Special radiologic imaging of paranasal sinuses. A prerequisite for functional endoscopic sinus surgery.

Endoscopic diagnosis and surgery of inflammatory diseases of the lateral nasal wall and the paranasal sinuses require a very detailed preoperative knowledge of the individual anatomical conditions and pathological changes. Pluridirectional tomography and computed tomography are used best to visualize this region. The complicated radiologic anatomy of the lateral nasal wall together with the most important anatomical variations are demonstrated and the underlying pathophysiologic principles of chronic recurrent sinusitis are discussed.

[Functional assessment, using micturition cystourethrography, of the unstable bladder in childhood].

The accuracy and reliability of a modified method of micturating urethrocystography for the diagnosis of uncontrolled detrusor contraction in unstable bladders (sphincter-detrusor dyssynergy during bladder filling) was studied in 100 patients aged between four and 15 years. A comparison with a urodynamic method showed an accuracy of 92%, sensitivity of 93% and specificity of 90.7%. The technique and indications are described and the clinical relevance of the results is discussed.

Unstable bladder in children: functional evaluation by modified voiding cystourethrography.

A slightly modified technique for voiding cystourethrography was applied in 100 patients between 4 and 15 years of age and used to study uninhibited detrusor contractions in the so-called unstable bladder syndrome. Uninhibited detrusor contractions result in a slowing or cessation of the flow of contrast material into the bladder and a simultaneous minimal or marked opening of the bladder neck, with variable filling of the posterior urethra. Comparison of the radiologic findings with the results of urodynamic studies demonstrated an overall accuracy of 92%, sensitivity of 93%, and specificity of 90.7% for the radiologic technique.

[Roentgen symptoms of early cancer of the breast].

Based on 68 verified cases of early breast cancer, the radiologic signs in early breast cancer are analyzed. The relative incidence of the different mammographic signs and the diagnostic value of each when it occurs in isolation are elaborated. The diagnostic and therapeutic consequences are discussed.

Radiological aspects of aspergillosis in the paranasal sinuses.

A retrospective radiographic study was performed on 142 proven cases of Aspergillosis in the paranasal sinuses. In all but two cases the mycosis was unilateral and in all cases the maxillary sinus was infected. A common radiographic presentation was a homogeneous opacity of the infected maxillary sinus, while about 50% of the cases showed nonspecific infectious changes of other paranasal sinuses additionally. Earlier stages of the disease showed an intraluminal soft tissue mass, representing the mycelium conglomerate. Bone destruction due to Aspergillosis could not be proven. 58% of the cases presented with intraluminal structures of metallic density. Histochemical studies proved these to consist mainly of tertiary Calciumphosphate. These concrements are considered to be pathognomonic of Aspergillosis.

Correlation between manometric and roentgenologic findings of diseases of the esophagus in infants and children.

Roentgenologic and manometric findings in diseases of the esophagus in infants and children were compared. With both procedures combined it was possible to determine the exact localization of the lower esophageal sphincter. For the demonstration of gastroesophageal reflux the roentgenologic water siphon test was used. The excellent correspondence of the results of the water siphon test with manometric findings demonstrated the reliability of this roentgenologic method for detection of reflux. In borderline cases simultaneous manometric and roentgenologic studies were also of great use for demonstration of hiatus hernia.

[Metal-dense structures in the paranasal sinuses--a reliable indication of aspergillosis?].

The significance of mycotic concretions of 'metal density' in the diagnosis of aspergillosis of the paranasal sinuses has been analysed in 47 patients with this diagnosis. The incidence, types, radiological appearances and chemical composition of these very dense calcifications are discussed. Demonstration of these concretions, if metal foreign bodies and displaced dental fillings are excluded, are evidence for a mycosis of the paranasal sinuses. Absence of these dense structures does not exclude aspergillosis and was present in our clinical material in only 50% of cases.

Aspergillosis of the paranasal sinuses.

The authors examined 105 proved cases of aspergillosis involving the paranasal sinuses or nasal fossa. Mycosis was always unilateral, and the maxillary sinus was infected in all cases. Early stages were manifested by an intraluminal soft-tissue mass representing the mass of mycelia. Fifty-nine patients (56%) demonstrated very dense intraluminal calcifications. Such dense concretions suggest the diagnosis of aspergillosis.

[Ischemic necrosis of the vertebral body].

The article reports on the x-ray signs of ischaemic necrosis of vertebral bodies, basing on a case history. The aetiology of this rare condition is discussed and the relevant literature is reviewed.

Congenital bile duct dilatation (Caroli's disease--Grumbach's disease).

Congenital bile duct dilatation is an autosomal recessive inherited disease. Pathologically tortuous dilated, dysplastic intrahepatic bile ducts were found. It may be associated with hepatic fibrosis (Grumbach's disease), or without (Caroli's disease). A common additional finding is an infantile polycystic kidney. Three cases are reported and the radiological and sonographic findings are discussed.

[Involvement of diaphragmatic lymph nodes in Hodgkin's disease].

The authors report on a case of Hodgkin's disease with enlargement of diaphragmal lymph nodes. Detailed attention is paid to the anatomy, radiological diagnosis and differential diagnosis of these mediastinal lymph nodes. The clinical importance of this group of lymph nodes in Hodgkin's disease and in non-Hodgkin lymphomas is discussed.

[Metastatic involvement of the breast].

The mammographic spectrum of metastases in the breast is illustrated by 43 cases with histologically confirmed findings. Haematogenous metastases appear as sharply demarcated foci, lymphogenous metastases produce thickening of the skin and reticular thickening of the subcutis and of the glandular tissue. Metastases from systemic haematological disease may appear in nodular or diffuse form.

[A computer macro-programme for osteoscintimetry and activity profile in Perthe's disease (author's transl)].

A computer macro-programme has been developed which permits quantitative scintigraphy by means of an activity profile. The slope and width of the profile are variable; a view at right angles to the epiphyseal plate of the femoral head is able to determine activity in the plate and in the epiphysis. Measurements of the abnormal and normal hip produce uptake rates. These indicate to what extent the abnormal femoral head and epiphyseal plate differ from the normal side, with respect to their uptake. The method and results are discussed.

[Computer-aided diagnosis of bone tumours and tumour-like skeletal abnormalities in childhood (author's transl)].

The method, use and results of computer-aided diagnosis of bone tumours and tumour-like skeletal abnormalities in childhood are described. One hundred and twenty-one histologically verified cases were analysed and subjected to computer-aided diagnosis. Statistical methods were used, resulting in an accuracy of 96.7% and 91.7% respectively. In order to obtain additional confirmation of the diagnosis, a so-called growth rate was calculated by the computer. This proved to be a reliable criterion for evaluating the biological behaviour of tumours, irrespective of the histological diagnosis. The reasons for misdiagnosis were discussed. Attention is drawn to the importance of an accurate analysis of the radiographs and its input into the computer.

[5-year scintigraphic study of children with Perthes disease. Diagnostic evaluation and therapeutic consequences].

The value of bone scintigraphy in the diagnosis and follow-up of Perthes disease has been studied in 179 patients. Amongst 63 patients with Perthes' disease, there were 11 in whom the diagnosis could be made by scintigraphy before there were radiological signs. This enables early treatment to be carried out and leads to a shorter course of the disease and improved end-results. Scintigraphy alone makes it possible to judge the actual state of revitalization of the necrotic femoral head. From this, it is possible to derive the indications for a displacement osteotomy before the radiological signs of a "head at risk" appear.

Conventional tomography and computed tomography-are both methods necessary for evaluation of facial lesions?

108 patients with verified traumatic, inflammatory and neoplastic lesions in the facial region and the pharyngeal space were evaluated both with conventional tomography and CT. Subsequently an analysis was made as to whether both methods were complementary or at least partially replaceable by each other. In traumatic lesions transverse axial CT combined with frontal tomography yielded optimal information. For the diagnosis of inflammatory and neoplastic lesions CT was superior to tomography with the exception of lateral tomograms which contributed additional information to diseases of the sphenoid sinus, the epipharynx, the pterygopalatine fossa and the orbital roof. The radiation dose to the eye is remarkably lower in CT than in tomography.
