General factors affecting the prognosis of thyroid cancer are age and sex of the patient. The size of the primary tumour seems to be unimportant whilst the thyroid capsule is intact. Distant metastases shorten the survival time, whereas in cases of differentiated carcinoma cervical lymph node have no prognostic influence. A combined therapy of near-total thyroidectomy, 131-J-treatment, and TSH-suppression therapy using thyroid hormones is most successful. Additional external radiation seems to exert some positive effects in special cases. The cell-type has a great prognostic influence irrespective of the above mentioned variables. Papillary carcinoma has the best prognosis with a 5-yrs and 10-yrs survival rate of 86% or 78% respectively. The rates for follicular carcinoma are 72% respectively or 60%, for oncocytotic carcinoma (Hürthle's cell cancer) 83% and 50%, and for medullary carcinoma (C-cell cancer) 70% or 55% respectively. The natural history of undifferentiated (anaplastic) carcinoma is nearly always fatal with survival rates of 9% and 3% for 5 years or 10 years respectively.

We report of an allotransplantation of neonatal pancreatic tissue. The donor was a three-week-old girl with nesidioblastosis, in which a subtotal pancreas resection had to be performed. The pancreas microfragments were injected transcapsularly into the spleen of a patient with juvenile diabetes. During the postoperative course only a short-termed improvement of the diabetic condition was seen.

Islet transplantation in human diabetes at present is confronted with two major obstacles: isolation of a sufficient number of islets and islet graft rejection. "Tissue banking" would enable islet pooling from various donors and offers furthermore the advantage of in vitro manipulations in order to reduce islet immunogenicity. Recently we have reported successful cryopreservation and subsequent transplantation of porcine islets (Bretzel, Beule, Schäfer, Schneider, Pfeiffer and Federlin 1979). These preliminary data deal with cryopreservation and transplantation of isolated rat islets and cryopreservation of isolated human islets.
A 36-year-old man suffering from chronic pancreatitis involving the whole organ had total duodenopancreatectomy. The Langerhans' islets were isolated from the extirpated organ and transplanted into the liver via portal artery. Insulin substitution could be lowered from 50 I.U. to 12 I.U. by the 5th post operative day. Repeatedly, raised insulin levels could be verified in response to high blood sugar loads. C-peptide and glucagon were also found. 7 months after transplantation the islets still seem to be functioning.
Intraportal transplantation of isogeneic adult islets in diabetic rats resulted in long-lasting amelioration of the metabolic disorder. The effect upon diabetes-induced kidney changes (enlargement of mesangial space, capillary changes, cell-proliferation) was examined quantitatively by morphometric studies. The major effect was a remarkable reduction of the mesangial space and re-widening of the capillary lumina. The number of endothelial cells was lowered. Regarding epithelial and mesangila cells no difference was observed between normal, diabetic and transplanted animals of age-matched groups.
