[CT cisternography in the diagnosis of nasal cerebrospinal fluid fistulas].

Four out of 12 patients with CT-cisternography were operated on. The CT findings were all confirmed at operation. In three patients with positive CT findings, the fistula was located at operation. In one case with negative CT findings, operation revealed no fistula either. Advantages of CT-cisternography are reliability of positive findings and the possibility of locating the fistula in some cases. The procedure bears relatively little risk and can be repeated.

DNA pattern in choroid plexus papillomas.

Report on Feulgen-cytophotometric DNA studies of 8 choroid plexus papillomas. 7 tumours reveal distinct diploid stemlines in their karyogram. One tumour with oncocytic transformation exhibits a stemline in the triploid stage. These findings reflect the DNA pattern of a benign euploid neoplasm of the there investigated tumours.

Supratentorial lobar pilocytic astrocytomas: report of 45 operated cases, including 9 recurrences.

45 cases with supratentorial, lobar--mainly temporo-parietal--pilocytic astrocytoma are analysed. The mostly highly vascularized tumours frequently present with histologically different tissue components. Formation of cysts of various size was found in 67%. The tumour usually ran a benign clinical course with a comparatively low incidence of recurrence. Malignant transformation of recurrent tumours was not observed. Radiotherapy of conventional dosage did not prove to have any influence on survival, neither post-operatively nor after re-operation. Transformation of persisting pluripotential cells of the subependymal periventricular plate after migration to the surface of the brain is being discussed as the origin of this variant of pilocytic astrocytoma.

Hemangiopericytoma of meninges. I. Histopathological variability and differential diagnosis.

A report on morphological variability of meningeal hemangiopericytomas (HP) is presented. 17 cases of HP (12 with intracranial, 5 with spinal localisation) were examined histologically. In 7 cases the tumor recurred, in 2 repeatedly. Metastases were found in 3 cases. As with peripheral HP, a surprising histological diversity is found with meningeal HP. Differential diagnosis can be difficult: Variants of meningioma, fibrous histiocytoma, paraganglioma and other rare richly vascular tumors have to be considered. Although hitherto little known, some additional signs as mucoid change and focal or diffuse fibrosis are helpful for diagnosis. HP of the meninges is considered a well defined entity apart from meningiomas.

Hemangiopericytoma of meninges. II. General and clinical data.

Meningeal hemangiopericytoma (HP) has mostly been viewed as a variant of meningioma. There is more and more evidence, however, that HP constitutes a separate entity among tumors of the central nervous system. The data of intracranial and spinal HP of 17 of our patients and from a review of the literature is compared to meningioma and to peripheral HP. Meningeal and peripheral HP present with a similar sex ratio, age distribution, tendency to metastasize and are almost never multiple. This is fundamentally different from meningioma. Other data of meningeal HP like interval between first operation and manifestation of a recurrence or metastasis and survival time cannot be compared to peripheral HP due to its special location. Prognosis of meningeal HP is distinctly worse than of meningioma, placing HP between malignancy grade I and II as specified by WHO.

Acute spinal epidural empyema. Observations from seven cases.

Seven cases with an acute epidural spinal empyema are presented. The best results are observed in cases with early treatment. Since this rare condition is not widely known and swift diagnosis and treatment is imperative, it is stressed that immediate referral of the patient with back pain and any signs of infection in addition to commencing neurological abnormalities to a neurological unit for myelography can be vital or can at least reduce the risk of permanent paraplegia.

Elective percutaneous dilatational tracheostomy. A new simple bedside procedure; preliminary report.

The development by the senior author (P.C.) of percutaneous tracheostomy from cricothyroidostomy and subcricoid fingertip tracheostomy is traced, and the technique and patient material of percutaneous subcricoid tracheostomy is presented. This new technique consists of inserting a tracheostomy tube by the use of a J guide wire inserted through a cannula into the tracheal lumen. Tapered dilators follow the guide wire and dilate the opening in the tracheal walls. A tracheostomy tube snugly fitted over a dilator is then passed into the trachea between the cricoid cartilage and the first tracheal ring. This procedure avoids the immediate and postoperative complications of "standard" tracheostomy. An experience of 134 tracheostomies of various types culminated in the development of the percutaneous technique. To date 26 such operations on 24 patients have been done with no significant complications due to the operation. The percutaneous technique should reduce the severity and incidence of intraoperative complications. Late complications, which have been no problem to date, are being evaluated with longer follow-up and with a greater patient population.

[In vitro susceptibility of bacteroidaceae to cefoxitin and cephalothin (author's transl)].

The activity of the new cephamycin C derivatives cefoxitin against 102 clinical isolates of Bacteroidaceae (95 Bacteroides strains and 7 Fusobacterium-Sphaerophorus cultures) was determined by tube dilution and standardized agar diffusion tests and compared to that of cephalothin. Cefoxitin was more active than cephalothin against 56 Bacteroides fragilis strains, 19 Bacteroides thetaiota-omicron strains and two Bacteroides splanchnicus strains. The difference seemed less pronounced with five Bacteroides vulgatus and four Bacteroides variabilis cultures. Three Bacteroides melaninogenicus and six Bacteroides oralis strains displayed similar sensitivity to both cephalosporins. At concentrations of 32 microng/ml or less, cefoxitin inhibited 91% and cephalothin 33% of the Bacteroides strains. Two strains of Sphaerophorus varius were likewise much more sensitive to cefoxitin than to cephalothin (MIC values 8 micron8/ml of cefoxitin and 256 microng/ml of cephalothin). Three other Sphaerophorus strains (the species necrophorus and freundii/mortiferum) and two of Fusobacterium fusiforme (Fusobacterium nucleatum) strains were very similar in their susceptibility to both cephalosporins. Statistical analysis of the relationship between MIC and zone size diameter showed relatively high correlation coefficients for cefoxitin and cephalothin (r=-0.741 and r=-0.703 respectively).

Pyramidal tract function during onset of brain death.

In 51 patients with primary brain lesions, who fulfilled the criteria of brain death, sequential recording of transcranial magnetic evoked potentials (TMEPs) and somatosensory evoked potentials (SEPs) were performed. In all comatose patients with apnoeic cranial nerve areflexia the TMEP could not be elicited, while the response after cervical magnetic stimulation was always preserved. Similarly, no cortical SEPs were preserved in apnoeic cranial nerve areflexia; however, the cervical somatosensory response was preserved in 44%. In deteriorating patients with coma grade III TMEPs were preserved in 3 instances, while cortical SEPs were already absent. Current brain death criteria, however, were not challenged, as TMEPs were absent in all 51 patients, at the latest when apnoea was noted.

Artifacts in MR imaging after surgical intervention.

Artifacts are encountered repeatedly in magnetic resonance (MR) imaging after neurological orthopedic surgery, although there is no evidence of metallic foreign bodies on radiography and CT. Experiments demonstrated that surgical filling materials, such as bone cement and bone transplants are not the cause of the artifacts. On the other hand, we have been able to show that short-term contact between a diamond drill and untempered operating instruments can cause abrasions and set free minute metallic fragments. Those particles are missed by X-ray procedures including CT but cause MR artifacts by local disturbance of the homogeneity of the magnetic field. Therefore, MR represents a highly sensitive method to trace extremely small amounts of magnetic substances. Postsurgical MR, however, may be of limited value because of the disturbing artifacts.

Age-related calcium/magnesium ratio in the ligamenta flava.

The Ca/Mg ratio in fibrous connective tissue may be related to its elastic property. Ligamenta flava were obtained from 76 patients during lumbar disc surgery, and Ca and Mg levels were determined by atomic absorption spectrometry. The Ca/Mg ratio was almost constant throughout all age groups. It is concluded that the constancy of the Ca/Mg ratio throughout the lifetime is an integral quality of elastic fibers which may be explained by the biological long half-life of elastin.
