[The action of Buphenin on uteroplacental haemodynamics (author's transl)].

The action of buphenin, a uterine relaxant from the group of adrenergic beta-mimetics, was examined on the blood pools of placenta, myometrium and heart, in 10 pregnant women. The method consisted of an i.v. injection of the radioisotope 113m-Indium and of registering the blood pools by external measurement of the gamma-activity. buphenin caused an increase in placental and myometrial blood pools with a corresponding decrease in cardiac blood pool. These changes were observed in all the cases examined and occurred whether the uterus had previously been inactive or in labour. The changes in blood pool were dependent on the dose of buphenin administered but there was no relationship apparent between the degree of pool changes and severity of pregnancy complication prevailing.

[Studies of the blood pool dynamic actions of dipyridamole in pregnancy (author's transl)].

The action of 2,6-bis(diethanolamino)-4,8-dipiperidinopyrimido-(5,4-d)pyrimidine (dipyridamole, Persantin) on utero-placental and cardiac blood pool dynamics was examined in 12 pregnant women. 113mIndium was injected i.v. and blood pool changes in the heart, placenta and myometrium were registered by external measurement of the gamma-activity. While there was a significant decrease in blood pool of the heart after dipyridamole, there were no marked changes in the blood pools of placenta or myometrium. The decrease in cardiac blood pool was interpreted as a sign of an increase in the cardiac output.

Triple radioisotope technique in etiologic evaluation of space-occupying lesions of the liver.

The findings from 43 patients with histologically proven focal liver disease examined by triple isotope tracer study are presented. The method consisted in a three step technique, the first step being an isotope scan after injection of 99mTc-sulfur colloid and 131I rose bengal. As a second step dynamic hepatic scintiangiography was carried out with 99mTc human serum albumin in order to assess the vascularity of a lesion detected on the initial scan. Thirdly, the patient was administered 67Ga to evaluate further the pathologic process. According to our results this triple radioisotope technique provides the clinician with valuable information on the possible etiology of focal liver disease.

[Determination of serum digoxin: a path to better control of therapy?].

The usefulness of serum digoxin measurements in the diagnosis of digitalis toxicity was studied prospectively in hospitalized patients. Serum levels were increased above 2 ng/ml in 28 of 32 cases with digoxin-induced arrhythmias and/or gastrointestinal symptoms, whereas only 1 of 35 non-toxic patients had an abnormal serum concentration. There was, however, no direct relationship between the type of toxic manifestation and absolute values of serum digoxin.

Studies on the haemodynamic action of complamin in pregnancy.

In the present work, an attempt has been made to verify whether Complamin improves placental circulation as suggested by several authors. Complamin was administered intravenously to patients in the last trimester of pregnancy and its action on blood pool dynamics was examined over 30 min. The results from 30 investigations showed that a significant reduction in uteroplacental blood pool occurred. The production of placental hypovolaemia, which led to a reduction in blood pool of more than 30% in some cases, is regarded as a potential danger to the fetus.

[The measurement of gastric emptying of solid food. Comparison of 2 solid phases by means of double isotope technic].

The suitability of a simple method of measuring the rate of gastric emptying of solids using 99mTc-marked filter paper pieces has been evaluated. In five healthy volunteers emptying of the filter paper pieces and of 198Au-tagged liver was measured simultaneously. In every proband the filter pieces left the stomach at a faster rate than the liver, but showed a linear emptying pattern in each experiment. Based on these observations and on recent results of other authors, a new concept of the gastric emptying of solids is presented. Within the framework of this model the use of the filter pieces in clinical studies to simulate a solid meal appears to be justified.

Prevention of postoperative thromboembolism by dextran 40, low doses of heparin, or xantinol nicotinate.

The efficacy of dextran-40 infusions or low-dose heparin or xantinol-nicotinate administration in preventing postoperative thromboembolic complications has been investigated in a prospective, controlled, randomised trial as part of an international multicentre trial. 382 patients over the age of forty years undergoing elective major general surgery were investigated; 100 had a complete protocol in the control, 94 in the heparin, 92 in the dextran, and 32 in the xantinol-nicotinate group. 31 patients died: 13 in the control group, 10 in the heparin group, 6 in the dextran group, and 2 in the xantinol-nicotinate group. In 4 controls, 6 in the heparin group, and 1 in the dextran group the cause of death was acute pulmonary embolism. 4 further controls, 1 in the heparin group, and 2 in the dextran group had emboli found at necropsy, which either contributed to the deaths or were incidental findings. The distribution of pulmonary emboli in this study did not show the difference between the control and the heparin group reported in the multicentre trial. The 125I-fibrinogen test was done in all patients. The frequency of isotopic deep-vein thrombosis was 36-0% in the controls, 12-8% in the heparin group, 21-7% in the dextran group, and 40-6% in the xantinol-nicotinate group. The difference between the controls and the heparin groups was highly significant and between the control and the dextran group, probably significant. There were significantly more side-effects in the heparin group than in the dextran group.

[Retention and dosage in radiation synovectomy with yttrium-90-silicate colloid].

Investigations on patients and on phantoms were performed to determine the retention and the absorbed dose in radiation synovectomy with 90Y silicate colloid. The electrons of 90Y are totally absorbed within the surrounding tissue and produce bremsstrahlung with a maximum intensity at about 110 keV. After application of 6 mCi 90Y the dose rate of the bremsstrahlung was measured at a distance of 10 cm over 13 knees in a total of 10 patients and was found to range between 0.6 and 1.3 mR/h. During a period of 3 hours up to 3 days after injection practically no removal of the material from the joint was observed. According to approximative calculations the mean dose absorbed in the knee ranged between 3650 and 7300 rad, and the absorbed dose in the synovium between 5500 and 11000 rad.

[Radioprotection during clinical and laboratory utilization of tritium-labeled thymidine (author's transl)].

In view of the new prescriptions for radioprotection of the Helevetic Confederacy (Eidgenössische Strahlenschutzverordnung) the problems of radioprotection connected with utilization of the pure beta-ray emitter tritium are exposed, since the latter frequently is used as a marker substance in biomedical investigations. Inorganic tritum is regarded as the least toxic radionuclide. With tritiated thymidine, very often used for study of cell proliferation and cellular kinetics because of its incorporation into DNA, the radiation risk has to be considered quite differently. Its radiotoxicity is estimated to be superior by a factor up to thousand. The increased risk, thus resulting, is discussed with regard to the findings by animal experimentation and to the toxicity in man. Practical recommendations for the use of tritium-thymidine are given.

[Dextran 40 in the prophylaxis of deep vein thrombosis in surgery (author's transl)].

The effectiveness of 4 X 500 ml dextran-40 infusions (Rheomacrodex) for the prevention of postoperative deep vein thrombosis was evaluated in a prospective, controlled, randomized study in urological and general surgical patients. The 125I-fibrinogen test was performed daily in all patients. 36 of 100 control patients developed deep vein thrombosis, but only 20 out of 92 (21.7%) did so in the dextran group (P less than 0.05). The number of patients showing bilateral deep vein thrombosis was significantly reduced under dextran prophylaxis (P less than 0.01). No side effects of dextran prophylaxis were seen.

Actions of the uterine relaxant, fenoterol, on uteroplacental hemodynamics in human subjects.

The action of fenoterol, a uterine relaxant of the adrenergic beta-mimetic group of drugs, has been investigated on placental, myometrial, and cardiac blood pools. 113mIndium was injected intravenously and the blood pools were measured by recording gamma activity externally. In eight cases with oxytocin-induced labor, fenoterol inhibited the typical blood pool changes of labor, i.e., contraction-synchronized decrease of placental and myometrial blood pools with increase in cardiac blood pool. Furthermore, fenoterol produced an increase in placental and myometrial pools with corresponding decrease of cardiac blood pool. These changes were observed consistently. In four other cases, of which three had spontaneous labor, similar effects were observed. The possible mechanisms of hemodynamic action of fenoterol, with reference to uterine relaxation, are discussed.

[The course of postoperative deep vein thrombi as judged by repeated phlebographies].

Phlebography was carried out in a prospective randomised, controlled study during which dextran and small doses of s.c. heparin have been compared in general surgical and urological patients, in 31 persons with proven deep vein thrombois diagnosed with the 125I-fibrinogen test. In 16 out of the 31 patients a second phlebogram coule be performed two weeks later. In this study all thrombi occurred intraoperatively. In 24 of the 31 patients, the appearance of thrombi was multilocular. Independent of the prevention instituted and irrespective of the therapy, the following phenomena could be seen 14 days after the first phlebogram: 9 thrombi retracted, 6 disappeared completely and 1 occluded the vein completely. 2 thrombi disappeared without any therapy whatsoever, the 4 others disappeared in patients receiving heparin and/or coumarin for treatment.

[Trends in the nuclear-medical diagnosis of thrombosis].

Use of the 125I-fibrinogen-uptake test for detection of freshly developing thrombi is well known. The administration of radiofibrinogen to detect preformed thrombus - the retrospective variant of the 125I-fibrinogen-uptake test - allows the diagnosis of established thrombosis. The correlation with ascending phlebography is 90%. 125I-Fibrinogen cannot be used for scanning, due to its low energy, while 131I-fibrinogen if used involves a high radiation dose to the patient. First experiments with new radiopharmaceuticals for detection of deep vein thrombosis are proposed. Indication lists for the retrospective variant of the 125I-fibrinogen-uptake test are given.

[First experiences with a new oral human pancreas function test with special emphasis on gastric voiding].

1 g of N--benzoyl-L-tyrosyl-PABA was administered orally to 50 test persons, i.e. controls and patients with known pancreatic disease. In the presence of chymotrypsin, paraaminobenzoic acid (PABA) is split from the peptide and excreted in the urine. The amount of PABA excreted serves as parameter of exocrine pancreatic function. In the control group a mean of 59.2 +/- 6.2% of the peptide PABA was excreted over a period of 6 h. PABA excretion in exocrine pancreatic deficiency was significantly less (p less than 0.001) than in controls. No overlap of data was noted. Gastric emptying rate - estimated by using indium-113m microcolloid - did not appear to influence urinary PABA excretion.
