Multiple arterial aneurysms.

An 8 1/2-yr-old boy presenting with excruciating abdominal pain had two abdominal aortic aneurysms excised. Angiography demonstrated additional aneurysms of the right brachial artery, right renal artery, right internal iliac artery and multiple aneurysms of the right popliteal and tibialis posterior arteries. The patient does not suffer from any of the conditions generally recognized to be associated with aortic and multiple arterial aneurysms. No similar case could be found in the English language literature.

Absence of deleterious effects of chronic microwave radiation on the eyes of rhesus monkeys.

Microwave irradiation of rhesus monkeys' eyes at 9.31 and 2.45 GHz and at an average power density of 150 mW per centimeter square is reported. Irradiation, beginning in 1976, of 17 monkeys (Macaca mulatta) was accomplished without restraint or anesthesia by training the monkeys to irradiate themselves. To data microwave radiation of these monkeys has not resulted in deleterious ocular effects.

Far generalization of visual analogies strategies by impulsive and reflective EMR students.

Cognitive style and its interaction with training effects in promoting the acquisition and far generalization of a visual analogies strategy by 22 male and 17 female EMR pupils was evaluated. Training was effective in producing acquisition and far generalization; cognitive style was not. We suggested that the lack of interaction between cognitive style and training was due to the fact that the training program modified the subjects' cognitive style, i.e., impulsives tended to perform more like reflectives.

Duplication of the gallbladder associated with childhood obstructive biliary disease and biliary cirrhosis.

A 4-yr-old girl presented with recurrent attacks of abdominal pain, vomiting, fever, and jaundice, progressing to biliary cirrhosis. The diagnosis of duplication of the gallbladder was established by transhepatic cholangiography. Clinical recovery and histologic improvement followed removal of the gallbladders. In every case of obstructive biliary disease or biliary cirrhosis in childhood, no effort should be spared in searching for a malformation of the biliary system.

Associative thinking in schizophrenia: a contextualist approach.

Explored the question of whether responses that appeared to be highly pathological on the basis of the word association test did indeed reflect an underlying aberrant associative process or whether the associations actually had a greater degree of meaning than was evident from the test (N = 60). Utilizing contextualist techniques designed by the authors, which were analogous to the word association test but yet measured schizophrenic associative thinking in specific contexts that more closely approximated natural language situations, the quality and purposiveness of the schizophrenics' associations were examined. The word association test technique was judged to be inadequate by itself to account for underlying associative processes involved in schizophrenic associative thought disorder. A majority of responses (70% schizophrenic, 81% nonschizophrenic) judged to be pathological on the basis of the word association test alone became meaningful in the context of a sentence created by the Ss to explain purposively their associations. The schizophrenics' experience of a stimulus word and consequent associations to that word became clearer when placed within an appropriate context (i.e., a sentence), rather than examined as isolated semantic features (as in the word association test).

Percutaneous nephrostomy for assessment of functional recovery of obstructed kidneys.

The use of percutaneous nephrostomy to evaluate the recoverability of function of the obstructed kidney is described and examined as a practical clinical method. Long-term drainage was used to assess the functional status of four kidneys and found to be of great value. The use of percutaneous nephrostomy to evaluate function has been mentioned previously but has not become established. In view of inaccuracy of other methods of assessment, we consider percutaneous nephrostomy to be the most accurate predictive test available today and suggest criteria for its use.

Evidence for in vivo internalization of human leukocyte elastase by alveolar macrophages.

Cell lysates from cultured human alveolar macrophages contain detectable amounts of an elastinolytic enzyme. Although particulate elastin was solubilized only after prolonged incubations, lysates readily hydrolyzed T-OC-alanyl-p-nitrophenol-ester. Hydrolysis of the latter substrate was inhibited by the leukocyte elastase site-specific inhibitor, N-ac-(ala)4-chloromethyl ketone. In addition, radioimmunoelectrophoresis of concentrated alveolar macrophage lysates, previously incubated with 3H diisopropyl-phosphofluoridate (DFP), revealed the presence of DFP binding material that comigrated with inactivated human leukocyte elastase. Human leukocyte elastase can cause lung lesions resembling pulmonary emphysema in experimental animals; therefore, the clearance of this enzyme by alveolar macrophages may represent a significant route for the removal of this potentially pathogenic enzyme from the lung.

Endoscopic findings in sleep apnea associated with acromegaly.

Two acromegalic patients presented with the additional complaint of excessive daytime sleepiness. Subsequent sleep laboratory evaluation revealed that each patient had concurrent obstructive sleep apnea. In each patient, endoscopic examination of the upper airway revealed that on inspiration, the soft tissue of the posterior and lateral hypopharynx invaginated into the laryngeal vestibule before any posterior movement of the tongue. Neither enlargement of the tongue nor in movement in relation to respiration appeared to be the primary factors in the etiology of the upper airway obstruction. After tracheostomy, both the obstructive sleep apnea and excessive daytime sleepiness were eliminated. Our case reports indicate that the treatment and prognosis of patients with acromegaly are affected when sleep apnea is concurrently diagnosed. When a patient is suspected of being acromegalic and also complains of excessive daytime sleepiness, obtaining a through sleep history from the patient as well as from the bed partner is essential. If sleep apnea is then diagnosed, a tracheostomy should be considered an initial and continuing part of the overall treatment plan.

Computed tomography of ureteral obstruction.

Although hydronephrosis can usually be diagnosed by urography and/or pyelography, the etiology of the obstruction may not be apparent. Computed tomography (CT) is usually helpful in evaluation of these cases. In 36 cases in which CT was performed solely to determine the cause of ureteral obstruction of uncertain etiology, it proved to be of value in 33 instances (91.7%). The disease processes encountered in this series included metastatic carcinoma to the ureter or periureteral tissues(22), lymphoma (one), primary ureteral tumor (two), radiolucent ureteral stone (four), adjacent ileocolitis (two), aortic or iliac artery aneurysm (two), fibrous band(one), urinoma (one), and radiation fibrosis (one). In three cases (8.3%) an etiologic diagnosis could not be made by CT. These patients had radiation therapy to treat malignant disease, and recurrent tumor responsible for ureteral obstruction could not be distinguished from radiation fibrosis.

Adjuvant antiplatelet therapy with aspirin in colo-rectal cancer.

Sixty-six patients with Duke's B2 or C colon or rectal cancer were randomized for treatment with aspirin (ASA), 600 mg, p.o., twice daily x 2 years or placebo (P). Compliance was checked in both groups by random measurement of blood salicylate levels. Fifty-seven patients are currently evaluable. No difference in disease-free (p = .66) or overall survival (p = .90) is present between ASA and P groups. The time at which ASA therapy is started (within 2 or within 4 weeks) following surgery does not affect these results. Aspirin at conventional dosage is ineffective in preventing the appearance of metastases in patients with colo-rectal cancer.
