Quantitative assessment of psychologic state of patients with anorexia nervosa or bulimia: response to caloric stimulus.

Hospitalized patients with anorexia nervosa (N = 17) or bulimia (N = 11) were given a standard liquid meal containing 400 calories. Using analogue scales, bulimic patients were found to have greater anxiety, lower mood, lower sexual arousal, and more fear of fatness than either control or anorectic patients. This finding of increased general "dysphoria" in bulimic patients persisted after the meal without any significant premeal to postmeal changes. Anorectic patients also differed from controls, but less than the bulimic patients. Some measures of anxiety correlated significantly with body mass index before the meal in bulimic patients, whereas in anorectic patients the correlation was significant only after the meal.

Mood change following bilateral hemisphere brain injury.

Fifteen patients with bilateral hemisphere brain injury secondary to thromboembolic stroke or trauma were evaluated for depression, cognitive impairment, and functional physical impairment. Analysis of CT scans and standardized mood scales revealed that patients with left anterior brain injury were significantly more depressed than patients without such injury. Depression severity was directly and significantly correlated with left lesion proximity to the frontal pole. Left lesion age, lesion temporal sequence, right lesion location, cognitive impairment, and functional physical impairment did not significantly correlate with depression. Depressive symptomatology previously shown to be associated with single left frontal lobe lesions appeared to dominate post-brain injury psychopathology regardless of location or temporal sequence of other brain lesions.

The effect of food consistency on iron absorption.

Iron absorption and the half-time of gastric emptying were measured concurrently in 13 normal volunteer subjects. Each subject ate two test meals of identical composition; one meal was prepared and served in the usual manner while the second was homogenized before serving. No correlation was found between the half-time of gastric emptying and the absorption of either heme or nonheme iron among subjects eating the conventional meal. Homogenization of the meal led to a 31% fall in the mean half-time of gastric emptying and an increase in iron absorption of 22% for nonheme and 42% for heme. These findings indicate that the physical form of a meal has a modest effect on iron absorption.

Intracortical kainic acid induces an asymmetrical behavioral response in the rat.

Five nanomolar doses of kainic acid injected into the right frontal cortex of rats produced a significantly greater spontaneous hyperactivity than identical injections into the left hemisphere. Hyperactivity began one week after injection and slowly continued to increase throughout the 30 day postoperative period. These results taken with our previous studies suggest that the neural asymmetry which leads to hyperactivity following right hemisphere lesions may be postsynaptic to cortical noradrenergic terminals.

Rubella immunity in older children, teenagers, and young adults: a comparison of immunity in those previously immunized with those unimmunized.

Because rubella continues to be a common illness in adolescents and young adults and because it has been suggested that booster rubella immunizations should be performed, we studied antibody prevalence in 459 predominantly adolescent patients in a pediatric group practice. Rubella antibody (PHA titer greater than or equal to 1:13.5) in previously immunized patients (89.6% of 385) was significantly more common than antibody in unimmunized patients and patients with a questionable history of immunization (70.3% of 74) (P less than 0.005). Twenty-three seronegative patients with a documented history of prior immunization were reimmunized and 22 had an IgG (secondary) antibody response and only one an IgM (primary) antibody response. Since all but one of our patients with previous immunization had a secondary immune response following revaccination, it seems likely that the level of protection in previously vaccinated individuals is considerably greater than 90%. Attention today should be directed at finding and immunizing unvaccinated teenagers and young adults and not in major booster vaccine programs.

Oral anticholinergics and gastric emptying.

The quarternary ammonium antimuscarinic drugs propantheline bromide and clidinium bromide, given orally at the usual therapeutic doses, delayed gastric emptying of a swallowed radiolabeled liquid meal as measured by a gamma camera. Delay of emptying was dose dependent. If an identical meal was given by gastric tube, there was no slowing of emptying by propantheline in the group as a whole. Six subjects who emptied the intubated meal more quickly with placebo had slowed emptying after 30 mg propantheline. In five others, intubation alone slowed gastric emptying while the addition of 30 mg propantheline caused a paradoxical acceleration of gastric emptying. Clidinium bromide, 5 mg, delayed gastric emptying to the same extent as 15 mg propantheline bromide without the marked suppression of salivary secretion induced by the latter.

Some observations on urinary retention and deep hip sepsis following total hip replacement.

A follow-up study was made of 256 hip replacements, performed at Dunedin Hospital by three orthopaedic surgeons in males aged 45 years and over for the six year period beginning 1975 through to the end of 1980. One surgeon (A) had an elaborate management policy designed to reduce postoperative urinary retention and his cases are compared with Surgeon B's whose methods of avoiding urinary retention would be more representative of the majority of orthopaedic surgeons. Analysis was performed of the 20 cases of urinary retention pooled from all three participating surgeons. Also analysed were the 24 cases of epidural anaesthetics in the entire series. Surgeon A had a significantly (P=.045) lower postoperative urinary retention rate of 3.23% compared to surgeon B (12.07%). Analysis of the 20 cases of urinary retention revealed that cases having retention after previous operations and having nothing done about it invariably went into retention again. The retention rate after the 24 epidural anaesthetics was 16.7% and the retention rate after the 232 general anaesthetic cases was 6.9%. This difference was not statistically significant (p=0.10).

Activities of four aminoglycosides at subbactericidal concentrations.

This report describes an activity range index derived from the minimal bactericidal and antibiotic concentrations of four aminoglycosides against four representatives of clinically important gram-negative bacilli. The ranking orders of in vitro activity differed from those using the minimal bactericidal concentration or the minimal antibiotic concentration as the sole criterion.

Bronchocentric granulomatosis: roentgenographic manifestations.

The roentgenographic manifestations of bronchocentric granulomatosis were reviewed in 15 patients. Nine patients had mass lesions. Four had alveolar infiltrates and 2 presented with reticulonodular infiltrates . Correlation of these radiographic patterns with the pathologic findings helps explain the varied morphologic appearance of this disease on the chest radiographs. Upper lobe involvement and unilateral disease were predominant. Seven of 15 patients (47%) presented with solitary mass lesions. Pleural reaction was occasionally noted. Hilar adenopathy and cavitation were infrequent. There was little correlation between radiographic pattern and clinical presentation.

A peripheralized digital image management system: prospectus.

The number of diagnostic radiology examinations being performed on digitally formatted imaging equipment is continually increasing. However, most digital data are recorded as analogue images and the films stored in a central file. The dynamic range of raw digital data is lost. Currently, the space and cost requirements for storage of all digital data have discouraged most users from attempting it. This article presents the concepts and describes the requirements and initial fabrication of a system that will capture and retain all digital data using a network of peripheralized image acquisition, display, and storage devices. Any piece of digitally formatted equipment can be interfaced into the system. A real-time computer capability provides that any digital examination can be reviewed at any display station for up to 10 days.

Congenital hypoplastic anemia (CHA) associated with congenital absence of the spleen.

A 19-year-old boy with congenital hypoplastic anemia (CHA) treated with long-term steroid therapy, presented with septic shock caused by Streptococcus pneumoniae. Peripheral blood smears revealed Howell-Jolly bodies and postmortem examination revealed the absence of the spleen, splenic vein and splenic artery. We have found no association of CHA and congenital asplenia previously reported in the literature or in a review of 12 additional patients with CHA at this institution. The case is reported to bring attention to the concurrence of these two rare conditions in one individual and to discuss the possible implications of this association.

Suction lesions of the frontal cerebral cortex in the rat induce asymmetrical behavioral and catecholaminergic responses.

Suction lesions of the right frontal cerebral cortex in rats induce a period of spontaneous hyperactivity. The hyperactivity, as measured by an increase in running wheel activity begins about one week post-operatively and continues throughout the remainder of a 30-day observation period. The increased activity is accompanied by a bilateral decrease in norepinephrine concentrations in both the ipsilateral and contralateral cortex and locus coeruleus. Identical lesions of the left frontal cerebral cortex produce neither the hyperactivity nor a decrease in norepinephrine concentrations. These experiments have reproduced many of the behavioral and biochemical asymmetries seen after middle cerebral artery ligation; however, suction lesions are both simpler to produce and histologically less variable in their effects.
