensin and over post-extrasystolic potentiation on 33 patients, by the simultaneous measure of the aortic pressure and the left ventricular volume. In every case left ventricular dp/dt max. and (dp/dt/Pt) max. have been measured before and during post-extrasystolic potentiation, and in 16 cases after vasomotor therapy. The end-systolic left ventricular pressure-volume ratio appears to be independent of loading modifications--more than dp/dt max. and (dp/dt/Pt) max.--and on the opposite to be a good index of inotropic state alterations.

[Temporary mechanical cardiocirculatory assistance in an adolescent suffering from subacute cardiac failure].

A case is reported of subacute carciac failure during the course of a non-obstructive cardiomypathy in an adolescent. When the exacerbation of cardiac failure, which was accompanied by severe arrhythmias, failed to respond to medical treatment, the combination of a veno-arterial bypass and membrane oxygenator with diastolic counter-pressure from an intra-aortic balloon was tried with success. A reasonable remission was obtained by this means, but the young patient died 8 months later from cardiac failure which proved resistant to treatment.

[Evaluation of static elasticity and characteristic impedence of the aorta. Their relationships with age, aortic pressure and ventricular ejection resistance].

The static elasticity (Ep) (Peterson) and the characteristic impedance of the aorta (Zo) (Mac Donald) were evaluated by the simultaneous measurement of the pressure and diameter of the ascending aorta in 30 patients: 18 subjects were free from any aortic pathology (Group I), subjects had persistant arterial hypertension (Groupe II), and 3 subjects had aortic incompetence (Group III). In the group I patients, Ep increased significantly with age, and with the aortic pressure and diameter. In groupe II, Ep increased significantly only with aortic pressure. A narrow correlation was found between the characteristic impedance of the ascending aorta and the systolic ejection resistance, except in those patients with aortic incompetence, in whom it seems that Zo is a better measure of ejection resistance. In the group I patients, impedance increased significantly with age. No significant difference was found in the characteristic impedence between patients of groups I and II, the aortic diameter being greater in the latter group.

[Left ventricular perforation after insertion of a mitral prosthesis. 6 cases].

The external ruptures of the left ventricule occuring in the first twelve postoperative hours and demonstrated by a disastrous haemorrhage or cardiac failure, the other traumatics accidents could be delayed and be observed during the first month, or even later. Essentially two mechanisms are implicated: -- Direct traumatism by a ball valve on the ventricular septrum. -- Sub-annular splitting in connection with the excision of a piece of calcareous valve. Two lines of approch can be offered to avoid these accidents: -- Choose a disc valve when the surgeon finds a small left ventricle below a stenosed and calcified mitral valve. -- Care in the decalcification of the mitral ring and the valvular resection, leaving in case of need a small calcified cuff for safety. The analysis of these findings clarify the main causes of left ventricular rupture after mitral replacement, rare complications, but ones which are very real.

[Cardiac hemochromatosis. Anatomic, clinical and hemodynamical data].

The cardiovascular manifestations of idiopathic familial haemochromatosis (I.H.) should be well known. Of 25 patients seen for the most part in a Nutrition-Endocrinology Service, 10 suffered a purely cardiological problem at some point in their course.14 out of 18 patients had definite haemodynamic abnormalities. The discovery of cardiac haemochromatosis necessitates: a) depletive therapy by venesection which in certain cases is associated with the returm to normal of clinical and haemodynamic abnormalities (8-18-25). b) careful investigation of the family which, when positive, gives evidence of the primary nature of the iron excess. In members of the family suffering from I.H., early preventive therapy by vensections avoids the development of cardiovascular complications which are responsible for approximately 10 to 15 p.cent of causes of death in association with I.H.

A statistical prognostic study of pump failure in acute myocardial infarction.

Repeated measurements of maemodynamic status were made and were subsequently subjected to analysis to assess prognosis in a group of patients with acute myocardial infarction and pump failure. Two multivariate methods were used: correspondence analysis and discriminant analysis. Both these methods agreed with the highest prognostic reliability of left ventricular function indices. Using various discriminant functions a prospective study was carried out on a test sample and a rate for well-classified subjects ranging from 80% to 90% was obtained. For each discriminant function the different causes of misclassification were analysed; among them a right ventricular dysfunction was allowed to account for discrepancies between expected and observed data on inferior infarction.

[Septal perforation of myocardial infarct. Progress and limitation of current treatment. Apropos of 28 recent cases].

The emergency treatment of septal perforation in a myocardial infarction was markedly improved by pre-operative preparation of the patient by an intra-aortic balloon. If one excludes 7 cases in which the favourable immediate course made possible a delayed operation, 21 cases, severe from the start, were observed in the last two years: 8 patients, were thought to be operable with 6 successes. The type of the patient, the age, the associated diseases, the gravity and extension of the local lesions explain that this complication nevertheless still remains very often beyond the present possibilities of the medical-surgical treatment.
