[Malignant tumor-like abdominal lymphoma in Whipple's disease].

A 43-year-old man had severe upper abdominal pain and weight loss of 8 kg for over three months. He underwent a laparotomy because, computed tomography having revealed numerous mesenteric and para-aortic lymph nodes, a malignant lymphoma was suspected. Histological examination of a mesenteric lymph node demonstrated exclusively extraintestinal Whipple's disease. The symptoms completely disappeared after the administration of 1.2 mega U penicillin G and 1 g streptomycin daily for 14 days, followed by twice daily 160 mg trimethoprim and 800 mg sulphamethoxazole.

A model for the comparison of pharmacologic agents influencing gallbladder contraction.

Investigations on pharmacologic effects on gallbladder contraction must take into consideration the spontaneous motility of the gallbladder. In 20 healthy subjects, we measured the area of the gallbladder sonographically over 10 h at hourly intervals. The spontaneous motility expressed as a coefficient of variation varied between 1.6 and 13% (mean value, 6.3%). Afterwards, we checked the cholecystokinetic action of ceruletide and fatty meal in eight subjects. Here, an alteration of the area by four times the previously found coefficient of variation was defined as significant. A significant contraction occurred earlier after ceruletide administration than after a fatty meal (10 min as compared to 20 min). This lasted longer (40 min. vs. 25 min) and led to a very much more marked reduction in area (69% as compared to 53%). These differences were significant on a 5% level. The end of the contraction did not differ (50 min as compared to 45 min). We conclude that drugs which affect gallbladder contraction can be compared with consideration of spontaneous motility.

A simple technique for sonographic estimation of liver volume.

A procedure for estimation of liver volume with the sonographic real-time technique is reported. Testing of this procedure in vitro revealed a very good linear correlation of sonographically determined liver volumes and liver volumes measured by water displacement. In an in-vivo measurement of 70 healthy subjects, a mean of 1402 ccm was determined for men and 1257 ccm for women. A statistically significant correlation was found with body weight and body surface. The method presented is simple and can be carried out in a short time during a routine examination.

[Functional and morphological changes in the stomach in irradiation of the para-aortic region].

Six assays were conducted in order to determine some functional and morphological parameters of the gastric mucosa in patients who underwent Co60 irradiations of the para-aortic area. Slight as well as marked morphological alterations were observed whereas the functional alterations were found to be marked in all cases. Basal gastric secretion and serum gastrin level showed a continuous reduction, however, in case of severe gastritis the stimulated secretion was increased. A possible correlation with increased tissue histamine levels is discussed.

[Differential diagnosis of jaundice by combined noninvasive diagnostic means using a logistic discriminant analysis].

Using a logistic discriminant function we retrospectively evaluated the diagnostic value of laboratory features and abdominal sonography in 70 patients with jaundice. 18 patients had an extrahepatic obstruction of the common bile duct (EHO), 22 patients had metastatic liver disease (MLD) and 30 patients had an infectious or toxic hepatocellular disease (HCD). The sensitivity resp. specificity of the 5 laboratory values AP, GGT, GPT, GOT and bilirubin was 22% resp. 90% for diagnosing EHO, 82% resp. 79% for diagnosing MLD and 67% resp. 68% for diagnosing HCD. The diagnostic value determined by their Chi2-value was AP greater than GPT greater than bilirubin greater than GOT greater than GGT. An undoubtedly dilated common bile duct was seen in 56% of the patients with EHO and in none of the other patients. Metastatic lesions were seen exclusively in 81% of the patients with MLD. No distinct sonographic sign could be found for the patients with HCD. The combination of AP, GPT and bilirubin with the result of abdominal sonography in a logistic discriminant function led to a correct a posteriori classification of all patients. Using a mathematical classification model jaundiced patients can be diagnosed on the basis of noninvasive methods alone and invasive procedures should be restricted to therapeutic interventions.

[Premedication for abdominal sonography--comparison of the efficacy of 2 dimethicone preparations].

Two commercially available preparations of dimethycon with different galenic properties (Paractol flüssig, Sab simplex) were tested for their usefulness as a premedication for abdominal sonography. According to the recommendations of the manufacturers each of 20 patients was pretreated with four different galenic preparations of the two substances that were administered in a randomly assigned sequence. 13 abdominal regions of interest were investigated for visualisation of organs and gas interposition. Both groups of drugs clearly improved the quality of imaging of the abdominal vessels and the epigastric structures, but Paractol proved to have significantly better effects. This is possibly due to a higher dosage of dimethycon in Paractol.

[Gallstones as depicted in the sonogram and radiogram. A comparative study].

Eighty-seven unselected gall stones were examined radiologically and by ultrasound in vitro. No relationship of diagnostic value could be established between the amount of calcification and stone demonstration, the sonogram being influenced by the size of the stone, its surface contour and its position in the ultrasound beam. Sonography is therefore not suitable for selecting those patients on whom litholysis could be carried out. Sonography appears to be useful only in a negative way by establishing the exact size of the concretion.

[Multiple fractures of the atlas with delayed fatal thrombosis of vertebral artery following "whiplash" injury of the neck (author's transl)].

Report of the case of a 62 year old male, who died in a car crash on his way home from duty, when he had a frontal collison with annother car at a speed of about 70 km per hour. After the collision, the car had completely turned over to its wheels. As the man had put on the seat belts, he had not been cast against the front window so that there were no head injuries. Following an unconsciousness of about ten minutes the patient recovered quickly and was already fully oriented when arrived at hospital. No neurological symptoms could be substantiated. He only suffered from pain of the neck and had a marked swelling of the left side of the neck. A burstin fracture of the atlas vertebra (Jefferson-fracture) was overlooked in routine X-rays of the craniovertebral region. Eight days after the accident, the hitherto "well improving" patient died suddenly and unexpectedly. The autopsy revealed an obstructing thrombosis of the left vertebral artery, less than 24 hours old. The wall of the artery showed numerous dissecting ruptures with intramural bleedings.

[Salivary and duodenal amylase production in patients with chronic hepatic diseases (author's transl)].

Investigating patients with chronic diseases of the liver, there will sometimes be found a symmetric hypertrophy of the parotid glands. Supposing, that enlargement of the parotid glands could equal an enhancement of parotid function, the authors investigated amylase and volume production of the parotid glands and at the same time that produced by pancreatic excretion during pancreozymin/secretin stimulation. They found a compensatory action of the parotidean function: In patients with only small liver cell damage they saw a diminished parotidean function together with an enhanced pancreatic excretion. In cases with severe liver cell alteration, the parotidean function was found to be greater than in normals, the pancreatic function diminished. The authors estimate the parotidean hypertrophy as a morphologic equivalent to the compensatory enhancement of parotidean function in cases with chronic liver diseases.

[Histological findings after transabdominal ligation-resection of the esophagus ad modum boerema (author's transl)].

The esophagus was histologically studied in ten patients, who died after transabdominal ligation-resection of the esophagus ad modum Boerema because of bleeding esophageal varices. The patients had survived the operation for various periods of time. The questioning was, if with this operation with a higher incidence of parietal leakage following mediastinitis and peritonitis has to be calculated. We could not confirm that hypothesis. On the contrary we recognized at the level of the ligation simulataneously degenerated muscles fibres and a regeneration of the esophageal wall by fibrous tissue compensation.

[Myocarditis and insulitis following coxsackie virus infection].

We report on the case of a 5 7/12-year-old girl, who presented an acute insulin-dependent diabetes mellitus nineteen days after the operative suture of an atrial septum defect, and who died in a hyperosmolar coma diabeticum in spite of an intensive therapy. At the autopsy there was shown a myocarditis with the histological characteristics of a virus infection. The islet tissue of the pancreas was largely necrotic, the great majority of the islets of Langerhans presented lymphocytic infiltrates. In a neutralisation test against coxsackie-B-viruses (CBV), which was conducted after death, there was found among others an antibody titer of 1:192 against CBV-4. With FITC-marked CBV-antiglobulin, in the islet tissue intracellular CBV-antigen could be proved. It is discussed that virus infections have an etiological importance for the incidence of a so-called juvenile diabetes in man.

[Insulitis and peracute diabetes mellitus (author's transl)].

We report on the case of a 10-month-old infant with Down's syndrome and acute insulin-dependent diabetes mellitus, who died of hyperosmolar diabetic coma 4 days after admission to the hospital in spite of intensive therapy. Characterizing this disease, a lymphocytic infiltration of the islets of Langerhanns with destruction of the islets was found. The specific localization of the inflammatory infiltrates as well as the histological findings correspond with experimental immune-insulitis in aminals, suggesting that immunological mechanisms play an essential pathogenic role. Virus etiology, diagnostic procedures, and therapeutic approaches are discussed.
