[Clinical aspects of obstructive pneumopathy (COPD)].
This paper gives a short overview of the most pertinent clinical features and diagnostic measures in chronic obstructive lung disease. In particular, the patient's smoking habits should be questioned extensively, a prolonged expiratory phase identified and pulmonary function tests performed properly.
[Hereditary angioedema: description of a sibship and review].
Report on a newly recognized sibship with hereditary angioedema (HAE). The 27 persons investigated include 8 with decreased concentrations of C-1INH and C4 in the Serum. Three of these are children without symptoms of HAE. Five patients have characteristic attacks of HAE, some of which are predominantly abdominal. The effects of treatment with danazol in 4 patients are described. HAE is briefly reviewed with special reference to pathogenesis and treatment.
Early stage of fulminant idiopathic pulmonary fibrosis cured by intense combination therapy using cyclophosphamide, vincristine, and prednisone.
A 19-year-old patient was treated who developed a life-threatening pulmonary insufficiency after having suffered from a flu-like illness for 13 days. Lung biopsy revealed an early stage of idiopathic pulmonary fibrosis. Chemotherapy by prednisone alone proved ineffective. A combination of prednisone and massive doses of cyclophosphamide and vincristine, however, was effective in halting the rapid progression of the disease. The patient recovered gradually; the results of the pulmonary function tests were close to normal 2 weeks after the onset of therapy. Lung biopsy showed normal lung histology 7 months later. The patient has had no complaints at all for more than 3 years after the episode so that we may assume that he recovered fully.
[Mechanisms which favor the development of opportunistic infections].
Opportunistic infections frequently occur in patients whose defense mechanisms are compromised. Some of the available defense mechanisms of the human organism are outlined, and the case is presented of a patient whose defense was weakened or destroyed, i.e. compromised by various mechanisms. It is important to recognize the increasing frequency of opportunistic infections inside and outside the hospital environment.
