Coronary artery surgery and direct coronary artery thrombolysis during acute myocardial infarction.

Twelve patients underwent direct coronary artery streptokinase thrombolysis during acute evolving myocardial infarction. Ten patients had restoration of orthograde pulmonary blood flow. Eight patients had significant myocardial salvage and clinical stabilization. Nine patients had subsequent coronary artery bypass surgery with no major hematological or cardiac complications. Five of these had surgery performed immediately. Two patients with no myocardial salvage after streptokinase had urgent surgery because of other critical coronary lesions. Seven patients with thrombolytic salvage of myocardium underwent urgent coronary artery bypass surgery without incident. The occurrence of two reinfarctions in streptokinase-improved patients waiting for surgery suggests that delay is unwarranted and coronary bypass surgery is needed to insure success.

[Sub-populations of T helper suppressor lymphocytes in diverse dermatoses using monoclonal antibodies. Preliminary results (author's transl)].

The total T lymphocyte and T cell subsets were counted in various dermatoses by indirect immunofluorescence using the monoclonal antibodies OKT3, OKT4, OKT8 which distinguish all peripheral T cells, helper T cells and suppressor T cells respectively. In particular, 7 cases of the Sézary syndrome and 7 cases of mycosis fungoides were studied. In addition, 42 other patients with diverse dermatoses such as psoriasis, atopic eczema, bullous disorders, sarcoidosis, lupus erythematosus and leprosy were studied and compared to 20 healthy controls. There was no significant increase in the total T cell percentage in the ratio of helper to suppressor lymphocytes in the Sézary syndrome with a normal balance in mycosis fungoides. Among the other dermatoses only atopic eczema showed a significant increase in the helper/suppressor ratio suggesting an imbalance of T cell subpopulations in this condition.

Anticipatory grief: a psychosocial concept reconsidered.

Formerly anticipatory grief was viewed as a potential coping mechanism for a prospective loss. More recently it has been studied in preventive psychiatry as a determinant of the severity of post-mortem grief. The authors in a critical analysis of methodological and theoretical inconsistencies recommend a reconsideration of the concept within a psychosocial context.

Smoking and warfarin disposition.

Nine cigarette smokers ingested an average daily 0.032 mg/kg dose of sodium warfarin for 2 wk while continuing to smoke and for an additional 2 wk after having abstained from cigarette smoking for a month washout phase. Steady-state plasma levels of warfarin, clearances, t 1/2, apparent volumes of distribution, steady-state prothrombin times, and plasma thiocyanate levels were measured during both the smoking and nonsmoking phases. During the nonsmoking phase there was a 13% increase in average steady-state warfarin level and a 13% decrease in warfarin clearance rate. There also was a 23% increase in warfarin t 1/2 and an 11% increase in the apparent volume of distribution. Prothrombin time did not change. Thiocyanate levels were 3 to 4 times as high during the smoking than the nonsmoking phase. It appears that cigarette smoking does affect warfarin clearance, t 1/2, and apparent volume of distribution, though the net effect on warfarin's pharmacodynamic activity is negligible, at least at doses which are ineffective therapeutically.

Retroviral transduction of T-cell antigen receptor beta-chain and myc genes.

Support for multistage models of oncogenesis has been provided by several highly leukaemogenic retrovirus isolates that have transduced more than one host cell gene. Where functional studies have been performed, these retroviral oncogenes show synergy for in vitro transformation and leukaemogenesis. In naturally occurring feline leukaemias associated with feline leukaemia virus (FeLV), retroviral transduction of myc is a frequent oncogenic mechanism. But evidence suggesting that the FeLV v-myc genes might be insufficient for leukaemogenesis was provided by the latency (12 weeks) and clonality of FeLV/v-myc-induced tumours and the absence of demonstrable in vitro transformation by these viruses. In the search for secondary leukaemogenic events in FeLV/v-myc tumours, we have identified a case of FeLV transduction of a T-cell antigen receptor beta-chain gene. The proviruses carrying this gene (which we have named v-tcr) were a separate population from those carrying v-myc. In its normal role, the T-cell receptor beta-chain forms part of a multimeric complex involved in antigen recognition and T-cell activation. We suggest that v-tcr is a novel viral oncogene which assisted v-myc in the genesis of a naturally occurring case of thymic lymphosarcoma.
