Digital radiography of the chest: clinical experience with a prototype unit.

A prototype digital unit dedicated to chest radiography was used to examine 50 selected patients for a comparison study of the capability of digital images and conventional chest radiographs to reveal normal anatomic structures and a variety of pathologic states. The images in both modes were submitted for interpretation to seven experienced radiologists and a standardized questionnaire completed for each. Visibility of seven anatomic structures in the mediastinum was consistently better on the digital images than on the conventional radiographs. With minor exceptions, pathologic states were equally well seen in the two systems. Despite the less familiar viewing format of the digital images, the mean confidence levels achieved were higher than for those on the conventional radiographs; this difference was statistically significant both for normal anatomic structures (p = 0.001) and pathologic states (p = 0.01). The advantages and disadvantages of the digital technique are discussed.

Pulmonary 'mainline' granulomatosis: talcosis of intravenous methadone abuse.

Seventeen intravenous abusers of methadone underwent clinical, roentgenologic and physiologic assessment. Two complained of dyspnea on exertion and two had cor pulmonale. Of 15 patients whose fundi were examined, nine had talc particles in their retinal vessels. The chest roentgenograms of seven showed a diffuse pin-point micronodular pattern and two of the seven also manifested volume loss, one with coalescence of opacities simulating progressive massive fibrosis. Twelve patients had some degree of pulmonary dysfunction, 10 with lowered steady state diffusing capacity and 11 with decreased flow rates (FEV1 and MMF). There was no hyperinflation, but two showed an increase in residual volume. Corticosteroid therapy was attempted on two and was ineffective. Necropsy on the one patient who died revealed severe pulmonary fibrosis and talc granulomas in lungs, liver, kidneys and lymph nodes.

Discrimination of normal and overinflated lungs and prediction of total lung capacity based on chest film measurements.

Plethysmography and roentgenography were performed on normal subjects and patients with emphysema. 23 measurements from postero-anterior and lateral roentgenograms were subjected to discriminant and regression analyses to discover (a) which variable(s) best distinguished normal from overinflated lungs and (b) whether these or other variables could be used to predict total lung capacity. A combination of measurements was found to work best in each case.

The roentgenologic diagnosis of chronic bronchitis: a reassessment with emphasis on parahilar bronchi seen end-on.

In an attempt to reassess the accuracy of the roentgenologic diagnosis of chronic bronchitis, we have studied the chest radiographs of approximately 300 men comprising roughly equal numbers of normal subjects and patients with chronic obstructive pulmonary disease (COPD). Emphasis has been placed on the thickness of bronchial walls visualized end-on in the parahilar zones, identified in roughly 80% of both groups. Step-wise discriminant analysis of six variables showed that the median estimate of bronchial wall thickness was of some value in discrimination.

Massive pulmonary gangrene: a severe complication of Klebsiella pneumonia.

SUMMARY: Massive pulmonary gangrene developed in two patients. Review of the literature reveals 10 other case reports of pulmonary gangrene complicating lobar pneumonia. Among the total of 12 patients whose cases have now been reported, all 4 patients who were treated nonsurgically died and the 8 who underwent surgical resection of the gangrenous lung survived. The present report emphasizes the necessity of early recognition and appropriate surgical treatment for a successful outcome.
